9623c48284 SMP: remove the tracking of apic id -> cpu id. Don't pass between bootloader and kernel.
Kernel doesn't use it, and it could be regenerated in the kernel if it did need it.

This also unlocks the apic range the bios can use. Previously the apic ids would have
to fit within 0..MAX_CPUS or it'd reject the cpu. Some boxes (mine in particular)
seem to sparsely populate the apic id so that the range is pretty large.

5aa0503c7c * Removed i386_translation_map_get_pgdir() and adjusted the one place where it
was used.
* Renamed X86VMTranslationMap to X86VMTranslationMap32Bit and pulled the paging
  method agnostic part into new base class X86VMTranslationMap.
* Moved X86PagingStructures into its own header/source pair.
* Moved pgdir_virt from X86PagingStructures to X86PagingStructures32Bit where
  it is actually used.

* Renamed i386_context_switch() to x86_context_switch().
* x86_context_switch() no longer sets the page directory.
  arch_thread_context_switch() does that explicitly, now. This allows to solve
  the TODO by reordering releasing the previous paging structures reference and
  setting the new page directory.

9de17be600 * Moved I/O APIC initialization to a later point, now the APCI and DPC modules
can safely be used.
* Since using the I/O APIC is disabled by default, I've removed the "return"
  that prevented its use when enabled. Let's see if it already does anything.
* Adapted other arch_int.cpp with a bit of cleanup.

33fbe25496 * Add code to allocate and free interrupt vectors for message signaled
interrupts (MSI).
* Add the remaining IDT entries and redirection functions in the interrupt code.
* Make the PIC end_of_interrupt() return a result to indicate whether the vector
  was handled by this PIC. If it isn't we now issue a apic_end_of_interrupt()
  in the assumption of apic local interrupt, MSI or IPI. This also removes
  the need for the gUsingIOAPIC global and doing manual apic_end_of_interrupt()
  calls in the SMP and timer code.

655f3b4161 Seperate the generic (local) APIC stuff into it's own file and use it from the
other places where previously the same functionality was duplicated. Also
seperated the header which was originally arch_smp.h into apic.h and arch_smp.h
again as some of it is MP and not actually APIC.

feddedab0c x86:
Added fields for temporary storage of the debug registers dr6 and dr7 to the
arch_cpu_info structure. The actual registers are stored at the beginning of
x86_exit_user_debug_at_kernel_entry() and read in
x86_handle_debug_exception().
The problem was that x86_exit_user_debug_at_kernel_entry() itself overwrote
dr7 and, if kernel breakpoints were enabled, dr6 could be overwritten anytime
after. So x86_handle_debug_exception() would find incorrect values in the
registers (definitely in dr7) and thus interpret the detected debug condition
incorrectly. Usually watchpoints were recognized as breakpoints.

cfefeee3f8 * Changed arch_debug_save_registers() to take an arch-specific structure
arch_debug_registers instead.
* Call arch_debug_save_registers() on all CPUs when entering the kernel
  debugger.
* Added debug_get_debug_registers() to return a specified CPU's saved
  registers.
* x86:
  - Replaced the previous arch_debug_save_registers() implementation. Disabled
    getting the registers via the gdb interface for the time being.
  - Fixed the "sc", "call", and "calling" commands to also work for threads
    running on another CPU.

d40a935560 Boot loader (x86 mmu.cpp):
* Made the page table allocation more flexible. Got rid of sMaxVirtualAddress
  and added new virtual_end address to the architecture specific kernel args.
* Increased the virtual space we reserve for the kernel to 16 MB. That
  should suffice for quite a while. The previous 2 MB were too tight when
  building the kernel with debug info.
* mmu_init(): The way we were translating the BIOS' extended memory map to
  our physical ranges arrays was broken. Small gaps between usable memory
  ranges would be ignored and instead marked allocated. This worked fine for
  the boot loader and during the early kernel initialization, but after the
  VM has been fully set up it frees all physical ranges that have not been
  claimed otherwise. So those ranges could be entered into the free pages
  list and would be used later. This could possibly cause all kinds of weird
  problems, probably including ACPI issues. Now we add only the actually
  usable ranges to our list.

Kernel:
* vm_page_init(): The pages of the ranges between the usable physical memory
  ranges are now marked PAGE_STATE_UNUSED, the allocated ranges
  PAGE_STATE_WIRED.
* unmap_and_free_physical_pages(): Don't free pages marked as unused.

dac21d8bfe * map_physical_memory() does now always set a memory type. If none is given (it
needs to be or'ed to the address specification), "uncached" is assumed.
* Set the memory type for the "BIOS" and "DMA" areas to write-back. Not sure, if
  that's correct, but that's what was effectively used on my machines before.
* Changed x86_set_mtrrs() and the CPU module hook to also set the default memory
  type.
* Rewrote the MTRR computation once more:
  - Now we know all used memory ranges, so we are free to extend used ranges
    into unused ones in order to simplify them for MTRR setup.
  - Leverage the subtractive properties of uncached and write-through ranges to
    simplify ranges of any other respectively write-back type.
  - Set the default memory type to write-back, so we don't need MTRRs for the
    RAM ranges.
  - If a new range intersects with an existing one, we no longer just fail.
    Instead we use the strictest requirements implied by the ranges. This fixes
    #5383.

