target/riscv: Fix VS mode interrupts forwarding.
Currently riscv_cpu_local_irq_pending is used to find out pending
interrupt and VS mode interrupts are being shifted to represent
S mode interrupts in this function. So when the cause returned by
this function is passed to riscv_cpu_do_interrupt to actually
forward the interrupt, the VS mode forwarding check does not work
as intended and interrupt is actually forwarded to hypervisor. This
patch fixes this issue.

target/riscv: Correctly implement TSR trap
As reported in: https://bugs.launchpad.net/qemu/+bug/1851939 we weren't
correctly handling illegal instructions based on the value of MSTATUS_TSR
and the current privledge level.

This patch fixes the issue raised in the bug by raising an illegal
instruction if TSR is set and we are in S-Mode.

RISC-V: Add a missing "," in riscv_excp_names
This would almost certainly cause the exception names to be reported
incorrectly.  Coverity found the issue (CID 1420223).  As per Peter's
suggestion, I've also added a comma at the end of the list to avoid the issue
reappearing in the future.

target/riscv: Emulate TIME CSRs for privileged mode
Currently, TIME CSRs are emulated only for user-only mode. This
patch add TIME CSRs emulation for privileged mode.

For privileged mode, the TIME CSRs will return value provided
by rdtime callback which is registered by QEMU machine/platform
emulation (i.e. CLINT emulation). If rdtime callback is not
available then the monitor (i.e. OpenSBI) will trap-n-emulate
TIME CSRs in software.

We see 25+% performance improvement in hackbench numbers when
TIME CSRs are not trap-n-emulated.

target/riscv: Respect MPRV and SPRV for floating point ops
mark_fs_dirty() is the only place in translate.c that uses the
virt_enabled bool. Let's respect the contents of MSTATUS.MPRV and
HSTATUS.SPRV when setting the bool as this is used for performing
floating point operations when V=0.

target/ricsv: Flush the TLB on virtulisation mode changes
To ensure our TLB isn't out-of-date we flush it on all virt mode
changes. Unlike priv mode this isn't saved in the mmu_idx as all
guests share V=1. The easiest option is just to flush on all changes.

target/riscv: Add the force HS exception mode
Add a FORCE_HS_EXCEP mode to the RISC-V virtulisation status. This bit
specifies if an exeption should be taken to HS mode no matter the
current delegation status. This is used when an exeption must be taken
to HS mode, such as when handling interrupts.

The MIP CSR is a xlen CSR, it was only 32-bits to allow atomic access.
Now that we don't use atomics for MIP we can change this back to a xlen
CSR.

25139bf7f8 target/riscv: progressively load the instruction during decode
The plugin system would throw up a harmless warning when it detected
that a disassembly of an instruction didn't use all it's bytes. Fix
the riscv decoder to only load the instruction bytes it needs as it
needs them.

This drops opcode from the ctx in favour if passing the appropriately
sized opcode down a few levels of the decode.

riscv: Separate FPU register size from core register size in gdbstub [v2]
The size of the FPU registers is dictated by the 'f' and 'd' features,
not the core processor register size. Processors with the 'd' feature
have 64-bit FPU registers. Processors without the 'd' feature but with
the 'f' feature have 32-bit FPU registers.

ba2ed84fe6 RISC-V Patches for the 5.0 Soft Freeze, Part 1
This patch set contains a handful of collected fixes that I'd like to target
 for the 5.0 soft freeze (I know that's a long way away, I just don't know what
 else to call these):
 
 * A fix for a memory leak initializing the sifive_u board.
 * Fixes to privilege mode emulation related to interrupts and fstatus.
 
 Notably absent is the H extension implementation.  That's pretty much reviewed,
 but not quite ready to go yet and I didn't want to hold back these important
 fixes.  This boots 32-bit and 64-bit Linux (buildroot this time, just for fun)
 and passes "make check".

RISC-V Patches for the 5.0 Soft Freeze, Part 1

This patch set contains a handful of collected fixes that I'd like to target
for the 5.0 soft freeze (I know that's a long way away, I just don't know what
else to call these):

* A fix for a memory leak initializing the sifive_u board.
* Fixes to privilege mode emulation related to interrupts and fstatus.

