* Set the memory type for the "BIOS" and "DMA" areas to write-back. Not sure, if
that's correct, but that's what was effectively used on my machines before.
* Changed x86_set_mtrrs() and the CPU module hook to also set the default memory
type.
* Rewrote the MTRR computation once more:
- Now we know all used memory ranges, so we are free to extend used ranges
into unused ones in order to simplify them for MTRR setup.
- Leverage the subtractive properties of uncached and write-through ranges to
simplify ranges of any other respectively write-back type.
- Set the default memory type to write-back, so we don't need MTRRs for the
RAM ranges.
- If a new range intersects with an existing one, we no longer just fail.
Instead we use the strictest requirements implied by the ranges. This fixes
#5383.
Overall the new algorithm should be sufficient with far less MTRRs than before
(on my desktop machine 4 are used at maximum, while 8 didn't quite suffice
before). A drawback of the current implementation is that it doesn't deal with
the case of running out of MTRRs at all, which might result in some ranges
having weaker caching/memory ordering properties than requested.

* Added vm_clear_page_mapping_accessed_flags() and
vm_remove_all_page_mappings_if_unaccessed(), which combine the functionality
of vm_test_map_activation(), vm_clear_map_flags(), and
vm_remove_all_page_mappings(), thus saving lots of calls to translation map
methods. The backend is the new method
VMTranslationMap::ClearAccessedAndModified().
* Started to make use of the cached page queue and changed the meaning of the
other non-free queues slightly:
- Active queue: Contains mapped pages that have been used recently.
- Inactive queue: Contains mapped pages that have not been used recently. Also
contains unmapped temporary pages.
- Modified queue: Contains unmapped modified pages.
- Cached queue: Contains unmapped unmodified pages (LRU sorted).
Unless we're actually low on memory and actively do paging, modified and
cached queues only contain non-temporary pages. Cached pages are considered
quasi free. They still belong to a cache, but since they are unmodified and
unmapped, they can be freed immediately. And this is what
vm_page_[try_]reserve_pages() do now when there are no more actually free
pages at hand. Essentially this means that pages storing cached file data,
unless mmap()ped, no longer are considered used and don't contribute to page
pressure. Paging will not happen as long there are enough free + cached pages
available.
* Reimplemented the page daemon. It no longer scans all pages, but instead works
the page queues. As long as the free pages situation is harmless, it only
iterates through the active queue and deactivates pages that have not been
used recently. When paging occurs it additionally scans the inactive queue and
frees pages that have not been used recently.
* Changed the page reservation/allocation interface:
vm_page_[try_]reserve_pages(), vm_page_unreserve_pages(), and
vm_page_allocate_page() now take a vm_page_reservation structure pointer.
The reservation functions initialize the structure -- currently consisting
only of a count member for the number of still reserved pages.
vm_page_allocate_page() decrements the count and vm_page_unreserve_pages()
unreserves the remaining pages (if any). Advantages are that reservation/
unreservation mismatches cannot occur anymore, that vm_page_allocate_page()
can verify that the caller has indeed a reserved page left, and that there's
no unnecessary pressure on the free page pool anymore. The only disadvantage
is that the vm_page_reservation object needs to be passed around a bit.
* Reworked the page reservation implementation:
- Got rid of sSystemReservedPages and sPageDeficit. Instead
sUnreservedFreePages now actually contains the number of free pages that
have not yet been reserved (it cannot become negative anymore) and the new
sUnsatisfiedPageReservations contains the number of pages that are still
needed for reservation.
- Threads waiting for reservations do now add themselves to a waiter queue,
which is ordered by descending priority (VM priority and thread priority).
High priority waiters are served first when pages become available.
Fixes#5328.
* cache_prefetch_vnode(): Would reserve one less page than allocated later, if

memory and page reservation functions have a new "priority" parameter that
indicates how deep the function may tap into that reserve. The currently
existing priority levels are "user", "system", and "VIP". The idea is that
user programs should never be able to cause a state that gets the kernel into
trouble due to heavy battling for memory. The "VIP" level (not really used
yet) is intended for allocations that are required to free memory eventually
(in the page writer). More levels are thinkable in the future, like "user real
time" or "user system server".
* Added "priority" parameters to several VMCache methods.
* Replaced the map_backing_store() "unmapAddressRange" parameter by a "flags"
parameter.
* Added area creation flag CREATE_AREA_PRIORITY_VIP and slab allocator flag
CACHE_PRIORITY_VIP indicating the importance of the request.
* Changed most code to pass the right priorities/flags.
These changes already significantly improve the behavior in low memory
situations. I've tested a bit with 64 MB (virtual) RAM and, while not
particularly fast and responsive, the system remains at least usable under high
memory pressure.
As a side effect the slab allocator can now be used as general memory allocator.
Not done by default yet, though.

object caches:
* Implemented CACHE_ALIGN_ON_SIZE. It is no longer set for all small object
caches, but the block allocator sets it on all power of two size caches.
* object_cache_reserve_internal(): Detect recursion and don't wait in such a
case. The function could deadlock itself, since
HashedObjectCache::CreateSlab() does allocate memory, thus potentially
reentering.
* object_cache_low_memory():
- I missed some returns when reworking that one in r35254, so the function
might stop early and also leave the cache in maintenance mode, which would
cause it to be ignored by object cache resizer and low memory handler from
that point on.
- Since ReturnSlab() potentially unlocks, the conditions weren't quite correct
and too many slabs could be freed.
- Simplified things a bit.
* object_cache_alloc(): Since object_cache_reserve_internal() does potentially
unlock the cache, the situation might have changed and their might not be an
empty slab available, but a partial one. The function would crash.
* Renamed the object cache tracing variable to SLAB_OBJECT_CACHE_TRACING.
* Renamed debugger command "cache_info" to "slab_cache" to avoid confusion with
the VMCache commands.
* ObjectCache::usage was not maintained anymore since I introduced the
MemoryManager. object_cache_get_usage() would thus always return 0 and the
block cache would not be considered cached memory. This was only of
informational relevance, though.
slab allocator misc.:
* Disable the object depots of block allocator caches for object sizes > 2 KB.
Allocations of those sizes aren't so common that the object depots yield any
benefit.
* The slab allocator is now fully self-sufficient. It allocates its bootstrap
memory from the MemoryManager, and the hash tables for HashedObjectCaches use
the block allocator instead of the heap, now.
* Added option to use the slab allocator for malloc() and friends
(USE_SLAB_ALLOCATOR_FOR_MALLOC). Currently disabled. Works in principle and
has virtually no lock contention. Handling for low memory situations is yet
missing, though.
* Improved the output of some debugger commands.

* Implemented a more elaborated raw memory allocation backend (MemoryManager).
We allocate 8 MB areas whose pages we allocate and map when needed. An area is
divided into equally-sized chunks which form the basic units of allocation. We
have areas with three possible chunk sizes (small, medium, large), which is
basically what the ObjectCache implementations were using anyway.
* Added "uint32 flags" parameter to several of the slab allocator's object
cache and object depot functions. E.g. object_depot_store() potentially wants
to allocate memory for a magazine. But also in pure freeing functions it
might eventually become useful to have those flags, since they could end up
deleting an area, which might not be allowable in all situations. We should
introduce specific flags to indicate that.
* Reworked the block allocator. Since the MemoryManager allocates block-aligned
areas, maintains a hash table for lookup, and maps chunks to object caches,
we can quickly find out which object cache a to be freed allocation belongs
to and thus don't need the boundary tags anymore.
* Reworked the slab boot strap process. We allocate from the initial area only
when really necessary, i.e. when the object cache for the respective
allocation size has not been created yet. A single page is thus sufficient.
other:
* vm_allocate_early(): Added boolean "blockAlign" parameter. If true, the
semantics is the same as for B_ANY_KERNEL_BLOCK_ADDRESS.
* Use an object cache for page mappings. This significantly reduces the
contention on the heap bin locks.

* Added "bool consumerLocked" parameter to VMCache::Unlock() and
ReleaseRefAndUnlock(). Since Unlock() may cause the cache to be merged with
a consumer cache, the flag is needed to prevent a deadlock in case the
caller still holds a lock to the consumer. Hasn't been a problem yet, since
that situation never occurred.
* VMCacheChainLocker: Reversed unlocking order to bottom-up. The other
direction could cause a deadlock in case caches would be merged, since the
locking order would be reversed. The way VMCacheChainLocker was used this
didn't happen, though.
* fault_get_page(): While copying a page from a lower cache to the top cache,
we do now unlock all caches but the top one, so we don't unnecessarily

* Reorganized the code for [un]mapping pages:
- Added new VMTranslationMap::Unmap{Area,Page[s]}() which essentially do what
vm_unmap_page[s]() did before, just in the architecture specific code, which
allows for specific optimizations. UnmapArea() is for the special case that
the complete area is unmapped. Particularly in case the address space is
deleted, some work can be saved. Several TODOs could be slain.
- Since they are only used within vm.cpp vm_map_page() and vm_unmap_page[s]()
are now static and have lost their prefix (and the "preserveModified"
parameter).
* Added VMTranslationMap::Protect{Page,Area}(). They are just inline wrappers
for Protect().
* X86VMTranslationMap::Protect(): Make sure not to accidentally clear the
accessed/dirty flags.
* X86VMTranslationMap::Unmap()/Protect(): Make page table skipping actually
work. It was only skipping to the next page.