Overall the new algorithm should be sufficient with far less MTRRs than before
(on my desktop machine 4 are used at maximum, while 8 didn't quite suffice
before). A drawback of the current implementation is that it doesn't deal with
the case of running out of MTRRs at all, which might result in some ranges
having weaker caching/memory ordering properties than requested.

bcc2c157a1 Refactored vm_translation_map:
* Pulled the physical page mapping functions out of vm_translation_map into
  a new interface VMPhysicalPageMapper.
* Renamed vm_translation_map to VMTranslationMap and made it a proper C++
  class. The functions in the operations vector have become methods.
* Added class GenericVMPhysicalPageMapper implementing VMPhysicalPageMapper
  as far as possible (without actually writing new code).
* Adjusted the x86 and the PPC specifics accordingly (untested for the
  latter). For the other architectures the build is, I'm afraid, seriously
  broken.

The next steps will modify and extend the VMTranslationMap interface, so that
it will be possible to fix the bugs in vm_unmap_page[s]() and employ
architecture specific optimizations.

- Replaced the "userOnly" parameter by a "flags" parameter, that allows to
    specify kernel and userland stack traces individually.
  - x86, m68k: Don't always skip the first frame as that prevents the caller
    from being able to record its own address.
* capture_tracing_stack_trace(): Replaced the "userOnly" parameter by
  "kernelOnly", since one is probably always interested in the kernel stack
  trace, but might not want the userland stack trace.
* Added stack trace support for VM cache kernel tracing.

34a48c70ef Added type nanotime_t (an int64 storing a nanoseconds value) and function
system_time_nsecs(), returning the system time in nanoseconds. The function
is only really implemented for x86. For the other architectures
system_time() * 1000 is returned.

bb163c0289 * Added a set_mtrrs() hook to x86_cpu_module_info, which is supposed to set
all MTRRs at once.
* Added a respective x86_set_mtrrs() kernel function.
* x86 CPU module:
  - Implemented the new hook.
  - Prefixed most debug output with the CPU index. Otherwise it gets quite
    confusing with multiple CPUs.
  - generic_init_mtrrs(): No longer clear all MTRRs, if they are already
    enabled. This lets us benefit from the BIOS's setup until we install our
    own -- otherwise with caching disabled things are *really* slow.
* arch_vm.cpp: Completely rewrote the MTRR handling as the old one was not
  only slow (O(2^n)), but also broken (resulting in incorrect setups (e.g.
  with cachable ranges larger than requested)), and not working by design for
  certain cases (subtractive setups intersecting ranges added later).
  Now we maintain an array with the successfully set ranges. When a new range
  is added, we recompute the complete MTRR setup as we need to. The new
  algorithm analyzing the ranges has linear complexity and also handles range
  base addresses with an alignment not matching the range size (e.g. a range
  at address 0x1000 with size 0x2000) and joining of adjacent/overlapping
  ranges of the same type.

This fixes the slow graphics on my 4 GB machine (though unfortunately the
8 MTRRs aren't enough to fully cover the complete frame buffer (about 35
pixel lines remain uncachable), but that can't be helped without rounding up
the frame buffer size, for which we don't have enough information). It might
also fix #1823.

ea40a61a84 Some big changes, still not working on my laptop, since hpet interrupts
aren't routed correctly over the 8259, it seems.
- Removed passing the hpet_regs around, since there's a static variable.
- Added lots of debug dprintfs.
- Fixed setting the timer interrupt to edge
- Timer is initialized once.
- Use the timer 0 instead of 2.
- Renamed register definitions to be more readable
- Use 64 bits registers and unions where applicable.
- Other things I don't remember

ea2abd110b * Renamed the ROUNDOWN macro to ROUNDDOWN. Also changed the implementation of
ROUNDUP to use '*' and '/' -- the compiler will optimize that for powers of
  two anyway and this implementation works for other numbers as well.
* The thread::fault_handler use in C[++] code was broken with gcc 4. At least
  when other functions were invoked. Trying to trick the compiler wasn't a
  particularly good idea anyway, since the next compiler version could break
  the trick again. So the general policy is to use the fault handlers only in
  assembly code where we have full control. Changed that for x86 (save for the
  vm86 mode, which has a similar mechanism), but not for the other
  architectures.
* Introduced fault_handler, fault_handler_stack_pointer, and fault_jump_buffer
  fields in the cpu_ent structure, which must be used instead of
  thread::fault_handler in the kernel debugger. Consequently user_memcpy() must
  not be used in the kernel debugger either. Introduced a debug_memcpy()
  instead.
* Introduced debug_call_with_fault_handler() function which calls a function
  in a setjmp() and fault handler context. The architecture specific backend
  arch_debug_call_with_fault_handler() has only been implemented for x86 yet.
* Introduced debug_is_kernel_memory_accessible() for use in the kernel
  debugger. It determines whether a range of memory can be accessed in the
  way specified. The architecture specific back end
  arch_vm_translation_map_is_kernel_page_accessible() has only been implemented
  for x86 yet.
* Added arch_debug_unset_current_thread() (only implemented for x86) to unset
  the current thread pointer in the kernel debugger. When entering the kernel
  debugger we do some basic sanity checks of the currently set thread structure
  and unset it, if they fail. This allows certain commands (most importantly
  the stack trace command) to avoid accessing the thread structure.
* x86: When handling a double fault, we do now install a special handler for
  page faults. This allows us to gracefully catch faulting commands, even if
  e.g. the thread structure is toast.

