Add several useful macros:

- ENTRY_NOPROFILE() and ASENTRY_NOPROFILE(), similar to ENTRY() and
ASENTRY(), but without the profiling prologue.
- GLOBAL()/ASGLOBAL() and LOCAL()/ASLOCAL(), for defining global and
local variables with C and ASM labels.
- BSS()/ASBSS(), for defining items in the BSS segment, with C or
ASM labels.
- PANIC("panic message") - shorthand for calling panic() from assembly code.
- VECTOR(), ASVECTOR(), VECTOR_UNUSED - shorthand for defining entries
in the vector table.
Also, change RCSID() to pad out the string to even boundary.

- Workaround for what is apparently a firmware bug - ignore the sector
size returned by the device. On my Firepower's floppy, block-size
is the same as max-transfer, which causes Lossage.
- Don't read the disklabel on a floppy; do what the ISA floppy driver
does, which is assign the entire disk to each "partition", although
we do not deal with the density stuff.

extention (for ps_strings).
- machine == powerpc, not PowerPC.
- Set up machine vector for OpenFirmware.
- Nuke the msgbuf for now; it's not mapped correctly.
- Add missing casts in some asm() directives.
- Enable PSL_RI earlier, and do *not* disable it in fake_spl(), since it
is needed for page faults.
- Some general cleanup.

NetBSD/powerpc boot program. Highligts:
- Rearrange a bit so that we can sanely add boot programs for
non-OpenFirmware systems.
- Add support for loading compressed kernels (for booting from
1.44M floppies).
- Add support for loading Elf kernels (Elf is the new standard format
for NetBSD/powerpc). (XXX Don't load symbols right now - the code
to do it is there, but I don't particularly like how space for the
symbol table is allocated; I will revisit this soon'ish).

(2) so that the file names better describe the systems which use them. (the
pci_swiz* files are for machines whose PCI interfaces require address
"swizzling." Later, there will be probably be other sets, e.g. pci_bwx*
for machines whose chipsets can easily deal with the Alpha BWX extensions
when doing device accesses.)

Change in the way receive buffer areas are handled. Before we gave
the chip 16 buffers, each 1536 bytes (big enough for one packet).
Now we're handing the chip 8 buffers, each 4 Kbytes, and letting
the chip fit as many packets as it can in each one. This should
help keep it from running out of buffer space. Also make some of
the performance-crucial routines inline. It made no measurable
difference except to make me feel better

allocation time. Also, do not copy the kernel's region administration
to every user pmap, especially since no memory appears to be allocated

and pmap_zero_page() (XXX).

by default, provide RCS IDs for NetBSD/alpha kernel files in kernel binaries.

This can be disabled (to save a bit of space) with the NO_KERNEL_RCSIDS
options, which is present but commented out in the ALPHA config file.
In ELF-format kernels, these strings are present in the kernel binary but
are not loaded into memory. (In ECOFF-format kernels, there's no easy way
to keep them from being loaded, so they _are_ loaded into memory.)

Rework PDMA read/write functions based on Matthias Pfaller's pc532 ncr

driver. Major highlights:
- Protect with splbio(), now that interrupts from the chip might be
enabled by default;
- Wait for current transfer to complete before returning, rather than
wait for previous transfer to complete before starting another;
- Instead of logging a "timeout" message when a target disconnects,
just return, letting the sbc_ready() and sbc_wait_not_req() print
timeout messages if such really did occur.
With interrupt changes to sbc_obio.c, disconnect/reselect now works if
configured into the kernel.

A) Attempt to get clock rate from ISP chip prior to bashing it over the head

in reset. If none there, try and get from the bus/platform specific code.
If a nonzero value for either, set the clock rate. This is why the PCI
card versions weren't working- they need to be set at 60MHZ, rather than
the default 40MHZ (which worked fine for the internal ISP chips on the
Alpha 8X00).
B) If a isp_poll returns failure (command never completed) to the caller
and no error is set in the xs struct, set XS_SELTIMEOUT. And then call...
C) Added isp_lostcmd function to try and ask the ISP chip about it's current
state as well as the state of commands for a particular target/lun. This is
going along to try and figure out why the very first command to the ISP always
seems to get swallowed up.