Notably absent is the H extension implementation.  That's pretty much reviewed,
but not quite ready to go yet and I didn't want to hold back these important
fixes.  This boots 32-bit and 64-bit Linux (buildroot this time, just for fun)
and passes "make check".

target/riscv: Fix tb->flags FS status
It was found that running libquantum on riscv-linux qemu produced an
incorrect result. After investigation, FP registers are not saved
during context switch due to incorrect mstatus.FS.

In current implementation tb->flags merges all non-disabled state to
dirty. This means the code in mark_fs_dirty in translate.c that
handles initial and clean states is unreachable.

This patch fixes it and is successfully tested with:
  libquantum

Thanks to Richard for pointing out the actual bug.

target/riscv: PMP violation due to wrong size parameter
riscv_cpu_tlb_fill() uses the `size` parameter to check PMP violation
using pmp_hart_has_privs().
However, if the size is unknown (=0), the ending address will be
`addr - 1` as it is `addr + size - 1` in `pmp_hart_has_privs()`.
This always causes a false PMP violation on the starting address of the
range, as `addr - 1` is not in the range.

In order to fix, we just assume that all bytes from addr to the end of
the page will be accessed if the size is unknown.

linux-user/riscv: Propagate fault address
The CPU loop tagged all the queued signals as QEMU_SI_KILL while it was
filling the `_sigfault` part of `siginfo`: this caused QEMU to copy the
wrong fields over to the userspace program.

Make sure the fault address recorded by the MMU is is stored in the CPU
environment structure.

In case of memory faults store the exception address into `siginfo`.

1.11 defines mcountinhibit, which has the same numeric CSR value as
mucounteren from 1.09.1 but has different semantics.  This patch enables

RISC-V: Fix a PMP bug where it succeeds even if PMP entry is off
The current implementation returns 1 (PMP check success) if the address is in
range even if the PMP entry is off. This is a bug.

For example, if there is a PMP check in S-Mode which is in range, but its PMP
entry is off, this will succeed, which it should not.

The patch fixes this bug by only checking the PMP permissions if the address is
in range and its corresponding PMP entry it not off. Otherwise, it will keep
the ret = -1 which will be checked and handled correctly at the end of the
function.

RISC-V: Check for the effective memory privilege mode during PMP checks
The current PMP check function checks for env->priv which is not the effective
memory privilege mode.

For example, mstatus.MPRV could be set while executing in M-Mode, and in that
case the privilege mode for the PMP check should be S-Mode rather than M-Mode
(in env->priv) if mstatus.MPP == PRV_S.

This patch passes the effective memory privilege mode to the PMP check.
Functions that call the PMP check should pass the correct memory privilege mode
after reading mstatus' MPRV/MPP or hstatus.SPRV (if Hypervisor mode exists).

RISC-V: Raise access fault exceptions on PMP violations
Section 3.6 in RISC-V v1.10 privilege specification states that PMP violations
report "access exceptions." The current PMP implementation has
a bug which wrongly reports "page exceptions" on PMP violations.

RISC-V: Only Check PMP if MMU translation succeeds
The current implementation unnecessarily checks for PMP even if MMU translation
failed. This may trigger a wrong PMP access exception instead of
a page exception.

For example, the very first instruction fetched after the first satp write in
S-Mode will trigger a PMP access fault instead of an instruction fetch page
fault.

This patch prioritises MMU exceptions over PMP exceptions and only checks for
PMP if MMU translation succeeds. This patch is required for future commits
that properly report PMP exception violations if PTW succeeds.

A wrong address is passed to `pmp_is_in_range` while checking if a
memory access is within a PMP range.
Since the ending address of the pmp range (i.e., pmp_state.addr[i].ea)
is set to the last address in the range (i.e., pmp base + pmp size - 1),
memory accesses containg the last address in the range will always fail.

For example, assume that a PMP range is 4KB from 0x87654000 such that
the last address within the range is 0x87654fff.
1-byte access to 0x87654fff should be considered to be fully inside the
PMP range.
However the access now fails and complains partial inclusion because
pmp_is_in_range(env, i, addr + size) returns 0 whereas
pmp_is_in_range(env, i, addr) returns 1.

target/riscv: More accurate handling of sip CSR
According to the spec, "All bits besides SSIP, USIP, and UEIP in the sip
register are read-only." Further, if an interrupt is not delegated to mode x,
then "the corresponding bits in xip [...] should appear to be hardwired to
zero. This patch implements both of those requirements.