We are now in much better shape to deal with double faults. Hopefully avoiding
the triple faults that some people have been experiencing on their hardware
and ideally even allowing to use the kernel debugger normally.

671a2442d9 More work towards making our double fault handler less triple fault prone:
* SMP:
  - Added smp_send_broadcast_ici_interrupts_disabled(), which is basically
    equivalent to smp_send_broadcast_ici(), but is only called with interrupts
    disabled and gets the CPU index, so it doesn't have to use
    smp_get_current_cpu() (which dereferences the current thread).
  - Added cpu index parameter to smp_intercpu_int_handler().
* x86:
  - arch_int.c -> arch_int.cpp
  - Set up an IDT per CPU. We were using a single IDT for all CPUs, but that
    can't work, since we need different tasks for the double fault interrupt
    vector.
  - Set the per CPU double fault task gates correctly.
  - Renamed set_intr_gate() to set_interrupt_gate and set_system_gate() to
    set_trap_gate() and documented them a bit.
  - Renamed double_fault_exception() x86_double_fault_exception() and fixed
    it not to use smp_get_current_cpu(). Instead we have the new
    x86_double_fault_get_cpu() that deducts the CPU index from the used stack.
  - Fixed the double_fault interrupt handler: It no longer calls int_bottom to
    avoid accessing the current thread.
* debug.cpp:
  - Introduced explicit debug_double_fault() to enter the kernel debugger from
    a double fault handler.
  - Avoid using smp_get_current_cpu().
  - Don't use kprintf() before sDebuggerOnCPU is set. Otherwise
    acquire_spinlock() is invoked by arch_debug_serial_puts().

Things look a bit better when the current thread pointer is broken -- we run
into kernel_debugger_loop() and successfully print the "Welcome to KDL"
message -- but we still dereference the thread pointer afterwards, so that we
don't get a usable kernel debugger yet.

cc77aba101 * Allocate a separate double fault stack for each CPU.
* Added x86_double_fault_get_cpu(), a save way to get the CPU index when in
  the double fault handler. smp_get_current_cpu() requires at least a somewhat
  intact thread structure, so we rather want to avoid it when handling a double
  fault. There are a lot more of those dependencies in the KDL entry code.
  Working on it...

0b11ecb18c * Always include the public arch_debugger.h headers. The structures defined
there are prefixed with the respective architecture name. Useful for remote
  debugging a different architecture.
* <x86/arch_debugger.h>: Introduced a structure for the FPU state, so that it
  isn't left to the debugger.
* Removed the _kern_get_thread_cpu_state() syscall. Was originally intended for
  bdb compatiblity, but isn't really needed.
* Kernel x86 arch_get_debug_cpu_state(): The use of fnsave was broken, since
  it reinits the FPU after saving the state. This resulted in weird results
  when debugging functions using the FPU. We now use fxsave, if available.
  Otherwise fnsave + frstor should be used -- not fully implemented yet.
  Same for arch_set_debug_cpu_state().

b0f12d64f4 Added transparent software breakpoint support for user debuggers:
* The bulk of the work -- i.e. juggling the software and hardware breakpoints,
  watchpoints, and memory reads/writes -- is done in the new class
  BreakpointManager.
* For the architectures a few capability macros have to be defined, one
  pointing to the software breakpoint instruction opcode. Done for x86.
* Some more simplifications in the user debugger code, made possible by the
  recently introduced debugger_changed_condition attribute.

568ade58d0 User debugger support:
* Generalized address checks. The debugger can now also read the commpage.
* Added new syscall _kern_get_thread_cpu_state() to get the CPU state of a
  not running thread. Introduced arch_get_thread_debug_cpu_state() for that
  purpose, which is only implemented for x86 ATM (uses the new
  i386_get_thread_user_iframe()).
* Don't allow a debugger to change a thread's "esp" anymore. That's the esp
  register in the kernel. "user_esp" can still be changed.
* Generally set RF (resume flag) in eflags in interrupt handlers, not only
  after a instruction breakpoint debug exception. This should prevent
  breakpoints from being triggered more than once (e.g. when the breakpoint is
  on an instruction that can cause a page fault). I still saw those with bdb
  in VMware, but that might be a VMware bug.

7515c32c1f Not sure, if this was only a gcc 4.3.3 bug or if I misunderstand something, but
gcc could apparently assume that the register assigned to the one in the
clobber list would keep its value (as can be observed when disassembling
add_debugger_command_etc()).
Using a dummy output register works around the problem and also avoids the
unnecessary initialization of the register.

Comments explaining the mystery welcome.

8b3b05cbf9 Synchronize the TSCs of all CPUs early in the boot process, so system_time()
will return consistent values. This helps with debug measurements for the time
being. Obviously we'll have to think of something different when we support
speed-stepping on models with frequency-dependent TSCs.

9a42ad7a77 When switching to a kernel thread we no longer set the page directory.
This is not necessary, since userland teams' page directories also
contain the kernel mappings, and avoids unnecessary TLB flushes. To make
that possible the vm_translation_map_arch_info objects are reference
counted now.

This optimization reduces the kernel time of the Haiku build on my
machine with SMP disabled a few percent, but interestingly the total
time decreases only marginally. Haven't tested with SMP yet, but for
full impact CPU affinity would be needed.