* Pulled the physical page mapping functions out of vm_translation_map into
a new interface VMPhysicalPageMapper.
* Renamed vm_translation_map to VMTranslationMap and made it a proper C++
class. The functions in the operations vector have become methods.
* Added class GenericVMPhysicalPageMapper implementing VMPhysicalPageMapper
as far as possible (without actually writing new code).
* Adjusted the x86 and the PPC specifics accordingly (untested for the
latter). For the other architectures the build is, I'm afraid, seriously
broken.
The next steps will modify and extend the VMTranslationMap interface, so that
it will be possible to fix the bugs in vm_unmap_page[s]() and employ
architecture specific optimizations.

device path + child partition name. When a "raw" device is unpublished the node
removal notification triggers the partition and child partitions to be
unpublished/removed. Since in that case the "raw" node is already unpublished
trying to resolve it in devfs_unpublish_partition() again to unpublish the child
partitions would fail, leaving the child partition nodes behind. When a new raw
device would then become available publishing its partitions would fail because
of these left behind nodes, causing bug #4587. Seeing that this code is more
compact and straight forward anyway I don't quite see why it was changed in the
first place.

access to a vm_page. It is basically an atomically accessed thread ID field
in the vm_page structure, which is explicitly set by macros marking the
critical sections. As a first positive effect I had to review quite a bit of
code and found several issues.
* Added several TODOs and comments. Some harmless ones, but also a few
troublesome ones in vm.cpp regarding page unmapping.
* file_cache: PrecacheIO::Prepare()/read_into_cache: Removed superfluous
vm_page_allocate_page() return value checks. It cannot fail anymore.
* Removed the heavily contended "pages" lock. We use different policies now:
- sModifiedTemporaryPages is accessed atomically.
- sPageDeficitLock and sFreePageCondition are protected by a new mutex.
- The page queues have individual locks (mutexes).
- Renamed set_page_state_nolock() to set_page_state(). Unless the caller says
otherwise, it does now lock the affected pages queues itself. Also changed
the return value to void -- we panic() anyway.
* set_page_state(): Add free/clear pages to the beginning of their respective
queues as this is more cache-friendly.
* Pages with the states PAGE_STATE_WIRED or PAGE_STATE_UNUSED are no longer
in any queue. They were in the "active" queue, but there's no good reason
to have them there. In case we decide to let the page daemon work the queues
(like FreeBSD) they would just be in the way.
* Pulled the common part of vm_page_allocate_page_run[_no_base]() into a helper
function. Also fixed a bug I introduced previously: The functions must not
vm_page_unreserve_pages() on success, since they remove the pages from the
free/clear queue without decrementing sUnreservedFreePages.
* vm_page_set_state(): Changed return type to void. The function cannot really
fail and no-one was checking it anyway.
* vm_page_free(), vm_page_set_state(): Added assertion: The page must not be
free/clear before. This is implied by the policy that no-one is allowed to
access free/clear pages without holding the respective queue's lock, which is
not the case at this point. This found the bug fixed in r34912.
* vm_page_requeue(): Added general assertions. panic() when requeuing of
free/clear pages is requested. Same reason as above.
* vm_clone_area(), B_FULL_LOCK case: Don't map busy pages. The implementation is
still not correct, though.
My usual -j8 Haiku build test runs another 10% faster, now. The total kernel
time drops about 18%. As hoped the new locks have only a fraction of the old
"pages" lock contention. Other locks lead the "most wanted list" now.

* Changed the rw_lock_{read,write}_unlock() return values to void. They
returned a value != B_OK only in case of user error and no-one checked them
anyway.
* Optimized rw_lock_read_[un]lock(). They are inline now and as long as
there's no contending write locker, they will only perform an atomic_add().
* Changed the semantics of nested locking after acquiring a write lock: Read
and write locks are counted separately, so read locks no longer implicitly
become write locks. This does e.g. make degrading a write lock to a read
lock by way of read_lock + write_unlock (as used in the VM) actually work.
These changes speed up the -j8 Haiku image build on my machine by a few
percent, but more interestingly they reduce the total kernel time by 25 %.
Apparently we get more contention on other locks, now.

* Added VMCache::MovePage() and MoveAllPages() to move pages between caches.
* VMAnonymousCache:
- _MergeSwapPages(): Avoid doing anything, if neither cache has swapped out
pages.
- _MergeSwapPages() does now also remove source cache pages that are
shadowed by consumer swap pages. This allows us to call _MergeSwapPages()
before _MergePagesSmallerSource(), save the swap page shadowing check
there and get rid of the vm_page::merge_swap flag. This is an
optimization based on the assumption that usually none or only few pages
are swapped out, so we save a lot of checks.
- Implemented _MergePagesSmallerConsumer() as an alternative to
_MergePagesSmallerSource(). The former is used when the source cache has
more pages than the consumer cache. It iterates over the consumer cache's
pages, moves them to the source and finally moves all pages back to the
consumer. The final move is relatively cheap (though unfortunately we
still have to update all pages' vm_page::cache field), so that overall we
save iterations of the main loop with the more expensive checks.
The optimizations particularly improve the common fork()+exec*() situations.
fork() uses CoW, which is implemented by putting two new empty caches between
the to be copied area and its cache. exec*() destroys one copy of the area,
its cache and thus causes merging of the other new cache with the old cache.
Since this usually happens in a very short time, the old cache does still
contain many pages and the new cache only few. Previously the many pages were
all checked and moved individually. Now we do that for the few pages instead.
A very extreme example of this situation is the Haiku image build. jam has a
huge heap (> 200 MB) and it fork()s+exec*()s for every action to be executed.
Since during the cache merging the cache is locked, any write access to a
heap page causes jam to block until the cache merging is done. Formerly that
took so long that it killed a lot of parallelism in multi-job builds. That
could be observed particularly well when lots of small actions where executed
(like the Link, XRes, Mimeset, SetType, SetVersion combos when building
executables/libraries/add-ons). Those look dramatically better now.
The overall speed improvement for a -j8 image build on my machine is only
about 15%, though.

multiple locking primitives are involved.

* simple_smp scheduler: Rewrote the interesting part of
enqueue_in_run_queue(). It always selects a target CPU for the inserted
thread, now. If no CPU is idle, the CPU running the thread with the lowest
priority is chosen. If the thread running on the target CPU has a lower
priority than the inserted one, it will be asked to reschedule. If that's
the current CPU, we'll return the correct value (wasn't done before at
all).
These changes help reducing latencies. On my machine in an idle system
playing music DebugAnalyzer shows maximum latencies of about 1 us. I still
find that a bit much, but it's several orders of magnitude better than
before. The -j8 Haiku image build time dropped about 10%.

implemented by means of an additional member in cpu_ent.
* Removed thread::keep_scheduled and the related functions. The feature
wasn't used yet and wouldn't have worked as implemented anyway.
* Resurrected an older, SMP aware version of our simple scheduler and made it
the default instead of the affine scheduler. The latter is in no state to
be used yet. It causes enormous latencies (I've seen up to 0.1s) even when
six or seven CPUs were idle at the same time, totally killing parallelism.
That's also the reason why a -j8 build was slower than a -j2. This is no
longer the case. On my machine the -j2 build takes about 10% less time now
and the -j8 build saves another 20%. The latter is not particularly
impressive (compared with Linux), but that seems to be due to lock
contention.

is only really implemented for x86. For the other architectures

waits for certain events on a given page, NotifyPageEvents() wakes up
waiting threads respectively.
* Used the new feature instead of condition variables for waiting on busy
pages. We save publishing and unpublishing of a condition variable whenever
a page is marked busy. There's only something to do, if there's at least
one thread waiting in the list of the respective cache. The general
assumption is that this is only rarely the case and even if it happens,
there should be only very few threads.
* Added an apparently missing notification in cache_io(). At least I didn't
see the reason for it not being there.