RISC-V: fix single stepping over ret and other branching instructions
This patch introduces wrappers around the tcg_gen_exit_tb() and
tcg_gen_lookup_and_goto_ptr() functions that handle single stepping,
i.e. call gen_exception_debug() when single stepping is enabled.

Theses functions are then used instead of the originals, bringing single
stepping handling in places where it was previously ignored such as jalr
and system branch instructions (ecall, mret, sret, etc.).

90c84c5600 qom/cpu: Simplify how CPUClass:cpu_dump_state() prints
CPUClass method dump_statistics() takes an fprintf()-like callback and
a FILE * to pass to it.  Most callers pass fprintf() and stderr.
log_cpu_state() passes fprintf() and qemu_log_file.
hmp_info_registers() passes monitor_fprintf() and the current monitor
cast to FILE *.  monitor_fprintf() casts it right back, and is
otherwise identical to monitor_printf().

The callback gets passed around a lot, which is tiresome.  The
type-punning around monitor_fprintf() is ugly.

Drop the callback, and call qemu_fprintf() instead.  Also gets rid of
the type-punning, since qemu_fprintf() takes NULL instead of the
current monitor cast to FILE *.

The various TARGET_cpu_list() take an fprintf()-like callback and a
FILE * to pass to it.  Their callers (vl.c's main() via list_cpus(),
bsd-user/main.c's main(), linux-user/main.c's main()) all pass
fprintf() and stdout.  Thus, the flexibility provided by the (rather
tiresome) indirection isn't actually used.

Drop the callback, and call qemu_printf() instead.

Calling printf() would also work, but would make the code unsuitable
for monitor context without making it simpler.

target/riscv: Zero extend the inputs of divuw and remuw
While running the GCC test suite against 4.0.0-rc0, Kito found a
regression introduced by the decodetree conversion that caused divuw and
remuw to sign-extend their inputs.  The ISA manual says they are
supposed to be zero extended:

    DIVW and DIVUW instructions are only valid for RV64, and divide the
    lower 32 bits of rs1 by the lower 32 bits of rs2, treating them as
    signed and unsigned integers respectively, placing the 32-bit
    quotient in rd, sign-extended to 64 bits. REMW and REMUW
    instructions are only valid for RV64, and provide the corresponding
    signed and unsigned remainder operations respectively.  Both REMW
    and REMUW always sign-extend the 32-bit result to 64 bits, including
    on a divide by zero.

Here's Kito's reduced test case from the GCC test suite

RISC-V: Add support for vectored interrupts
If vectored interrupts are enabled (bits[1:0]
of mtvec/stvec == 1) then use the following
logic for trap entry address calculation:

 pc = mtvec + cause * 4

In addition to adding support for vectored interrupts
this patch simplifies the interrupt delivery logic
by making sync/async cause decoding and encoding
steps distinct.

The cause code and the sign bit indicating sync/async
is split at the beginning of the function and fixed
cause is renamed to cause. The MSB setting for async
traps is delayed until setting mcause/scause to allow
redundant variables to be eliminated. Some variables
are renamed for conciseness and moved so that decls
are at the start of the block.

RISC-V: Allow interrupt controllers to claim interrupts
We can't allow the supervisor to control SEIP as this would allow the
supervisor to clear a pending external interrupt which will result in
lost a interrupt in the case a PLIC is attached. The SEIP bit must be
hardware controlled when a PLIC is attached.

This logic was previously hard-coded so SEIP was always masked even
if no PLIC was attached. This patch adds riscv_cpu_claim_interrupts
so that the PLIC can register control of SEIP. In the case of models
without a PLIC (spike), the SEIP bit remains software controlled.

This interface allows for hardware control of supervisor timer and
software interrupts by other interrupt controller models.

RISC-V: Add hooks to use the gdb xml files.
The gdb CSR xml file has registers in documentation order, not numerical
order, so we need a table to map the register numbers.  This also adds
fairly standard gdb hooks to access xml specified registers.

