After setting CP15 bits in arm_set_cpu_on() the cached hflags must
be rebuild to reflect the changed processor state. Without rebuilding,
the cached hflags would be inconsistent until the next call to
arm_rebuild_hflags(). When QEMU is compiled with debugging enabled
(--enable-debug), this problem is captured shortly after the first
call to arm_set_cpu_on() for CPUs running in ARM 32-bit non-secure mode:

The ASPEED AST2600 clocks the generic timer at the rate of HPLL. On
recent firmwares this is at 1125MHz, which is considerably quicker than
the assumed 62.5MHz of the current generic timer implementation. The
delta between the value as read from CNTFRQ and the true rate of the
underlying QEMUTimer leads to sticky behaviour in AST2600 guests.
Add a feature-gated property exposing CNTFRQ for ARM CPUs providing the
generic timer. This allows platforms to configure CNTFRQ (and the
associated QEMUTimer) to the appropriate frequency prior to starting the
guest.
As the platform can now determine the rate of CNTFRQ we're exposed to
limitations of QEMUTimer that didn't previously materialise: In the
course of emulation we need to arbitrarily and accurately convert
between guest ticks and time, but we're constrained by QEMUTimer's use
of an integer scaling factor. The effect is QEMUTimer cannot exactly
capture the period of frequencies that do not cleanly divide
NANOSECONDS_PER_SECOND for scaling ticks to time. As such, provide an
equally inaccurate scaling factor for scaling time to ticks so at least
a self-consistent inverse relationship holds.

Prepare for SoCs such as the ASPEED AST2600 whose firmware configures
CNTFRQ to values significantly larger than the static 62.5MHz value
currently derived from GTIMER_SCALE. As the OS potentially derives its
timer periods from the CNTFRQ value the lack of support for running
QEMUTimers at the appropriate rate leads to sticky behaviour in the
guest.
Substitute the GTIMER_SCALE constant with use of a helper to derive the
period from gt_cntfrq_hz stored in struct ARMCPU. Initially set
gt_cntfrq_hz to the frequency associated with GTIMER_SCALE so current
behaviour is maintained.

The corner-case codepath was adjusting nexttick such that overflow
wouldn't occur when timer_mod() scaled the value back up. Remove a use
of GTIMER_SCALE and avoid unnecessary operations by calling
timer_mod_ns() directly.

When using `query-cpu-definitions` using `-machine none`,
QEMU is resolving all CPU models to their latest versions. The
actual CPU model version being used by another machine type (e.g.
`pc-q35-4.0`) might be different.
In theory, this was OK because the correct CPU model
version is returned when using the correct `-machine` argument.
Except that in practice, this breaks libvirt expectations:
libvirt always use `-machine none` when checking if a CPU model
is runnable, because runnability is not expected to be affected
when the machine type is changed.
For example, when running on a Haswell host without TSX,
Haswell-v4 is runnable, but Haswell-v1 is not. On those hosts,
`query-cpu-definitions` says Haswell is runnable if using
`-machine none`, but Haswell is actually not runnable using any
of the `pc-*` machine types (because they resolve Haswell to
Haswell-v1). In other words, we're breaking the "runnability
guarantee" we promised to not break for a few releases (see
qemu-deprecated.texi).
To address this issue, change the default CPU model version to v1
on all machine types, so we make `query-cpu-definitions` output
when using `-machine none` match the results when using `pc-*`.
This will change in the future (the plan is to always return the
latest CPU model version if using `-machine none`), but only
after giving libvirt the opportunity to adapt.

qemu_log_lock() now returns a handle and qemu_log_unlock() receives a
handle to unlock. This allows for changing the handle during logging
and ensures the lock() and unlock() are for the same file.
Also in target/tilegx/translate.c removed the qemu_log_lock()/unlock()
calls (and the log("\n")), since the translator can longjmp out of the
loop if it attempts to translate an instruction in an inaccessible page.

Mostly, Error ** is for returning error from the function, so the
callee sets it. However kvmppc_hint_smt_possible gets already filled
errp parameter. It doesn't change the pointer itself, only change the
internal state of referenced Error object. So we can make it Error
*const * errp, to stress the behavior. It will also help coccinelle
script (in future) to distinguish such cases from common errp usage.
While there, rename the function to
kvmppc_error_append_smt_possible_hint().

Similar to CPU and machine classes, "-accel" class names are mangled,
so we have to first get a class via accel_find and then instantiate it.
Provide a new function to instantiate a class without going through
object_class_get_name, and use it for CPUs and machines already.

register, present on POWER5+ and later processors.

The Access Segment Descriptor Register (ASDR) provides information about
the storage element when taking a hypervisor storage interrupt. When
performing nested radix address translation, this is normally the guest
real address. This register is present on POWER9 processors and later.
Implement the ADSR, note read and write access is limited to the
hypervisor.

The Processor Utilisation of Resources Register (PURR) and Scaled
Processor Utilisation of Resources Register (SPURR) provide an estimate
of the resources used by the thread, present on POWER7 and later
processors.
Currently the [S]PURR registers simply count at the rate of the
timebase.
Preserve this behaviour but rework the implementation to store an offset
like the timebase rather than doing the calculation manually. Also allow
hypervisor write access to the register along with the currently
available read access.