* Reimplemented VMKernelAddressSpace. It is somewhat inspired by Bonwick's
vmem resource allocator (though we have different requirements):
- We consider the complete address space to be divided into contiguous
ranges of type free, reserved, or area, each range being represented by
a VMKernelAddressRange object.
- The range objects are managed in an AVL tree and a doubly linked list
(the latter only for faster iteration) sorted by address. This provides
O(log(n)) lookup, insertion and removal.
- For each power of two size we maintain a list of free ranges of at least
that size. Thus for the most common case of B_ANY*_ADDRESS area
allocation, we find a free range in constant time (the rest of the
processing being O(log(n))) with a rather good fit. This should also
help avoiding address space fragmentation.
While the new implementation should be faster, particularly with an
increasing number of areas, I couldn't measure any difference in the -j2
haiku build. From a cursory test the -j8 build hasn't tangibly benefitted
either.

simplify migration of the area management, but as a side effect, it also
makes area deletion O(1) (instead of O(n), n == number of areas in the
address space).
* Moved more area management functionality from vm.cpp to VMAddressSpace and
VMArea structure creation to VMArea. Made the list and list link members
itself private.
* VMAddressSpace tracks its amount of free space, now. This also replaces
the previous mechanism to do that only for the kernel address space. It
was broken anyway, since delete_area() subtracted the area size instead of
adding it.
* vm_free_unused_boot_loader_range():
- lastEnd could be set to a value < start, which could cause memory
outside of the given range to be unmapped. Haven't checked whether this
could happen in practice -- if so, it would be seriously unhealthy.
- The range between the end of the last area in the range and the end of
the range would never be freed.
- Fixed potential integer overflows when computing addresses.

CreateAsmStructOffsetsHeader mechanism to generate a header with macros
defined to the sizes of the structures we're interested in and when compiling
in C mode define the structures as "struct { char bytes[size]; }".
It works in principle, but due to how jam works, one would have to specify the
dependency to the generated header for all sources that include it directly or
indirectly.

* x86 CPU module:
- Implemented the new hook.
- Prefixed most debug output with the CPU index. Otherwise it gets quite
confusing with multiple CPUs.
- generic_init_mtrrs(): No longer clear all MTRRs, if they are already
enabled. This lets us benefit from the BIOS's setup until we install our
own -- otherwise with caching disabled things are *really* slow.
* arch_vm.cpp: Completely rewrote the MTRR handling as the old one was not
only slow (O(2^n)), but also broken (resulting in incorrect setups (e.g.
with cachable ranges larger than requested)), and not working by design for
certain cases (subtractive setups intersecting ranges added later).
Now we maintain an array with the successfully set ranges. When a new range
is added, we recompute the complete MTRR setup as we need to. The new
algorithm analyzing the ranges has linear complexity and also handles range
base addresses with an alignment not matching the range size (e.g. a range
at address 0x1000 with size 0x2000) and joining of adjacent/overlapping
ranges of the same type.
This fixes the slow graphics on my 4 GB machine (though unfortunately the
8 MTRRs aren't enough to fully cover the complete frame buffer (about 35
pixel lines remain uncachable), but that can't be helped without rounding up
the frame buffer size, for which we don't have enough information). It might
also fix#1823.

* It seems to be even harder to understand basic locking primitives: when you
think about it, it shouldn't surprise you that conditional variables never
return B_WOULD_BLOCK. This fixes gdb again.
* Added tracing support to the ports subsystem.
* get_port_message() will now resize the port heap if needed (but will also
take timeouts into account while doing so, more or less). The initial port
space is 4MB (as before), the growth rate is the same, and the system wide
limit is arbitrarily set to 64 MB (all swappable). A team limit has been set
to 8 MB, but is not enforced yet. Since ports are using up address space in
the kernel, those seems to be proper limits.
* This also fixes a strange, and rare lockup where the mouse cursor would still
move, but everything else would basically hang, but look perfectly normal from
KDL on the first look. As recently happened on Brecht's laptop, and debugged
by mmlr and me: the cbuf space got used up when lots of windows wanted to
redraw after a workspace switch. The app_server wouldn't answer anymore to
client requests, but thought it would have done so, as LinkSender::Flush()
doesn't care if it got a B_NO_MEMORY (the ports will now block until memory
is available if possible, so that should not be a problem anymore).
* Improved "port" KDL command, it now also prints the messages in the port.

- Pulled the module-related implementation part of BaseDevice into new class
AbstractModuleDevice and made all methods of BaseDevice virtual. Small
adjustments to devfs to be happy with the new BaseDevice interface.
- Added BaseDevice subclass FileDevice, which maps the interface to a file's
file descriptor. Still got a few TODOs, but should basically work.
- Use FileDevice for publishing file disk devices in devfs. Now those do
actually work, though there's some BFS trouble with one of the images I
tested.

* The vesa driver no longer uses VGA programming if the chip does not support
VGA compatibility.
* The VESA driver now tries to set the DAC to 8 bits per color gun.
* In VESA modes, the driver no longer tries to use VGA programming; introduced
the new vesa_set_indexed_colors() that is now used for palette programming.
This should fix wrong colors of 8 bit BWindowScreen users with VESA on real
hardware (emulators usually didn't mind either way).
* Note that the app_server needs to maintain a palette per 8 bit screen, as
right now, the colors are garbled after a workspace switch. Stefano, are you
looking into that already?

- added some multiboot support code:
- dump some of the passed info,
- parse command line (skip the 'kernel' name and pass the rest to stage2_args.arguments),
- added an add_stage2_driver_settings() function which takes stage2_args.arguments and translates it into safe mode driver settings, a bit dumb for now.
This allows using qemu -kernel haiku_loader -append 'debug_screen true' and get debug output without having to enter the menu (once multiboot info is used to determine the boot device too).
The idea is to allow passing driver settings and using them to pass extra stuff (like 'force_keymap fr' and other stuff for demo), and to help automate tests ('run_test /bin/sometest').
This should answer Axel's question :)

allow for more parallelism. Also introduce seperate locks for the bins and
for page allocation. This greatly reduces lock contention and reduces the
duration the locks are held due to them overall protecting less code. Now only
allocations of the same size hitting the same allocator or allocating larger
chunks of memory should block. Previously, basically any allocation and also
free would be mutually exclusive, making it scale pretty badely.
* Added memalign_nogrow(). As it uses heap_memalign() anyway, there's no real
reason not to allow for an alignment.
* Some cleanup.

ROUNDUP to use '*' and '/' -- the compiler will optimize that for powers of
two anyway and this implementation works for other numbers as well.
* The thread::fault_handler use in C[++] code was broken with gcc 4. At least
when other functions were invoked. Trying to trick the compiler wasn't a
particularly good idea anyway, since the next compiler version could break
the trick again. So the general policy is to use the fault handlers only in
assembly code where we have full control. Changed that for x86 (save for the
vm86 mode, which has a similar mechanism), but not for the other
architectures.
* Introduced fault_handler, fault_handler_stack_pointer, and fault_jump_buffer
fields in the cpu_ent structure, which must be used instead of
thread::fault_handler in the kernel debugger. Consequently user_memcpy() must
not be used in the kernel debugger either. Introduced a debug_memcpy()
instead.
* Introduced debug_call_with_fault_handler() function which calls a function
in a setjmp() and fault handler context. The architecture specific backend
arch_debug_call_with_fault_handler() has only been implemented for x86 yet.
* Introduced debug_is_kernel_memory_accessible() for use in the kernel
debugger. It determines whether a range of memory can be accessed in the
way specified. The architecture specific back end
arch_vm_translation_map_is_kernel_page_accessible() has only been implemented
for x86 yet.
* Added arch_debug_unset_current_thread() (only implemented for x86) to unset
the current thread pointer in the kernel debugger. When entering the kernel
debugger we do some basic sanity checks of the currently set thread structure
and unset it, if they fail. This allows certain commands (most importantly
the stack trace command) to avoid accessing the thread structure.
* x86: When handling a double fault, we do now install a special handler for
page faults. This allows us to gracefully catch faulting commands, even if
e.g. the thread structure is toast.
We are now in much better shape to deal with double faults. Hopefully avoiding
the triple faults that some people have been experiencing on their hardware
and ideally even allowing to use the kernel debugger normally.

* Added x86_double_fault_get_cpu(), a save way to get the CPU index when in
the double fault handler. smp_get_current_cpu() requires at least a somewhat
intact thread structure, so we rather want to avoid it when handling a double

class to BOpenHashTable and changed the interface slightly so that HashTableLink
became superfluous.

pressing Alt-SysReq + key.
* By default, only Alt-SysReq+'d' is used as a means to deliberately enter
the kernel debugger. F12 belongs to userland again, now :-)
* Debugger add-ons now have another optional method to implement their own
emergency keys - 'd' for the debugger cannot be overridden, though.
* The mechanism can be turned off via a new kernel setting, so it's not that
easy anymore to "crash" Haiku if you don't want to.

- While walking down the cache chain, we keep all upper caches locked.
- When we have to unlock -- when waiting for a busy page or reading a page in
-- we unlock completely, including the address space, and restart
vm_soft_fault().
- Folded fault_get_page() and fault_find_page() into one.
This simplifies and improves things considerably:
- We no longer need dummy pages.
- We no longer need vm_area::no_cache_change.
- #2710 is fixed, since we no longer hold the address space lock while
waiting.
* vm_soft_fault(): When we have found our page, we first check whether a page
is already mapped at the address. If it is already our page, we just change
its protection. If not, we unmap it first. Fixes race conditions when multiple
threads fault at the same address at the same time.
* fault_get_page(): When copying a read-only page from a lower cache, no longer
mark it active, since at least for the fault area it is shadowed from then on.
* vm_set_area_protection(): Fixed potential overflow for in the
vm_translation_map::protect() call.

gcc could apparently assume that the register assigned to the one in the
clobber list would keep its value (as can be observed when disassembling
add_debugger_command_etc()).
Using a dummy output register works around the problem and also avoids the
unnecessary initialization of the register.