47c40a10a1 * Prefixed memset_physical() and memcpy_to_physical() with "vm_",
added vm_memcpy_from_physical() and vm_memcpy_physical_page(), and
  added respective functions to the vm_translation_map operations. The
  architecture specific implementation can now decide how to implement
  them most efficiently. Added generic implementations that can be used,
  though.
* Changed vm_{get,put}_physical_page(). The former no longer accepts
  flags (the only flag PHYSICAL_PAGE_DONT_WAIT wasn't needed anymore).
  Instead it returns an implementation-specific handle that has to be
  passed to the latter. Added vm_{get,put}_physical_page_current_cpu()
  and *_debug() variants, that work only for the current CPU,
  respectively when in the kernel debugger. Also adjusted the
  vm_translation_map operations accordingly.
* Made consequent use of the physical memory operations in the source
  tree.
* Also adjusted the m68k and ppc implementations with respect to the
  vm_translation_map operation changes, but they are probably broken,
  nevertheless.
* For x86 the generic physical page mapper isn't used anymore. It is
  suboptimal in any case. For systems with small memory it is too much
  overhead, since one can just map the complete physical memory (that's
  not done yet, though). For systems with large memory it counteracts
  the VM strategy to reuse the least recently used pages. Since those
  pages will most likely not be mapped by the page mapper anymore, it
  will keep remapping chunks. This was also the reason why building
  Haiku in Haiku was significantly faster with only 256 MB RAM (since
  that much could be kept mapped all the time).
  Now we're using a different strategy: We have small pools of virtual
  page slots per CPU that are used for the physical page operations
  (memset_physical(), memcpy_*_physical()) with CPU-pinned thread.
  Furthermore we have four slots per translation map, which are used to
  map page tables.

These changes speed up the Haiku image build in Haiku significantly. On
my Core2 Duo 2.2 GHz 2 GB machine about 40% to 20 min 40 s (KDEBUG
disabled, block cache debug disabled). Still more than factor 3 slower
than FreeBSD and Linux, though.

74785e79db * Added "from" address space parameter to vm_swap_address_space()/
arch_vm_aspace_swap().
* The x86 implementation does now maintain a bit mask per
  vm_translation_map_arch_info indicating on which CPUs the address
  space is active. This allows flush_tmap() to avoid ICI for user
  address spaces when the team isn't currently running on any other CPU.
  In this context ICI is relatively expensive, particularly since we map
  most pages via vm_map_page() and therefore invoke flush_tmap() pretty
  much for every single page.
  This optimization speeds up a "hello world" compilation about 20% on
  my machine (KDEBUG turned off, freshly booted), but interestingly it
  has virtually no effect on the "-j2" haiku build time.

18f2a9c17d Added support for architecture specific debug variables, e.g. for
referencing iframe registers. Their prefix is "$". E.g. "$eax" refers to
the eax register of the current iframe. The features cooperates with the
"in_context" command, i.e. "in_context 92 $eip = 0" will set the eip
register of thread 92 to 0 (thus sealing its fate ;-)).

bca3215f8a * Introduced x86_get_double_fault_stack(), which returns the address
and size of the double fault stack.
* is_kernel_stack_address() does now also check whether the given
  address is on the double fault stack. This fixes stack traces on

69dddbdea1 Initial work towards supporting IO APICs. This cannot work for PCI interrupts
though until we use ACPI for proper PCI IRQ routing through the IO APIC.
Therefore the IO APIC code path is not yet enabled and the IO APIC isn't used.
ISA interrupts would work though, as would PCI interrupts if you'd hardcode
them for your specific configuration.
Note that this change also modifies some parts in the bootloader and in the PIC
setup to make local APICs available even on non-SMP systems. This causes APIC
timers to be used instead the normal PIT if it is available (also on non-SMP
configurations).
Also fixes some general errors in SMP and PIC code as well as some code cleanup.

6b202f4e3d * Introduced new header directory headers/private/system which is supposed
to contain headers shared by kernel and userland (mainly libroot).
* Moved quite a few private kernel headers to the new location. Split
  several kernel headers into a shared part and one that is still kernel
  private. Adjusted all affected Jamfiles and source in the standard x86
  build accordingly. The build for other architectures and for test code
  may be broken.
* Quite a bit of userland code still includes private kernel headers.
  Mostly those are <util/*> headers. The ones that aren't strictly
  kernel-only should be moved to some other place (maybe
  headers/private/shared/util).

d648afb8d7 * For each userland team the kernel creates an area in the userland
address space that is fully locked and marked B_KERNEL_AREA. It can
  thus be accessed by the kernel without additional checks.
* For each userland thread we do create a user_thread structure in that
  area. The structure is accessible from userland via TLS, using the
  private get_user_thread() function.
* Introduced private userland functions [un]defer_signals(). They can be
  used to cheaply disable/re-enable signal delivery. They use the
  user_thread::defer_signals/pending_signals fields which are
  checked/updated by the kernel.