Push the buffer cleanup code into scsi_done(), and split it so that biodone()
...
is called *after* the driver `done' routine. This fixes disk I/O statistics
on SCSI devices.
Also, calling the `done' routine with a `complete' argument of 0 and actually
having it do anything meaningful loses in at least 3 ways, so just nuke the
argument altogether and don't call it this way. If the driver needs to do
some error handling, that's what `err_handler' is for.

Do the untimeout in ncr53c9x_done instead of just before
almost every call to ncr53c9x_done as was done previously.
Make ncr53c9x_sense schedule its own timeout for the new
command it is starting (request sense), separate from the
timeout for the command that just completed.

Comment out DIAGNOSTIC panic in trap code translation. The DIAGNOSTIC is

correct, but unfortunately in trap.c trapsignal(p, SIGSEGV, code) passes
the address of the fault in code, instead of the trap code. So we punt for
now doing checking for trap code validity until this gets fixed.

Fix an ... interesting bug that resulted from namespace collision.
...
Description:
- A BSD pseudo-device initialization routine is declared as
void <pseudo-device name>attach __P((int count));
in ioconf.c by config(8). main() calls these functions
from a table.
- IP Filter has functions iplattach() and ipldetach() (or,
in the NetBSD case, were erroneously renamed ipfilterattach()
and ipfilterdetach()). These functions are used to establish
and disestablish the IP Filter "filter rule check" hook in
the IP input/output stream. They are declared:
int iplattach __P((void));
int ipldetach __P((void));
..and are expected to return a value by iplioctl().
- When main() calls (by sheer coincidence!) iplattach(),
the filter hook is established, and the IP Filter machinery
labeled as "initialized". This causes all packets, whether or
not the user intents to use filter rules, to be passed to
the filter rule checker if "ipfilter" is configured into the
kernel.
- As a result of the above, a kludge existed to default to
passing all packets (I can only assume that when this was
originally committed, the symptom of the bug was noticed by
the integrator, but the bug not actually found/fixed).
- In iplioctl(), if the SIOCFRENB ioctl is issued with an
argument of "enable" (i.e. user executed "ipf -E"), iplattach()
will notice that the machinery is already initialized and
return EBUSY.
Fix:
- Rename iplattach()/ipldetach() to ipl_enable() and ipl_disable().
- Create a pseudo-device entry stub named ipfilterattach()
(NetBSD case) or iplattach() (all other). This is a noop; none
of the machinery should be initialized until the caller expicitly
enables the filter with ipf -E. Add a comment to note that.

I noticed a few semi-serious bugs while doing this merge, one of which
has existed for a fairly long time. Some of them are addressed in this
commit (because they caused the kernel to not compile), and are annoted
by "XXX" and "--thorpej". The other one will be addressed shortly in
a future commit, and, as far as I can tell, affects all operating systems
which IP Filter supports.

Update and enhancement to the mbuf code, to support use of non-cluster

external storage. Highlights:
- additional "void *" argument to (*ext_free)(), an opaque
cookie for use by the free function.
- MCLALLOC() and MCLFREE() calls are gone. They are replaced
by MEXTADD() (add external storage to mbuf), MEXTMALLOC()
(malloc() external storage and attach to mbuf), and
MEXTREMOVE() (remove external storage from mbuf).
- completely new external storage reference counting
mechanism; mclrefcnt[] is gone.
These changes will eventually be used to pass driver DMA buffers up
the network stack, and reduce/eliminate copies in certain code paths
(e.g. NFS writes).

Work around an ambiguity in the SCSI spec. Some implementations,
...
particularly older ones, don't do a SAVE DATA POINTER before disconnecting.
When the driver reconnects, it does an implicit RESTORE POINTERS, and
restores the data pointer from *before* the last transfer. If the driver
calculates the residual using the data pointer and doesn't account for
this, the residual will be wrong.
What we do is test for the SDEV_AUTOSAVE quirk when we see a disconnect
message, falling through to do a SAVE DATA POINTER if the quirk applies
to this target.
The workaround was suggested by Mycroft, who also provided the explanation
of the problem. Actual code was (mostly) ripped off from the 53C9x driver.

In pmap_remove(), check for an empty segment before calling one of the

helper functions. This cuts down the number of needless function calls
by approx. 80%, which has a healthy effect on the responsiveness of
a machine under heavy process creation/teardown loads.
The VM system seems to be fond of asking to delete page mappings
which aren't there..