* Implemented a way to do asynchronous pre-fetching when mapping files.
* There are slight code duplications in some places that could benefit
from cleaning up, but nothing too bad.
* Implementing smarter ways to trigger prefetching and more analysis of
the situations in the kernel would be nice. Currently up to 10 MB
of every mapped file are pre-fetched without further analysis.
* The speed improvement is nice for certain operations. On our test
system (real hardware), Firefox took 9 seconds from being launched
to display a window. Now it takes 5 seconds. Both measurements
right after booting. The same system took 35 seconds from launching
Haiku in the GRUB menu to displaying the Tracker desktop background
image. Now it takes 27 seconds.
* We didn't have the chance to check out the effects of this on the
CD boot, but potentially, they could speed it up a lot.

- Hopefully fixed locking issues related to notifying the profiler thread
for good. We still had an inconsistent locking order, since the scheduler
notification callbacks are invoked with the thread lock held and have to
acquire the object lock then, while the other callbacks acquired the object
lock first and as a side effect of ConditionVariable::NotifyOne() acquired
the thread lock. Now we make sure the object lock is the innermost lock.
- Track the number of dropped events due to a full buffer.
_user_system_profiler_next_buffer() returns this count now.
- When scheduling profiling events are requested also listen to wait objects
and generate the respective profiling events. We send those events lazily
and cache the infos to avoid resending an event for the same wait object.
- When starting profiling we do now generate "thread scheduled" events for
the already running threads.
- _user_system_profiler_start(): Check whether the parameters pointer is a
userland address at all.
- The system_profiler_team_added event does now also contain the team's name.
* Added a sem_get_name_unsafe() returning a semaphore's name. It is "unsafe",
since the caller has to ensure that the semaphore exists and continues to
exist as long as the returned name is used.
* Adjusted the "profile" and "scheduling_recorder" according to the system
profiling changes. The latter prints the number of dropped events, now.

* Added debugger commands to resolve usb_ids to pipes.
* Adjusted the physical memory allocator to be usable in a slimmed down mode
when running inside the kernel debugger.
* Implemented USB keyboard support for KDL through a kernel debugger add-on.
* Added kgetc() and made use of it where previously individual methods were used
to ensure that reading characters always goes through the kernel debugger
add-ons and the other methods.
This has some preconditions to meet though:
1) The keyboard must be in the boot protocol (currently the case but needs to
be revisited once we have a full usb_hid).
2) The keyboard must be attached to a UHCI root port (i.e. not use EHCI or OHCI,
also not through hubs unless those are USB 1.1).
3) the usb_hid driver has to be opened for this to work. This means that for the
time between initializing USB and when usb_hid is opened by the input_server
there is no keyboard support.
Also note that this has no way of detecting hot-plug, meaning that you can't
re-attach your USB keyboard from the hub to the root port once in KDL.
On the bright side of things, since this is a non-destructive mechanism it is
possible to enter and leave KDL without loosing the USB state.
Tested OK in QEMU, not tested on real hardware yet, will see in a few minutes.

It causes the interrupt handler to be inserted at the very end of the list
instead of at the top. It is intended to be used as a workaround when a
interrupt handler cannot know if it actually handled the interrupt. This
should never be used by native drivers. Also if we know that the result is
not valid because of this flag we won't disable the vector in case we count
many unhandled interrupts as those numbers are then unreliable.
* Moved B_NO_LOCK_VECTOR to be a private flag as well.
* Made the interrupt handler list a simple manually maintaned singly linked list

This is not necessary, since userland teams' page directories also
contain the kernel mappings, and avoids unnecessary TLB flushes. To make
that possible the vm_translation_map_arch_info objects are reference
counted now.
This optimization reduces the kernel time of the Haiku build on my
machine with SMP disabled a few percent, but interestingly the total
time decreases only marginally. Haven't tested with SMP yet, but for
full impact CPU affinity would be needed.

them most efficiently. Added generic implementations that can be used,

* Changed vm_{get,put}_physical_page(). The former no longer accepts
flags (the only flag PHYSICAL_PAGE_DONT_WAIT wasn't needed anymore).
Instead it returns an implementation-specific handle that has to be
passed to the latter. Added vm_{get,put}_physical_page_current_cpu()
and *_debug() variants, that work only for the current CPU,
respectively when in the kernel debugger. Also adjusted the
vm_translation_map operations accordingly.
* Made consequent use of the physical memory operations in the source
tree.
* Also adjusted the m68k and ppc implementations with respect to the
vm_translation_map operation changes, but they are probably broken,
nevertheless.
* For x86 the generic physical page mapper isn't used anymore. It is
suboptimal in any case. For systems with small memory it is too much
overhead, since one can just map the complete physical memory (that's
not done yet, though). For systems with large memory it counteracts
the VM strategy to reuse the least recently used pages. Since those
pages will most likely not be mapped by the page mapper anymore, it
will keep remapping chunks. This was also the reason why building
Haiku in Haiku was significantly faster with only 256 MB RAM (since
that much could be kept mapped all the time).
Now we're using a different strategy: We have small pools of virtual
page slots per CPU that are used for the physical page operations
(memset_physical(), memcpy_*_physical()) with CPU-pinned thread.
Furthermore we have four slots per translation map, which are used to
map page tables.
These changes speed up the Haiku image build in Haiku significantly. On
my Core2 Duo 2.2 GHz 2 GB machine about 40% to 20 min 40 s (KDEBUG
disabled, block cache debug disabled). Still more than factor 3 slower
than FreeBSD and Linux, though.

* Introduced flag "invoke_scheduler" in the per CPU structure. It is
evaluated in hardware_interrupt() (x86 only ATM).
* Introduced SMP_MSG_RESCHEDULE_IF_IDLE message, which enters the
scheduler when the CPU currently runs an idle thread.
* Don't do dprintf() "CPU x halted!" when handling a SMP_MSG_CPU_HALT
ICI message. It uses nested spinlocks and could thus potentially
deadlock itself (acquire_spinlock() processes ICI messages, so it
could already hold one of the locks). This is a pretty likely scenario
on machines with more than two CPUs, but is also possible when the
panic()ing thread holds the threads spinlock. Probably fixes#2572.
* Reworked the way the kernel debugger is entered and added a "cpu"
command that allows switching the CPU once in KDL. It is thus possible
to get a stack trace of the thread not on the panic()ing CPU.
* When a thread is added to the run queue, we do now check, if another
CPU is idle and ask it to reschedule, if it is. Before this change, the
CPU was continuing to idle until the quantum of the idle thread
expired. Speeds up the libbe.so build about 8% on my machine (haven't
tested the full Haiku image build yet).
* When spinlock debugging is enabled (DEBUG_SPINLOCKS) we also record
the spinlock acquirer on non-smp machines. Added "spinlock" debugger
command to get the info.
* Added debugger commands "ici" and "ici_message", printing info on
pending ICI message respectively on a given one.
* Process not only a single ICI message in acquire_spinlock() and other
places, but all pending ones.
* Also process ICI messages when waiting for a free one -- avoids a
potential deadlock.
* Mask out non-existing CPUs in send_multicast_ici(). panic() instead of
just returning when there's no target CPU left.

vm_translation_map_arch_info indicating on which CPUs the address
space is active. This allows flush_tmap() to avoid ICI for user
address spaces when the team isn't currently running on any other CPU.
In this context ICI is relatively expensive, particularly since we map
most pages via vm_map_page() and therefore invoke flush_tmap() pretty
much for every single page.
This optimization speeds up a "hello world" compilation about 20% on
my machine (KDEBUG turned off, freshly booted), but interestingly it
has virtually no effect on the "-j2" haiku build time.

buffer format, the buffer can now also contain other events than just
stack traces. ATM these are only references to the image events
(created/deleted). Therefore we no longer have to flush the profiling
buffer after such an event, since the debugger can exactly match the
samples. Since we couldn't flush when the profiling timer hit while the
thread was in the kernel, that wasn't working that well anyway.
"profile -f" fails to translate stack trace addresses only very rarely,
now.