* Implemented automatic syscall restarts:
  - A syscall can indicate that it has been interrupted and can be
    restarted by setting a respective bit in thread::flags. It can
    store parameters it wants to be preserved for the restart in
    thread::syscall_restart::parameters. Another thread::flags bit
    indicates whether it has been restarted.
  - handle_signals() clears the restart flag, if the handled signal
    has a handler function installed and SA_RESTART is not set. Another
    thread flag (THREAD_FLAGS_DONT_RESTART_SYSCALL) can prevent syscalls
    from being restarted, even if they could be (not used yet, but we
    might want to use it in resume_thread(), so that we stay
    behaviorally compatible with BeOS).
  - The architecture specific syscall handler restarts the syscall, if
    the restart flag is set. Implemented for x86 only.
  - Added some support functions in the private <syscall_restart.h> to
    simplify the syscall restart code in the syscalls.
  - Adjusted all syscalls that can potentially be restarted accordingly.
  - _user_ioctl() sets new thread flag THREAD_FLAGS_IOCTL_SYSCALL while
    calling the underlying FS's/driver's hook, so that syscall restarts
    can also be supported there.
* thread_at_kernel_exit() invokes handle_signals() in a loop now, as
  long as the latter indicates that the thread shall be suspended, so
  that after waking up signals received in the meantime will be handled
  before the thread returns to userland. Adjusted handle_signals()
  accordingly -- when encountering a suspending signal we don't check
  for further signals.
* Fixed sigsuspend(): Suspending the thread and rescheduling doesn't
  result in the correct behavior. Instead we employ a temporary
  condition variable and interruptably wait on it. The POSIX test
  suite test passes, now.
* Made the switch_sem[_etc]() behavior on interruption consistent.
  Depending on when the signal arrived (before the call or when already
  waiting) the first semaphore would or wouldn't be released. Now we
  consistently release it.
* Refactored _user_{read,write}[v]() syscalls. Use a common function for
  either pair. The iovec version doesn't fail anymore, if anything could
  be read/written at all. It also checks whether a complete vector
  could be read/written, so that we won't skip data, if the underlying
  FS/driver couldn't read/write more ATM.
* Some refactoring in the x86 syscall handler: The int 99 and sysenter
  handlers use a common subroutine to avoid code duplication.

34b3b26b3b Merged branch haiku/branches/developer/bonefish/optimization revision
23139 into trunk, with roughly the following changes (for details svn
log the branch):
* The int 99 syscall handler is now fully in assembly.
* Added a sysenter/sysexit handler and use it on Pentiums that support
  it (via commpage).
* Got rid of i386_handle_trap(). A bit of functionality was moved into
  the assembly handler which now uses a jump table to call C functions
  handling the respective interrupt.
* Some optimizations to get user debugger support code out of the
  interrupt handling path.
* Introduced a thread::flags fields which allows to skip handling of
  rare events (signals, user debug enabling/disabling) on the
  common interrupt handling path.
* Got rid of the explicit iframe stack. The iframes can still be
  retrieved by iterating through the stack frames.
* Made the commpage an architecture independent feature. It's used for
  the real time data stuff (instead of creating a separate area).
* The x86 CPU modules can now provide processor optimized versions for
  common functions (currently memcpy() only). They are used in the
  kernel and are provided to the userland via commpage entries.
* Introduced build system feature allowing easy use of C structure
  member offsets in assembly code.

Changes after merging:
* Fixed merge conflict in src/system/kernel/arch/x86/arch_debug.cpp
  (caused by refactoring and introduction of "call" debugger command).

f108445b5e * Initial support for ACPI tables to detect multiprocessor configurations
* ACPI is evaluated first as it also handles things like multi core or hyper threading setups
* Removed other (disabled) hyper threading code per the notes in the corresponding ToDo
* Limit the detected CPU count to 2 for now as I wasn't able to get it working in either emulation nor real hardware with more than 2 CPUs
* Added a reserved byte to the mp_config_table struct, it worked only by luck as the compiler did padding there to get to the same size

I can now boot my Core 2 Quad with two out of four processors active :-)

We will use SRP (supervisor root pointer) register to hold the current thread pointer.
Its use by the pmmu is optional and I don't plan on using it.
There are 32 other bits left.
Wonder if weshouldn't use it for system_time() instead... will see, but there are no other usable regs.

to the private VM types are including vm_types.h now.
* Removed vm_page, vm_area, vm_cache, and vm_address_space typedefs; it's

* Made commpage.h self-containing.

667f1eeb67 * Added support for setting (hardware) break-/watchpoints in the kernel.
Can be enabled by defining KERNEL_BREAKPOINTS in arch/user_debugger.h
  and will provide the arch_{set,clear}_kernel_{break,watch}point()
  function. Hitting a break-/watchpoint will throw the thread into KDL.
* Finally added a comment, what's the point of
  i386_reinit_user_debug_after_context_switch(), since I wonder every
  time I see it. Should be optimized aways soon.