    The fpu xml from gdb 8.3 has unused register #, 65 and make first
    csr register # become 69. We register extra register on gdb to correct
    csr offset calculation

during the refactor to decodetree we removed the manual decoding that is
necessary for c.jal/c.addiw and removed the translation of c.flw/c.ld
and c.fsw/c.sd. This reintroduces the manual parsing and the

RISC-V: Add misa runtime write support
This patch adds support for writing misa. misa is validated based
on rules in the ISA specification. 'E' is mutually exclusive with
all other extensions. 'D' depends on 'F' so 'D' bit is dropped
if 'F' is not present. A conservative approach to consistency is
taken by flushing the translation cache on misa writes. misa_mask
is added to the CPU struct to store the original set of extensions.

RISC-V: Implement existential predicates for CSRs
CSR predicate functions are added to the CSR table.
mstatus.FS and counter enable checks are moved
to predicate functions and two new predicates are
added to check misa.S for s* CSRs and a new PMP
CPU feature for pmp* CSRs.

Processors that don't implement S-mode will trap
on access to s* CSRs and processors that don't
implement PMP will trap on accesses to pmp* CSRs.

PMP checks are disabled in riscv_cpu_handle_mmu_fault
when the PMP CPU feature is not present.

RISC-V: Implement modular CSR helper interface
Previous CSR code uses csr_read_helper and csr_write_helper
to update CSR registers however this interface prevents
atomic read/modify/write CSR operations; in addition
there is no trap-free method to access to CSRs due
to the monolithic CSR functions call longjmp.

The current iCSR interface is not safe to be called by
target/riscv/gdbstub.c as privilege checks or missing CSRs
may call longjmp to generate exceptions. It needs to
indicate existence so traps can be generated in the
CSR instruction helpers.

This commit moves CSR access from the monolithic switch
statements in target/riscv/op_helper.c into modular
read/write functions in target/riscv/csr.c using a new
function pointer table for dispatch (which can later
be used to allow CPUs to hook up model specific CSRs).

A read/modify/write interface is added to support atomic
CSR operations and a non-trapping interface is added
to allow exception-free access to CSRs by the debugger.

The CSR functions and CSR dispatch table are ordered
to match The RISC-V Instruction Set Manual, Volume II:
Privileged Architecture Version 1.10, 2.2 CSR Listing.

An API is added to allow derived cpu instances to modify
or implement new CSR operations.

target/riscv/pmp.c: Fix pmp_decode_napot()
Currently, start and end address of a PMP region are not decoded
correctly by pmp_decode_napot().

Let's say we have a 128KB PMP region with base address as 0x80000000.
Now, the PMPADDRx CSR value for this region will be 0x20003fff.

The current pmp_decode_napot() implementation will decode PMPADDRx
CSR as t1=14, base=0x100000000, and range=0x1ffff whereas it should
have decoded PMPADDRx CSR as t1=14, base=0x80000000, and range=0x1fff.

This patch fixes the base value decoding in pmp_decode_napot() when
PMPADDRx CSR is not -1 (i.e. 0xffffffffffffffff).

RISC-V: Respect fences for user-only emulators
Our current fence implementation ignores fences for the user-only
configurations.  This is incorrect but unlikely to manifest: it requires
multi-threaded user-only code that takes advantage of the weakness in
the host's memory model and can be inlined by TCG.

This patch simply treats fences the same way for all our emulators.
I've given it to testing as I don't want to construct a test that would
actually trigger the failure.

Our fence implementation has an additional deficiency where we map all
RISC-V fences to full fences.  Now that we have a formal memory model
for RISC-V we can start to take advantage of the strength bits on our
fence instructions.  This requires a bit more though, so I'm going to
split it out because the implementation is still correct without taking
advantage of these weaker fences.

target/riscv/pmp.c: pmpcfg_csr_read returns bogus value on RV64
pmp_read_cfg() returns 8-bit value, which is combined together to form a single pmpcfg CSR.
The default promotion rules will result in an integer here ("i*8" is integer, which
flows through) resulting in a 32-bit signed value on most hosts.
That's bogus on RV64I, with the high bits of the CSR being wrong.

As part of plumbing MemTxAttrs down to the IOMMU translate method,
add MemTxAttrs as an argument to address_space_translate()
and address_space_translate_cached(). Callers either have an
attrs value to hand, or don't care and can use MEMTXATTRS_UNSPECIFIED.