* Changed module image ref counting. A referenced module has single
reference to its image, which is released when the module becomes
unreferenced.
* get_module() for a reference module will not try to re-get and re-set
the module's image anymore. That could lead to a similar module (from
different paths) being loaded at the same time. A module from a new
file can only be loaded when the old one has been put completely.
* Simplified B_KEEP_ALIVE module handling a bit. When the module is
initialized, we add another reference, which we'll never free. Thus
the module remains loaded without special handling. Removed
module_image::keep_loaded. A B_KEEP_ALIVE module remains referenced
and thus its image remains referenced, too.
* Removed module::file, a cached path to the module's image. An
optimization that wouldn't work with multiple root directories for
modules (/boot/beos/..., /boot/common/...) or when module files were
moved. get_module() does now always search the image file, when the
module is still unreferenced. This should be a bit slower than before,
but I didn't notice any difference in VMware at least. If it turns out
to be a problem we could introduce a more intelligent cache that stays
up to date by using node monitoring.

samples per tick. The stack depth is used as a maximum.

now flush the buffer as soon as it is 70% full, *if* we didn't interrupt
a kernel function. When the buffer runs full and we still haven't hit a
user function, we drop the tick. The number of dropped ticks is recorded
and sent to the debugger with the next update message.
Reverted the previous partial solution (the temporary disabling of
profiling while in debugger support code).

code. This fixes potential deadlocks which could occur when a
profiling timer event occurred that would require the buffer to be
flushed while the thread was just sending something to the debugger or

since we should never try to flush the buffer when the timer event
occurred in the kernel, since the thread might hold a lock that the
debugger thread could try to acquire. Will implement a more general
solution later.

thread how long the thread waited on each locking primitive in total.

* Implement color palette generation for the boot splash images in the
generate_boot_screen build tool. Only 4-bit screen support is missing now.

This should fix black boot screens for graphics cards that don't support

#2177 almost looks like the one, but it looks more like the mode is out
of range if I understand the ticket right.

* Reorganized merging of caches a bit. Renamed MergeStore() to Merge()
and moved some more functionality into it. The method also moves the
pages from source to consumer, now. This is necessary, since
VMAnonymousCache needs to consider both physical pages and swap pages
at the same time. Before we first moved the physical pages and the
swap pages later, which was broken for two reasons: (1) A swap page in
the consumer cache shadows a physical page of the source cache, which
we ignored. (2) A source cache's physical page that also had a swap
page would lose the latter in the process when moved to the consumer
cache, i.e. if the page was not marked modified, it could be stolen
and its data would be lost.
These changes improve the situation when building Haiku with 256 MB RAM
in that jam doesn't crash anymore, but in my test the system became
totally unusable after about an hour or 7000 targets (GUI froze). For
some reason it didn't manage to free pages anymore although swapping
heavily.

Ingo suggestions: instead of having one global sem_undo list,
we now have two local list, one per semaphore set and one per team
which is held in its xsi_sem_context structure, along with a mutex.
A mutex has also been added to the semaphore set class in order to
protect the local list, but also in order to (hopefully) improve
concurrency.

* Added vm_page_get_stats() which fills in the memory related part of
the system_info structure. Used and cached pages are computed
differently, now. The "available" (== not committed) memory is no

actually used/free pages (with swap support enabled it is even
less meaningful, since we first commit swap space when possible).
We do also consider the memory used by the block cache as cached
pages, now. All in all these changes should fix the memory statistics

include the guard pages. Adjusted the kernel and boot loader code
accordingly -- the guard pages size is added/not removed respectively.
The stack size passed to _kern_spawn_thread() is now the actually usable
size, and it is no longer possible to specify a size smaller than or
equal to the guard pages size.

classes have grown new methods and attributes to deal with that. The
"finished" callbacks have got additional parameters to indicate
whether the transfer was only partial and how much has been
transferred. Other callbacks and functions have a size_t* in/out
parameter instead of a simple size_t, now.
* vfs_{read,write}_pages() do now use the I/O request framework instead
of the underlying FS's {read,write}_pages() hooks (those should be
unused now). Furthermore they've got an additional "flags" parameter,
which is passed to IORequest::Init(), i.e. it allows to specify that
the given vecs refer to physical addresses.
* The file cache's read_into_cache() reads directly into physical
pages, now.
* Fixed bug in DoIO::IO(): The offset was not adjusted, so that all
pages were incorrectly transferred from/to the same location.
* Fixed broken subrequest scheduling loop head in
do_iterative_fd_io_iterate().
* Adjusted the test driver and implemented its io() hook. Using this
driver I/O requests are passed all the way from the VFS/VM to the
driver and through the I/O scheduler. It even seems to work. :-)
* Added missing const to the iovec* parameter of the IORequest::Init()
methods.
* Disabled some debug output by default. Added new optional debug

to provide asynchrounous (or only synchronous, if asynchronous is not

{read,write}_pages(). None of the FS implementations implement them
yet.
* Implemented some support functions for request-based I/O. File system
implementations can use do_fd_io() which passes an I/O request to the
layer responsible for a given FD, and do_iterative_fd_io(), which
translates a request for a file to subrequests for the underlying
device and passes them on. Both fall back to synchrounous processing
when the io() hook is not supported.
Furthermore added vfs_synchronous_io() which should be handy for the
devfs to perform io_requests synchronously for devices that don't
support the io() hook.

It does its job by using part of libsupc++, with fake malloc and friends to make sure it won't double fault.
Works here, but cp-demangle must be extracted by hand from the lib, can't get the rule to work as I want, Ingo ?
Maybe using it directly without malloc hack would be ok with 16KB buffer, but I'm not sure of that.

this is no longer the case, the argument has become superfluous. For

* VMCache:
- Renamed vm_cache to VMCache, merged it with vm_store and made it a
C++ class with virtual methods (replacing the store operations).
Turned the different store implementations into subclasses.
- Introduced MergeStore() callback, changed semantics of Commit().
- Changed locking and referencing semantics. A reference can only be
acquired/released with the cache locked. An unreferenced cache is
deleted and a mergeable cache merged when it is unlocked. This
removes the "busy" state of a cache and simplifies the page fault
code.
* Added VMAnonymousCache, which will implement swap support (work by
Zhao Shuai). It is not integrated and used yet, though.
* Enabled the mutex/recursive lock holder asserts.
* Fixed DoublyLinkedList::Swap().
* Generalized the low memory handler to a low resource handler. And made
semaphores and reserved memory handled resources. Made
vm_try_resource_memory() optionally wait (with timeout), and used that
feature to reserve memory for areas.

- implemented 040 mmu init. doing so I spotted a bug in ARAnyM which ignored [ID]TT0. Linux likely doesn't use them but I was too lazy touse temporary page tables. I also noticed bitfields were in wrong order, to be fixed.

though until we use ACPI for proper PCI IRQ routing through the IO APIC.
Therefore the IO APIC code path is not yet enabled and the IO APIC isn't used.
ISA interrupts would work though, as would PCI interrupts if you'd hardcode
them for your specific configuration.
Note that this change also modifies some parts in the bootloader and in the PIC
setup to make local APICs available even on non-SMP systems. This causes APIC
timers to be used instead the normal PIT if it is available (also on non-SMP
configurations).
Also fixes some general errors in SMP and PIC code as well as some code cleanup.

This allows drivers to lock the memory outside of the original team context.
* create_area_etc() got a struct team as first argument, but that should have
been a team_id.

It follows the semantics of the BFS R/W lock, though, that is, if you unlock
your write lock before the read locks, the read locks effectively become
write locks, too.

serve, bin sizes and page size. This minimizes the amount of "large"
allocations made in heaps that don't have a bin for the allocation size
(combining multiple pages). This is desirable as such large allocations
are generally pretty inefficient, and also because it separates larger from
smaller allocations better, making the chance of a heap becoming empty higher.
For now there are three heap classes "small", "large" and "huge", with a
predefined set of bin sizes for each. This might need some finetuning later on.
Reduce the grow size to 4MB though as the allocations should now be spread
across heap classes which each grow on their own.

for the kernel a bit, since it can just copy the buffer and check
whether it looks OK instead of messing around with individual strings.
The runtime loader also gets a flattened array.

* Use that nogrow version in the VM code to avoid a deadlock with the address
space lock when a grow operation would try to create an area while a malloc
happened from such a function in the VM.
* When waiting for a grow to happen, notify the waiting thread from the grower
also if it failed to allocate a new heap. Otherwise a thread would just sit
there and wait until another thread requested growing too and that one
succeeded (or just forever in the worst case).
* Make the dedicated grow heap growable too. If the current grow heaps run low
on memory it will instruct the grower to allocate a new grow heap. This
reduces the likelyhood of running out of memory with no way to grow to a
minimum. As the growing is done asynchronously it is still possible to
happen, but it is highly unlikely as the grow heap is solely used to
allocate memory in the process of creating new heap areas and it will even
try using normal public memory if the dedicated memory has run out.
* Reduced the dedicated grow heap from 2 to 1MB. As it can now grow itself, it
doesn't need to last so long.
* Extract heap creation into it's own function that does area creation and heap
attach and use this function for growing normal and grow heaps.