1cbf8f4b3c initial support for a commpage, which is a chunk of memory in high kernel space with user readonly permissions.
The first use is to let the kernel decide what the preferred syscall mechanism is at boot time and copy the
appropriate user space code there. Can be used for routines the kernel can decide best how to use (memcpy, some
timing routines, etc).

badc7b674e yet another fix for #1018, which has at this point blossomed into a reorg of how AP cpus are initialized.
the new cpuid stuff was apparently exacerbating an existing problem where various bits of low level
cpu code (specifically get_current_cpu) weren't really initialized before being used. Changed the
order to set up a fake set of threads to point each cpu at really early in boot to make sure that at
all points in code it can get the current 'thread' and thus the current cpu.
A probably better solution would be to have dr3 point to the current cpu which would then point to the 
current thread, but that has a race condition that would require an int disable, etc.

dfb5375d18 clean up TSS initialization.
Now two complete tss structures exist within the per-cpu structure. Instead
of having to create a seperate area per each one, initialize them in place.
Also, the old mechanism to getting all of the cpus to get initialized was 
subtly broken, but still managed to work. Now, just force all the cpus to
initialize at boot, which makes the actual swapping of esp0 somewhat simpler.

dcdc4f4b43 pulled over some stuff from newos:
at boot, per cpu, detect the cpu, pull down all the relevant cpuid bits and
save them into the per-cpu structure. Changed most of the code scattered here
and there that reads the cpuid to use a new api, x86_check_feature, which looks
at the saved bits.
Also changed the system_info stuff to read from these bits.
While i was at it, refreshed all the bits to be current.

8fc075ac5c * There was no reason to copy the "userland calls exit_thread()" stub with interrupts
turned off - accessing userland memory. Now, arch_thread_enter_userspace() does that
  job, and as a result, may also fail.
* dump_thread() now directly prints the info of the current thread when used without

d51ce54011 * Added the opportunity to add temporary debug interrupt handlers for
arch dependent code (they will be removed as soon as someone else
  asks for these interrupt lines).
* Added an interrupt driven keyboard handler to the kernel that uses
  this technique. As a result, you can now press F12 to enter the kernel
  debugger before the input_server has been started, and Control-Alt-Delete
  should reboot the system (actually I did not test the latter yet).

f4b0f67cf6 Added [arch_]int_post_device_manager() which is invoked after
the device manager is initialized. For x86 it does nothing, but
for PPC it searches for a supported interrupt controller and
remembers it for later use.
arch_int_{enable,disable}_io_interrupt() are implemented as
well as handling of external exceptions (aka as I/O interrupts).
We'll see later how well that works.

7afc16f059 * Fixed a bug regarding storing/restoring FPSCR in the interrupt
code. The stack pointer was not adjusted, hence we were
  overwriting the previous register value. But it looks like I
  missed to check in the arch_cpu.h with the iframe structure
  including the floating point registers anyway.
* Backported the ELF PPC relocation code from the boot loader to
  the kernel.
* Fixed the PPC version of arch_thread_switch_kstack_and_call().
  Apparently the signature had changed, but the assembly
  implementation was not adjusted accordingly.
* sc prints more registers now (LR, CR, CTR, XER,...).
* Fixed several occurences of not-working fault handlers.
  Apparently the compiler realized, that the "error" label was
  never jumped to (by the code it knew), and optimized the
  respective code away. Now we use a trick to make it think the
  error label might actually be jumped to. I wonder whether the
  x86 version has the same problem when being compiled with GCC4.
* Adopted the x86 page fault handling interrupt code.

ba61df6d0b Beginnings of APM support: we now connect to the APM BIOS in 32 bit protected mode.
We don't do anything with it yet, though, so the BIOS will probably ignore us since

6c678c57c7 * The exception vector offset is now also part of the iframe.
* Cloned iframe stack management from x86.
* Reimplemented arch_thread_{get,set}_current_thread(). The
  thread structure is stored in SPRG2. It is set to NULL in
  arch_cpu_preboot_init(), now. A non-null current thread
  causes all kinds of undesired behavior in early boot code.
* We establish the address space mappings we know from the
  Open Firmware as areas. At least those in kernel address
  space. The ones in userland address space are tougher.
  Fortunately on my Mac mini there aren't any save the
  boot_loader stack, which is not needed any longer anyway.
* Added stack trace support to the kernel debugger. Mostly
  cloned and adjusted the x86 code. Some bits are still
  missing, like stack traces for other threads.

7176dd57e5 Reworked the exception handling code. The former one ran into the
void after turning off BAT for the segment containing itself.
The monster macro for the exception vector code was not really
elegant besides being too long for the 32 byte performance
monitor exception slot. Furthermore wasting three of the SPRG*
registers as cheap scratch memory wasn't that nice either.

We now have a three-step approach: The exception vectors
themselves contain only five instructions which branch to common
code at the beginning of the same physical page. That one sets
up BAT for itself, turns address translation back on and jumps
into the kernel. There we turn off BAT again, dump an iframe,
and enter the actual exception handler (/dispatcher). Upon return
the registers are restored from the iframe and we get back to the
place where the exception occurred.

262e0a636b We use the same strategy for computing the system time as on x86 now.
The time base conversion factor is the 32 bit value
  2^32 * 1000000 / time base frequency,
so the system time can be computed by
  system time = time base * conversion factor / 2^32.
The expression in system_time() looks more complicated now, but is
actually much faster (factor 2.5 on my Mac mini). I'm positively
surprised, how good the assembly looks, that GCC 4 generates. There's
not that much potential for optimization by hand-coding the function.

e55e1a0e66 Implemented the PPC specific RTC support. We search for an "rtc"
device in the Open Firmware implementation of boot loader and
pass its path to the kernel, where it's opened and used for
getting/setting the real time. The expensive atomic_*64() on PPC
32-bit make things a bit more complicated. Moreover, missing
64 bit multiplication and division instructions won't really
allow system_time() to be anywhere near as fast as on x86. :-/