RISC-V: Make mtvec/stvec ignore vectored traps
Vectored traps for asynchrounous interrupts are optional.
The mtvec/stvec mode field is WARL and hence does not trap
if an illegal value is written. Illegal values are ignored.

Later we can add RISCV_FEATURE_VECTORED_TRAPS however
until then the correct behavior for WARL (Write Any, Read
Legal) fields is to drop writes to unsupported bits.

RISC-V: Add mcycle/minstret support for -icount auto
Previously the mycycle/minstret CSRs and rdcycle/rdinstret
psuedo instructions would return the time as a proxy for an
increasing instruction counter in the absence of having a
precise instruction count. If QEMU is invoked with -icount,
the mcycle/minstret CSRs and rdcycle/rdinstret psuedo
instructions will return the instruction count.

RISC-V: Use [ms]counteren CSRs when priv ISA >= v1.10
Privileged ISA v1.9.1 defines mscounteren and mucounteren:

* mscounteren contains a mask of counters available to S-mode
* mucounteren contains a mask of counters available to U-mode

Privileged ISA v1.10 defines mcounteren and scounteren:

* mcounteren contains a mask of counters available to S-mode
* scounteren contains a mask of counters available to U-mode

mcounteren and scounteren CSR registers were implemented
however they were not honoured for counter accesses when
the privilege ISA was >= v1.10. This fix solves the issue
by coalescing the counter enable registers. In addition
the code now  generates illegal instruction exceptions
for accesses to the counter enabled registers depending
on the privileged ISA version.

- Coalesce mscounteren and mcounteren into one variable
- Coalesce mucounteren and scounteren into one variable
- Makes mcounteren and scounteren CSR accesses generate
  illegal instructions when the privileged ISA <= v1.9.1
- Makes mscounteren and mucounteren CSR accesses generate
  illegal instructions when the privileged ISA >= v1.10

RISC-V: Allow S-mode mxr access when priv ISA >= v1.10
The mstatus.MXR alias in sstatus should only be writable
by S-mode if the privileged ISA version >= v1.10. Also MXR
was masked in sstatus CSR read but not sstatus CSR writes.
Now we correctly mask sstatus.mxr in both read and write.

mtval/stval must be set on all exceptions but zero is
a legal value if there is no exception specific info.
Placing the instruction bytes for illegal instruction

RISC-V: Hardwire satp to 0 for no-mmu case
satp is WARL so it should not trap on illegal writes, rather
it can be hardwired to zero and silently ignore illegal writes.

It seems the RISC-V WARL behaviour is preferred to having to
trap overhead versus simply reading back the value and checking
if the write took (saves hundreds of cycles and more complex
trap handling code).

RISC-V: Update E and I extension order
Section 22.8 Subset Naming Convention of the RISC-V ISA Specification
defines the canonical order for extensions in the ISA string. It is
silent on the position of the E extension however E is a substitute
for I so it must come early in the extension list order. A comment
is added to state E and I are mutually exclusive, as the E extension
will be added to the RISC-V port in the future.

RISC-V: Workaround for critical mstatus.FS bug
This change is a workaround for a bug where mstatus.FS
is not correctly reporting dirty after operations that
modify floating point registers. This a critical bug
or RISC-V in QEMU as it results in floating point
register file corruption when running SMP Linux due to
task migration and possibly uniprocessor Linux if
more than one process is using the FPU.

This workaround will return dirty if mstatus.FS is
switched from off to initial or clean. According to
the specification it is legal for an implementation
to return only off, or dirty.

RISC-V Build Infrastructure
This adds RISC-V into the build system enabling the following targets:

- riscv32-softmmu
- riscv64-softmmu
- riscv32-linux-user
- riscv64-linux-user

This adds defaults configs for RISC-V, enables the build for the RISC-V
CPU core, hardware, and Linux User Emulation. The 'qemu-binfmt-conf.sh'
script is updated to add the RISC-V ELF magic.

RISC-V Physical Memory Protection
Implements the physical memory protection extension as specified in
Privileged ISA Version 1.10.

PMP (Physical Memory Protection) is as-of-yet unused and needs testing.
The SiFive verification team have PMP test cases that will be run.

Nothing currently depends on PMP support. It would be preferable to keep
the code in-tree for folk that are interested in RISC-V PMP support.