B_DEBUG_SPINLOCK_CONTENTION is defined to 1. It typedefs spinlock to a
structure (thus breaking BeOS binary compatibility), containing a
counter which is incremented whenever a thread has to wait for the
spinlock.
* Added macros for spinlock initialization and access and changed
code using spinlocks accordingly. This breaks compilation for BeOS --
the macros should be defined in the respective compatibility wrappers.
* Added generic syscall to get the spinlock counters for the thread and
the team spinlocks.

second command is invoked whenever the first command has written a
complete line. The line is passed as last argument to the second
command. The new command flag B_KDEBUG_PIPE_FINAL_RERUN causes the
second command to be invoked again (with NULL argument) after the
first command is done.
* Added kprintf_unfiltered() and kputs_unfiltered() which bypass the
pipe mechanism and directly print to the bluescreen/serial output.
* Moved most commands from debug.cpp to the new
debug_builtin_commands.cpp.
* B_KDEBUG_DONT_PARSE_ARGUMENTS commands don't get an argument anymore,
if it would consist of white space only.
* Added new debugger command return value B_KDEBUG_ERROR, which
indicates that executing the command failed. This return code will
abort a complete pipe.
* Since debugger commands can nest (i.e. one command can invoke another
one) the setjmp()/longjmp() mechanism to restore the stack after a
page fault in a command needs more than one jump buffer.
* Added abort_debugger_command(), which longjmp()s out of the currently
executed command. This will also abort the current pipe.
* When pagination is enabled pressing "a" will abort the running command
(as opposed to "q" which only disables the blue screen output, but
lets the command continue).
* Added debugger commands:
- "grep" which can be used to filter output by pattern. Removed the
"filter" command and the underlying mechanism that did that before.
- "head" which prints only the first lines of output of another
command.
- "wc" counts lines, words, and characters of another command's
output.

Changed condition variables so that it is allowed to block (e.g. lock
mutexes etc.) between Add() and Wait(). This fixes#2059, since the
block writer used them this way and could thusly fail to wait for a
condition variable, causing a temporary stack object to be used past its

* Changed the semantics of unnamed semaphores. Before parent and child
of a fork() would always share an earlier created semaphore. Now we do
that only, if the "shared" parameter of sem_init() was true. That's
still not quite the behavior Linux and Solaris have, but should be
perfectly fine with how reasonable code would use the API.
* There's a global table for shared unnamed semaphores now. ATM a
semaphore is leaked when no one explicitly destroys it (just as with
named sems).
* Enforce per-team and global semaphore number limits.

* Trivial adjustments of code using mutexes. Mostly removing the
mutex_init() return value check.
* Added mutex_lock_threads_locked(), which is called with the threads
spinlock being held. The spinlock is released while waiting, of
course. This function is useful in cases where the existence of the
mutex object is ensured by holding the threads spinlock.

but doesn't need a semaphore (it uses thread blocking and a simple
queue instead). Initialization can't fail. In fact it is ready to use
without initialization when living in the bss segment, also in the
early boot process. It's as fast as a benaphore in cases of low lock
contention, and faster otherwise. Only disadvantage is the higher
immediate memory footprint of 16 bytes.
* Changed how the "thread" and "threads" debugger commands list the
objects they are waiting for. Cutexes are also included.

thread_block(), thread_unblock(),...) that allow a thread to wait for
something without needing a semaphore or condition variable. It can
simply block and another thread can unblock it. Supports timeouts and
interrupting. Both semaphores and condition variables use this
common mechanism, now.
* Semaphores:
- Some simplifications due to the thread blocking mechanism.
- Changed locking order to sem -> thread. It was the other way around
before and when introducing the wait_for_objects() support I had
also introduced a situation where the locking was reverse, which
could potentially cause a dead lock on SMP systems.
- Instead of queueing thread structures, a semaphore queues
queued_thread entries now, which are created on the stack. The
thread::sem structure could thus be removed.
- Added sem_entry::net_count, which is sem_entry::count plus the
acquisition count of all waiting threads. This number is needed in
remove_thread_from_sem() and instead of computing it there we
maintain it.
- Fixed remove_thread_from_sem(). It would not unblock threads, if
the sem count was <= 0.
- Made sem::last_acquirer unconditional. It is actually needed for
sem_info::latest_holder. Fixed fill_sem_info() accordingly.
- Added some optional tracing output, though only via ktrace_printf().
* Condition variables:
- Could be simplified significantly through the use of the thread
blocking mechanism. Removed a good deal of unnecessary code.
- Moved the ConditionVariableEntry "flags" parameter from Wait() to
Add(), and adjusted all places where condition variables are used
accordingly.
* snooze() uses thread_block_with_timeout() instead of a semaphore.
* Simplified thread interrupting in the signal and user debugger code.
Instead of separate functions for threads waiting on a semaphore or
condititon variable, we only have a single thread_interrupt(), now.

- If the hook of the timer we're cancelling is currently being
executed, we do now wait till it is finished. This is how the BeBook
specifies the function to behave.
- Periodic timers would not be cancelled, if their hook was being
invoked at the same time, since they weren't in the queue during
that time.
- Since we know the CPU on which the timer is scheduled (timer::cpu),
we don't have to look through any other CPU queue to find it.
- Fixed the return value. It should report whether the timer had
already fired, and was not always doing that.
* Added private add_timer() flag B_TIMER_ACQUIRE_THREAD_LOCK. It causes
the thread spinlock to be acquired before the event hook is called.
cancel_timer() doesn't wait for timers with the flag set. Instead we
check in the timer interrupt function after acquiring the thread
spinlock whether the timer was cancelled in the meantime. Calling
cancel_timer() with the thread spinlock being held does thus avoid any
race conditions and won't deadlock, if the event hook needs to acquire
the thread spinlock, too. This feature proves handy for some kernel
internal needs.
* The scheduler uses a B_TIMER_ACQUIRE_THREAD_LOCK timer now and
cancel_timer() instead of the no longer needed
_local_timer_cancel_event().

created by {vm,_user}_map_file() with REGION_NO_PRIVATE_MAP. When
fork()ing those areas won't be copied CoW, but rather be cloned. This
is needed for mmap() MAP_SHARED.
* {vm,_user}_map_file() also accept an FD argument < 0, in which case an
anonymous area is created.
* Implemented mmap() and munmap(). Currently there's the restriction
that we can't partially unmap areas. Otherwise the functions should be
rather compliant. We also support the non-POSIX extension
MAP_ANONYMOUS.

* Changed common_fcntl() to also work for non-vnode-based FDs. Only the
operations that require a vnode do fail now. F_SETFL uses the new
fd_ops::set_flags() if available, falling back to the vnode respective

in those cases, but might not be anymore if we ever change fd_vnode().
ftp can at least connect to servers again, now, but apparently we still

- The net_stack driver is no longer used. Instead we have a kernel
module which is directly used by syscall implementations in the
kernel. I.e. we no longer tunnel those functions through ioctls, but
have normal syscalls.

are no longer vnode based.

- Moved most file_system_module_info hooks into separate structures.
Those that operate on mounted volumes to fs_volume_ops, those
operating on a vnode to fs_vnode_ops.
- Got rid of the fs_volume, fs_cookie, fs_vnode typedefs. We use void*
again.
- Instead of a void* volume and node cookie hooks are passed a
fs_volume and fs_vnode structure pointer, which contain the cookie
and an ops pointer (fs_volume a few more things).
- The VFS {new,publish,get,...}_vnode() functions take a fs_volume*
instead of the volume ID. So does vfs_get_fs_node_from_path().
- Added type and flags arguments to publish_vnode() and the
get_vnode() hook and removed the type argument from lookup() hook.
Added vnode::type using formerly unused bits to store the node type.
Simplified a few things in the VFS due to the now always available
node type.
- Added fs_volume_ops::{create,delete}_sub_vnode() and
fs_vnode_ops::get_super_vnode() hooks. They are used to support file
system layers, e.g. allowing to extend an FS not supporting BeOS
attribute with attribute support. Needs some more work in the VFS.
- Added fs_vnode_ops::create_special_node() hook for creating special

while still allowing to open() entries. 2) When the FS has other
layers on to of it (or the respective node) it might not be

The dividing line is back although with a little more spacing. Also the icon
spacing is a little increased and I added a subtle outline as well as a little
brighter background for the active icon look. The placement logic is now not
independant of each other anymore, otherwise I couldn't center both images
as a unit. But only the vertical placement is affected.

The disk device manager now creates and scans a device when a "raw" node is

partitions does not yet work however, any insights are welcome.

- Added a "root" vnode to the io_context. It is used for resolving
paths and converting nodes to paths instead of sRoot. Some more
passing around of io_context structures was necessary.
- Introduced a new lock sIOContextRootLock to protect
io_context::root. The current uses of io_context::io_mutex
(put_vnode(), remove_vnode() while holding it) looked too suspicious
to use that mutex in vnode_path_to_vnode().
- Added _kern_change_root() syscall and chroot() libroot function.
- Added chroot coreutils program to the image. Funnily it seems to be
much easier to set up a little jail than under Linux (just copy
bash and libroot.so into respective subdirs; mount another pipefs
if you want pipe support).
With Haiku allowing direct access to directories via inode IDs
jailing is obviously not very secure at the moment.
- Added /var/empty to the image. It will be the chroot target for ssh.