09bb4e9ac5 The real_time_data structure contains an architecture specific
substructure now (that's the only member actually). The system time
offset is therefore accessed via architecture specific accessor
functions.
Note, that this commit breaks the PPC build. Since I want to rename at

7afa713acb Reorganized the boot platform dependencies in the kernel a bit.
Basically the architecture specific code is now responsible to
init and make use of the platform specific code, now. The reason
being that we have only one kernel per platform and thus cannot
decide at compile time, which platform to use (if any).
The PPC implementation features an abstract base class PPCPlatform
(implemented for all supported platforms) through which platform
support is provided.

a1bcf2c880 * The OF memory management callback is now set in arch_mmu_init().
According to the spec we need to set it before taking over the MMU,
  but we can't call it before arch_mmu_init(), since we need the OF
  to allocate the page table. So we do it after we have allocated
  the new page table.
* Added PPC specific kernel_args: The virtual address ranges we want
  to keep in the kernel. We fill that in with the translations we
  find when initializing the MMU stuff. We remove the memory the
  boot loader occupies from those. Besides the stack for the boot
  loader only the OF stuff remains.
* arch_mmu_allocate() now starts to search at KERNEL_BASE for a free
  virtual address when no particular address is requested. This saves
  us further trouble in the kernel, since those allocations would
  need to be remapped otherwise.

10a83ac31f * Added tlbia() and tlbie() macros. Replaced ptesync() by ppc_sync();
the fact that I couldn't find ptesync in an otherwise more complete
  documentation I downloaded yesterday made me suspicious.
* arch_cpu_global_TLB_invalidate() uses tlbia now. The instruction is
  optional, but so is tlbie (how I understood it is that both exist,
  when the architecture implementation has a TLB). And the former loop
  looked just scary.
* Implemented arch_cpu_user_TLB_invalidate(). It does just the same as
  arch_cpu_global_TLB_invalidate().
* Some changes with respect to synchronization required on page table
  and segment register updates.
* Some more minor renaming. Pulled a new function
  remove_page_table_entry() out of unmap_tmap().
* In arch_vm_translation_map_init_post_area() we do now remap the page
  table into the kernel address space, if it was without before. The
  page table might actually be a good application for BAT, though.

79f73dbc56 * vm_page::offset is now called cache_offset and is now an uint32 instead of off_t;
this saves 4 bytes per page. To compensate the loss of bytes, the offset is now

51a3c450be The short story: we now have MTRR support on Intel and AMD CPUs (the latter
has not yet been tested, though - I'll do this after this commit):
* Removed the arch_memory_type stuff from vm_area; since there are only 8 memory
  ranges on x86, it's simply overkill. The MTRR code now remembers the area ID
  and finds the MTRR that way (it could also iterate over the existing MTRRs).
* Introduced some post_modules() init functions.
* If the other x86 CPUs out there don't differ a lot, MTRR functionality might
  be put back into the kernel.
* x86_write_msr() was broken, it wrote the 64 bit number with the 32 bit words
  switched - it took me some time (and lots of #GPs) to figure that one out.
* Removed the macro read_ebp() and introduced a function x86_read_ebp()
  (it's not really a time critical call).
* Followed the Intel docs on how to change MTRRs (symmetrically on all CPUs
  with caches turned off).
* Asking for memory types will automatically change the requested length to
  a power of two - note that BeOS seems to behave in the same, although that's
  not really very clean.
* fixed MTRRs are ignored for now - we should make sure at least, though,
  that they are identical on all CPUs (or turn them off, even though I'd
  prefer the BIOS stuff to be uncacheable, which we don't enforce yet, though).

0c67510ae1 Enabled SMP detection again - since it's very likely that it doesn't work on
your system, I've also added a "Disable SMP" safemode option. The NO_SMP
define is still there, and will be removed once SMP works flawlessly.

d08379a80b * Now we deal with all x86 exceptions, i.e. we no longer panic() when a
user application performs a division by zero or causes a general
  protection fault. For some exceptions (e.g. machine check) I wasn't
  quite sure whether they can be caused by user apps at all, so we panic()
  in those cases. Wouldn't harm, if someone more knowledgable would check
  this, though.
* Removed the unused fault handling stuff, respectively moved the little
  that was used into x86/arch_int.c.

f33c8020e2 Removed debug/console.c, we no longer need it. frame_buffer_console_init() is now
called by debug_init_post_vm().
Since the availability of a blue screen specific getchar() is static anyway, there
is no need for the sBlueScreenGetChar variable (only the message "only serial input
available" gets lost, but since that is platform specific anyway...).
Hello blue screen! We now have an on-screen KDL, to be enabled by the kernel
setting "bluescreen", just like on BeOS.
The blue screen does not yet support any cursor actions or backspace, though (need
to grab some stuff from our console driver).