- The kernel stores the group IDs in the team structure. They are
correctly inherited on fork() and load_image_etc().
- Implemented getgroups() for real, i.e. it retrieves the groups
associated with the process.
- Implemented setgroups(), initgroups() and (the BSDish)

"group database" /etc/group (if existing).

finally created a solution to avoid that: Header files that contain
configuration settings (and nothing else) go to build/config_headers.
To change settings, create a directory build/user_config_headers (which
is ignored by svn), copy the respective header there and modify it at
your leisure. Currently only tracing_config.h has been moved to the new
location, but more files will follow eventually. It is also recommended
to move optional macro definitions in Jamfile (as for BFS) to a config
header instead; the build system will then automatically rebuild on
changes.

* Added team::flags. Currently only used for setting a flag when a team
has exec()ed.
* Some improvements of _user_setpgid():
- It failed incorrectly when the target process was a process group
leader. According to the standard it shall fail when the process is
a session leader. Moving a process group leader to another process
group is fine, even if that leaves the group leaderless.
- Fixed race conditions. We need to recheck the error conditions when
we hold the team spinlock. Otherwise the situation could change
while we allocated the new process group. This was one of the
reasons for bug #1799 -- after the shell fork()'s both parent and
child invoke setpgid() for the child.
- Fixed behavior for pid == pgid. It doesn't necessarily mean that a
new group has to be created.
- Fixed update of target process group orphaned state.
- Squashed TODO: setpgid() on a child is supposed to fail after the
child has exec()ed.

This eliminates the edge case where the grow thread would not be able to create
a new area because no memory could be allocated for the allocation of the area.
As this case cannot happen anymore, it is also not possible to deadlock in
memalign. Therefore the timeout (which would only have prevented the deadlock
but wouldn't have solved the edge case anyway) has been removed too.
Add options to dump the dedicated grow heap and to only print the current heap
count to the "heap" debugger command.

but there's a special handling for 0 us relative timeouts. Syscalls
usually return B_WOULD_BLOCK instead of B_TIMED_OUT in this case, and

* Implemented automatic syscall restarts:
- A syscall can indicate that it has been interrupted and can be
restarted by setting a respective bit in thread::flags. It can
store parameters it wants to be preserved for the restart in
thread::syscall_restart::parameters. Another thread::flags bit
indicates whether it has been restarted.
- handle_signals() clears the restart flag, if the handled signal
has a handler function installed and SA_RESTART is not set. Another
thread flag (THREAD_FLAGS_DONT_RESTART_SYSCALL) can prevent syscalls
from being restarted, even if they could be (not used yet, but we
might want to use it in resume_thread(), so that we stay
behaviorally compatible with BeOS).
- The architecture specific syscall handler restarts the syscall, if
the restart flag is set. Implemented for x86 only.
- Added some support functions in the private <syscall_restart.h> to
simplify the syscall restart code in the syscalls.
- Adjusted all syscalls that can potentially be restarted accordingly.
- _user_ioctl() sets new thread flag THREAD_FLAGS_IOCTL_SYSCALL while
calling the underlying FS's/driver's hook, so that syscall restarts
can also be supported there.
* thread_at_kernel_exit() invokes handle_signals() in a loop now, as
long as the latter indicates that the thread shall be suspended, so
that after waking up signals received in the meantime will be handled
before the thread returns to userland. Adjusted handle_signals()
accordingly -- when encountering a suspending signal we don't check
for further signals.
* Fixed sigsuspend(): Suspending the thread and rescheduling doesn't
result in the correct behavior. Instead we employ a temporary
condition variable and interruptably wait on it. The POSIX test
suite test passes, now.
* Made the switch_sem[_etc]() behavior on interruption consistent.
Depending on when the signal arrived (before the call or when already
waiting) the first semaphore would or wouldn't be released. Now we
consistently release it.
* Refactored _user_{read,write}[v]() syscalls. Use a common function for
either pair. The iovec version doesn't fail anymore, if anything could
be read/written at all. It also checks whether a complete vector
could be read/written, so that we won't skip data, if the underlying
FS/driver couldn't read/write more ATM.
* Some refactoring in the x86 syscall handler: The int 99 and sysenter
handlers use a common subroutine to avoid code duplication.

* Tracing of allocations, reallocations and frees
* Leak checking infrastructure to dump allocations
The leak checking code records the team and thread id when an allocation is
made as well as stores the originally requested size. It also adds the
"allocations" debugger command that can dump all current allocations (usually
a huge list) or filter by either a team or thread id. This way it's easily
possible to find leftover allocations of no more active teams/threads.
Combined with the tracing support one might be able to track down the time and
reason of an allocation and possibly find the corresponding leak if it is one.
Note that kernel heap leak checking has to be enabled manually by setting the
KERNEL_HEAP_LEAK_CHECK define to 1.

and pages are now kept in lists as well. This allows to return free pages once
a bin does not need them anymore. Partially filled pages are kept in a sorted
linked list so that allocation will always happen on the fullest page - this
favours having full pages and makes it more likely lightly used pages will get
completely empty so they can be returned. Generally this now goes more in the
direction of a slab allocator.
The allocation logic has been extracted, so a heap is now simply attachable to
a region of memory. This allows for multiple heaps and for dynamic growing. In
case the allocator runs out of free pages, an asynchronous growing thread is
notified to create a new area and attach a new heap to it.
By default the kernel heap is now set to 16MB and grows by 8MB each time all
heaps run full.
This should solve quite a few issues, like certain bins just claiming all pages
so that even if there is free space nothing can be allocated. Also it obviously
does aways with filling the heap page by page until it overgrows.
I think this is now a well performing and scalable allocator we can live with
for quite some time. It is well tested under emulation and real hardware and
performs as expected. If problems come up there is an extensive sanity checker
that can be enabled by PARANOID_VALIDATION that covers most aspects of the
allocator. For normal operation this is not necessary though and is therefore
disabled by default.

I'm still not fully happy with the command. It should be possible to
define a filter and then comfortably scroll through the matching
entries. Currently having to specify an index range of the unfiltered
entries is rather unhandy in combination with filtering.

that is a little more powerful than BeOS'. It features:
- Persistent and temporary uint64 variables. The former kind is set
only by the user. The latter (those prefixed "_") can be set
automatically by commands, thus e.g. making it easier to access
members of a dumped structure. They are unset when the next command
is invoked. The special temporary variable "_" is defined as a
command's return value.
- Expressions can contain nested command invocations using brackets
("[ ... ]").
- Command lines are parsed by the expression parser, too. They can
contain command invocations (in brackets) and expressions (in
parentheses).
* Added debugger commands:
- expr: Evaluates the given expression and prints the result.
- unset: Undefines a variable.
- vars: Prints the values of all defined variables.
* Moved debugger command code into its own source file.

As (our) gcc unfortunately uses atexit() to clean up lazily initialized static
variables inside functions we have to ensure that we do the right thing with
unloadable shared objects. In case a shared object was unloaded that installed
an atexit() hook the application would crash on exit. We now implement a
callback into libroot that is used to call all the atexit() hooks of a
component that is to be unloaded. Most prominently this fixes the media_server
crash at shutdown.

* vm_cache_resize() will clear that flag, and the writer will check it
again after it wrote back the page (which will fail when it's outside
the file bounds), and if it's cleared, it will get rid of the page
(if the file has been resized again in the mean time, writing it will
succeed then, and we'll keep the page around).

* Changed the way the file cache works: instead of reading/writing to the
underlying device directly, it can now be used for any data source, ie.
also network file systems.
* As a result, the former pages_io() moved to the VFS layer, and can now be
called by a file system via {read|write}_file_io_vec_pages() (naming
suggestions are always welcomed :-)). It now gets an FD, and uses that to
communicate with the device (via its fs_{read|write}_pages() hooks).
* The file_cache_{read|write}() functions must now be called without holding
an I/O relevant file system lock. That allows the file cache to prepare the
pages without colliding with the page writer, IOW the "mayBlock" flag can
go into the attic again (yay!).
* This also results in a much better performance when the system does I/O and
is low on memory, as the page writer can now finally write back some pages,
and that even without maxing out the CPU :)
* The API changes put slightly more burden on the fs_{read|write}_pages()
hooks, but in combination with the file_map it's still pretty straight
forward. It just will have to dispatch the call to the underlying device
directly, usually it will just call its fs_{read|write}_pages() hooks
via the above mentioned calls.
* Ported BFS and FAT to the new API, the latter has not been tested, though.
* Also ported the API changes to the fs_shell. I also completely removed its
file cache level page handling - the downside is that device access is no
longer cached (ie. depends on the host OS now), the upside is that the code
is greatly simplified.

handling in the cases where notifications during the operation make

vm_page_reserve_pages() and vm_page_allocate_page() will now steal pages from
the inactive queue as needed.
* We currently never steal active pages anymore, but this might need to be
revised later (therefore, the page scanner never waits anymore, but uses
mutex_trylock() to lock a cache).
* The page scanner and writer now both run at normal priority - let's see how
that will work out.
* Introduced an inactive queue.
* Instead of shuffling pages around in the queue (and therefore destroying LRU)
the page stealing mechanism now uses a marker page to be able to release the
page lock without losing its position in the queue.
* The page writer now always grabs the whole release count of the semaphore, so
that there won't be a huge backlog to catch up with.
* vm_page_num_free_pages() now also includes the inactive queue as well as the
reserved pages (they are no longer regarded as free pages).