9a63135f7c Cleanup of the debug stuff, prepared for blue screen debugging.
kernel_debugger() didn't do enough before; panic() did all the work - but
since the former is a public function as well, I moved all the functionality
to it. Also fixed a possible buffer overrun in panic().
Renamed dbg_* to debug_*.
"serial_debug_port" setting did not ignore negative values.

b20667b35b {install|remove}_io_interrupt_handler() now correctly handle the B_NO_ENABLE_COUNTER flag.
Lots of cleanup:
- moved B_NO_ENABLE_COUNTER flag definition out of int.h to KernelExport.h, as it's
  described in the BeBook (although it's probably not really used that often :))
- int.c no longer has any platform dependent code (+ 0x20 on interrupt numbers is gone);
  it's now entirely handled in the arch/x86/ section.
- the io_vectors[] is now statically initialized, instead of allocated from the heap
- removed {install|remove}_interrupt_handler(); they weren't that useful, arch_smp_init()
  is now calling install_io_interrupt_handler() correctly instead
- introduced a new arch_int.h header file that currently contains NUM_IO_VECTORS only
  (though on x86, it also has ARCH_INTERRUPT_BASE == 0x20).
- changed the return type from {install|remove}_io_interrupt_handler() from "long" to
  "status_t"
- rearranged and cleaned the PIC initialization code, made the PIC code more prominent
- changed comments that talk about a non existing 8239 (the PIC chip is actually 8259)
- moved arch/x86/interrupts.h to the source directory, as it's not used outside
- added BeOS compatible interrupts_enabled() function, that should replace our
  equivalent (and private) are_interrupts_enabled()

8e26b085e5 Fixed double fault handler. Personally I disclaim all responsiblity
for these changes. I was mostly just staring in amazement at the screen
while Axel and Thomas were discussing IA32 internals. A particularly
fascinating moment was when Thomas produced the cause of a bug we had
been trying to track down for hours off the top of his head (of course
iret behaves specially when the NT bit is set :-). His slowness must be
excused though, since he hadn't slept for more then 30 hours. ;-)

The code doesn't wholeheartedly deal with multi-processor machines yet.
Axel will certainly do some cleanup...

e5b0e0b5e3 Changed the set_segment_descriptor()'s usage of the limit/granularity
flag. Now, it will choose how to set the granularity by evaluating the
limit.
This call was actually already used this way in the kernel, so that
the TLS and TSS segments were much too large (harmless but incorrect).

9d06770cdc The iframe stack is now in a special structure iframe_stack.
Introduced a gBootFrameStack that is used until the first thread structure
is available - this allows stack crawls and useful register dumps during
early startup. Could also be solved differently by making sure there is
always a thread structure installed in %dr3 (ie. the boot thread would
get a static thread structure instead of a static iframe stack only).
This might be a better solution as i386_handle_trap() would no longer
need to check for an existing thread structure.

edb5566393 Fixed struct tss; there is no ss3/sp3.
Moved ptentry/pdentry to arch_vm_translation_map.c and renamed them to
page_table_entry and page_directory_entry.
Fixed a race condition that happened when memory was remapped (which
can currently happen because lock_memory() does not work correctly, and
there might be other conditions as well, like certain vm_store fault
handlers). Now, page table and directory entries are updated atomically.

85001a7cc6 * Fixed checking for DR6 bits after a debug exception (we were and'ing
with the bit number, not the respective mask).
* Added a small hack to allow single stepping to work in qemu.
  Apparently the BS bit in DR6 is not set when the debug exception
  is handled. So we always assume that a single step event occurred,
  when we couldn't recognize any other event, if the hack is enabled.

86a48e24ad Got inspired by mmu_man's dmidecode release and added a simple identification
of BIOS32, SMBIOS, and DMI services.
Also implemented a get_bios32_service() function which you can use to get
access to that service (ie. for use by the PCI bus manager).

67a53f72af - the arch_vm_translation_map.c exported functions now have the arch_ prefix
- the above file now also exports a function to finally complete the kernel's
  address space translation map after semaphores became available (was just
  not done before)
- renamed some functions, especially the *_init2() functions are now called
  like *_init_post_area() to indicate they are called after the area functionality
  became available
- removed the _struct suffix from certain structures
- fixed some return types
- added prototype for vm_free_unused_boot_loader_range() to be called by
  arch code.

9175b5a5e0 Cleanup of the header mess: removed the redundant APIC/mp structure definitions
from smp_priv.h and stage2_priv.h. Created a new file smp_apic.h which now
contains those definitions.
Made the APIC_xxx register offsets only base offsets (without referencing a
particular variable).
Other small cleanups (i.e. "unsigned int" to uint32).

074a16b53e Added a private tls.h file with definitions for the reserved TLS slots.
Renamed i386_set_kstack() to i386_set_tss_and_kstack(), because that's what
it does.
Added a new function arch_thread_init_tls() which must be called after
having allocated the TLS area.
Renamed arch_thread_initialize_kthread_stack() to arch_thread_init_kthread_stack()
to be more consistent.
Changed the parameters for arch_thread_enter_uspace() - it now gets a pointer
to the thread structure and takes the user stack pointer from there (which
might also be architectural different).

7208e95f0e Moved the gdt_idt_descr struct from arch_stage2.h to stage2_priv.h.
Moved the tss_descriptor structure to descriptor.h, updated it to be a
segment_descriptor structure, and provided inlines for set_tss_descriptor(),
set_segment_descriptor(), set_segment_descriptor_base(), and
clear_segment_descriptor().
Also added defines for the different privilege levels and descriptor types.
Removed the unusused and incorrect TSS definition, introduced new
TSS_BASE_SEGMENT and TLS_BASE_SEGMENT macros.
Removed include of arch/cpu.h in arch_cpu.h.