* The page writer now pushes the pages of busy vnodes to the end of the queue,
so that it won't pick them up again too soon (the vnode destruction would
be in the process of writing those pages back, anyway).
* The page thief now triggers the page writer to run once it has to steal

* _user_get_partitionable_spaces() doesn't need to copy the buffer into
the kernel, since it is no input parameter. It also copies back the
actual partitionable spaces count on error, now -- B_BUFFER_OVERFLOW

returned too.
* Fixed several instances of syscall implementations that unloaded a disk
system, although they didn't load it in the first place. This screwed

interfere with the page thief, we always need to have reserved a page for
this upfront. I introduced a function to the vm_translation_map layer that
estimates how much pages a mapping might need at maximum. All functions that
map a page now call this and reserve the needed pages upfront.
It might not be a nice solution, but it works.
* The page thief could run into a panic when trying to call vm_cache_release_ref()
on a non-existing (NULL) cache.
* Also, it will now ignore wired active pages.
* There is still a race condition between the page writer and the vnode
destruction - writing a page back needs a valid vnode, but that might just
have been deleted.

system initialize() hooks. It's often the only info about the
partition one needs and thus locking the partition just to get it is
no longer necessary.
* intel partitioning system:
- Removed passing around block sizes. We require 512 byte sectors
anyway. In fact using the parent partition's block size was even
wrong.
- Simplified writing the partition map sector.
- Simplified and corrected the partition map initialization.
- We don't fail identifying a partition anymore, if the partition map
contains no partitions. We would never identify a freshly
initialized partition map before.
- Made pm_identify() more intelligent: It determines the priority to
return depending on whether the partition is the device itself and
whether we have recognized child partitions.

* Reworked the select support:
- The io_context additionally stores a table of lists of select_infos,
which enables it to deselect events of a pending select() when
closing a FD. This prevents a race condition potentially causing a
write to stale memory.
- The opaque selectsync* passed to FSs is now actually a select_info*.
This was necessary, since the FDs deselect() hook (unlike the
select() hook) doesn't take a "ref" argument and deselecting a
single info (e.g. caused by a premature close()) was not possible.
The select() hook's "ref" argument has become superfluous.
- It should now be relatively easy to implement a poll_on_steroids()
that can also wait for objects other than FDs (e.g. semaphores,
ports, threads etc.).
* Set/reset the signal mask in common_select(). This makes pselect()
work as required.
* Reorganized vfs_resize_fd_table().

* We now have a page writer that takes some pages from the modified queue
and writes it back every few seconds. It can be triggered by the page
scanner to do that more often, though. That mechanism can be greatly
improved once we have our I/O scheduler working.
* Removed vm_page_write_modified_page() again - it was all "eaten up" by
the page writer.
* Reworked vm_page_write_modified_pages() a bit: it now uses
vm_test_map_modification() and vm_clear_map_flags() instead of the
iterating over all areas which wouldn't even work correctly.

* Removed the vm_cache/vm_store ref_count duality that besides being a bit ugly
also created the page dameon cache retrieval problem: now, only areas (and
cache consumers) retrieve a reference to the store (and therefore, the vnode).
The page daemon doesn't need to care about this at all anymore, and the pseudo
references of the vm_cache could be removed again.
* Rearranged deletion of vnodes such that its ID can be reused directly after
fs_remove_vnode() has been called.
* vm_page_allocate_page() no longer panics when it runs out of pages, but just
waits for new pages to become available using the new sFreeCondition condition

Callers simply use MultiAddressSpaceLocker:: AddAreaCacheAndLock() to
do that. This resolves an open TODO, that the areas' base, size, and
protection fields were accessed without their address spaces being
locked.
* vm_copy_area() does now always insert a cache for the target area. Not
doing that would cause source and target area being attached to
the same cache in case the target protection was read-only. This
would make them behave like cloned areas, which would lead to trouble
when one of the areas would be changed to writable later.
* Fixed the !writable -> writable case in vm_set_area_protection(). It
would simply change the protection of all mapped pages for this area,
including ones from lower caches, thus causing later writes to the
area to be seen by areas that shouldn't see them. This fixes a problem
with software breakpoints in gdb. They could cause other programs to
be dropped into the debugger.
* resize_area() uses MultiAddressSpaceLocker::AddAreaCacheAndLock() now,
too, and could be compacted quite a bit.

* ConditionVariableEntry::Add() did not correctly insert the entry into
the per-thread list of entries (the next link of the previous entry
was not adjusted), which could leave the entry unnotified when the
previous entry was notified, thus leaving it in the respective
condition variable's list after the end of its life time. This should
fix a crashing bug I rarely encountered.
* Added debug checks in the PrivateConditionVariableEntry
constructor/destructor that should have helped me to find
forementioned bug hours earlier, had I been bright enough to realize
that I didn't include <debug.h> and those KDEBUG guarded checks were

dead_children, are used to track the state of stopped/continued

* A team does have a job_control_entry, which is allocated at team
creation time. It will be inserted into the parent's
{stopped,continued}_children lists as the team's main thread is
stopped/continued and removed when waitpid() retrieves the child
state. When the team dies the entry is detached from the team and goes
into the parent's dead_children list.
* Removed the wait_for_any field from team_dead_children. It was solely
used to avoid deletion of the contained entries in certain situations.
wait_for_child() (the waitpid() backend) always deletes an entry now,
regardless of whether other threads are waiting; that's in
accordance with the waidpid() specification. wait_for_thread() removes
the entry only, if the caller is the parent of the respective team.
* Introduced team_set_job_control_state() which performes the job
control entry transitions between the respective lists and wakes up
threads waiting in wait_for_child(). It is invoked on team death and
when the team's main thread receives job control signals.
* Reorganized wait_for_child(). It handles WCONTINUED and WUNTRACED now,
too. Removed a block that interpreted the supplied ID as thread ID.
* Added missing parts in waitpid().
Job control starts to work, though it seems to have some glitches.

This prevents a page from the lower (to be discarded) cache from still

The Haiku kernel seems now to run shockingly stable. ATM, we have more

* Images preloaded by the boot loader had to be modules to be of any use
to the kernel. Extended the mechanism so that any images not accepted
by the module code would later be tried to be added as drivers by the
devfs. This is a little hacky ATM, since the devfs manages the drivers
using a hash map keyed by the drivers inode ID, which those drivers
obviously don't have.
* The devfs emulates read_pages() using read(), if the device driver
doesn't implement the former (all old-style drivers), thus making it
possible to BFS, which uses the file cache which in turn requires
read_pages(), on the device. write_pages() emulation is still missing.
* Replaced the kernel_args::boot_disk structure by a KMessage, which can
more flexibly be extended and deals more gracefully with
arbitrarily-size data. The disk_identifier structure still exists,
though. It is added as message field in cases where needed (non net
boot). Moved the boot_drive_number field of the bios_ia32 platform
specific args into the message.
* Made the stage 1 PXE boot loader superfluous. Moved the relevant
initialization code into the stage 2 loader, which can now be loaded
directly via PXE.
* The PXE boot loader does now download a boot tgz archive via TFTP. It
does no longer use the RemoteDisk protocol (it could actually be
removed from the boot loader). It also parses the DHCP options in the
DHCPACK packet provided by PXE and extracts the root path to be
mounted by the kernel.
* Reorganized the boot volume search in the kernel (vfs_boot.cpp) and
added support for network boot. In this case the net stack is
initialized and the network interface the boot loader used is brought
up and configured. Since NBD and RemoteDisk are our only options for
net boot (and those aren't really configurable dynamically) ATM, the
the boot device is found automatically by the disk device manager.
Booting via PXE does work to some degree now. The most grievous problem
is that loading certain drivers or kernel modules (or related activity)
causes a reboot (likely a triple fault, though one wonders where our
double fault handler is on vacation). Namely the keyboard and mouse input
server add-ons need to be deactivated as well as the media server.
A smaller problem is the net server, which apparently tries to
(re-)configure the network interface we're using to boot, which
obviously doesn't work out that well. So, if all this stuff is disabled
Haiku does fully boot, when using the RemoteDisk protocol (not being
able to use keyboard or mouse doesn't make this a particular fascinating
experience, though ;-)). I had no luck with NBD -- it seemed to have
protocol problems with the servers I tried.