The virtual timebase register (VTB) is a 64-bit register which
increments at the same rate as the timebase register, present on POWER8
and later processors.
The register is able to be read/written by the hypervisor and read by
the supervisor. All other accesses are illegal.
Currently the VTB is just an alias for the timebase (TB) register.
Implement the VTB so that is can be read/written independent of the TB.
Make use of the existing method for accessing timebase facilities where
by the compensation is stored and used to compute the value on reads/is
updated on writes.

The power7_set_irq() and power9_set_irq() functions set this but it is
never used actually. Modern Book3s compatible CPUs are only supported
by the pnv and spapr machines. They have an interrupt controller, XICS
for POWER7/8 and XIVE for POWER9, whose models don't require to track
IRQ input states at the CPU level.
Drop these lines to avoid confusion.

When a CPU is reset, QEMU makes sure no interrupt is pending by clearing
CPUPPCstate::pending_interrupts in ppc_cpu_reset(). In the case of a
complete machine emulation, eg. a sPAPR machine, an external interrupt
request could still be pending in KVM though, eg. an IPI. It will be
eventually presented to the guest, which is supposed to acknowledge it at
the interrupt controller. If the interrupt controller is emulated in QEMU,
either XICS or XIVE, ppc_set_irq() won't deassert the external interrupt
pin in KVM since it isn't pending anymore for QEMU. When the vCPU re-enters
the guest, the interrupt request is still pending and the vCPU will try
again to acknowledge it. This causes an infinite loop and eventually hangs
the guest.
The code has been broken since the beginning. The issue wasn't hit before
because accel=kvm,kernel-irqchip=off is an awkward setup that never got
used until recently with the LC92x IBM systems (aka, Boston).
Add a ppc_irq_reset() function to do the necessary cleanup, ie. deassert
the IRQ pins of the CPU in QEMU and most importantly the external interrupt
pin for this vCPU in KVM.

A write to the SCR can change the effective EL by droppping the system
from secure to non-secure mode. However if we use a cached current_el
from before the change we'll rebuild the flags incorrectly. To fix
this we introduce the ARM_CP_NEWEL CP flag to indicate the new EL
should be used when recomputing the flags.

ARMv8.2 introduced support for Data Cache Clean instructions
to PoP (point-of-persistence) - DC CVAP and PoDP (point-of-deep-persistence)
- DV CVADP. Both specify conceptual points in a memory system where all writes
that are to reach them are considered persistent.
The support provided considers both to be actually the same so there is no
distinction between the two. If none is available (there is no backing store
for given memory) both will result in Data Cache Clean up to the point of
coherency. Otherwise sync for the specified range shall be performed.

This change ensures that the FPU can be accessed in Non-Secure mode
when the CPU core is reset using the arm_set_cpu_on() function call.
The NSACR.{CP11,CP10} bits define the exception level required to
access the FPU in Non-Secure mode. Without these bits set, the CPU
will give an undefined exception trap on the first FPU access for the
secondary cores under Linux.
This is necessary because in this power-control codepath QEMU
is effectively emulating a bit of EL3 firmware, and has to set
the CPU up as the EL3 firmware would.

HSTR_EL2 offers a way to trap ranges of CP15 system register
accesses to EL2, and it looks like this register is completely
ignored by QEMU.
To avoid adding extra .accessfn filters all over the place (which
would have a direct performance impact), let's add a new TB flag
that gets set whenever HSTR_EL2 is non-zero and that QEMU translates
a context where this trap has a chance to apply, and only generate
the extra access check if the hypervisor is actively using this feature.
Tested with a hand-crafted KVM guest accessing CBAR.

HCR_EL2.TID3 requires that AArch32 reads of MVFR[012] are trapped to
EL2, and HCR_EL2.TID0 does the same for reads of FPSID.
In order to handle this, introduce a new TCG helper function that
checks for these control bits before executing the VMRC instruction.
Tested with a hacked-up version of KVM/arm64 that sets the control
bits for 32bit guests.

HCR_EL2.TID1 mandates that access from EL1 to REVIDR_EL1, AIDR_EL1
(and their 32bit equivalents) as well as TCMTR, TLBTR are trapped
to EL2. QEMU ignores it, making it harder for a hypervisor to
virtualize the HW (though to be fair, no known hypervisor actually
cares).
Do the right thing by trapping to EL2 if HCR_EL2.TID1 is set.

qmp_query_cpu_definitions() passes @errp to get_max_cpu_model(), then
frees any error it gets back. This effectively ignores errors.
Dereferencing @errp is wrong; see the big comment in error.h. Passing
@errp is also wrong, because it works only as long as @errp is neither
@error_fatal nor @error_abort. Introduced in commit 38cba1f4d8
"s390x: return unavailable features via query-cpu-definitions".
No caller actually passes such @errp values.
Fix anyway: simply pass NULL to get_max_cpu_model().

get_max_cpu_model() dereferences @errp when
kvm_s390_get_host_cpu_model() fails, apply_cpu_model() dereferences it
when kvm_s390_apply_cpu_model() fails, and s390_realize_cpu_model()
dereferences it when get_max_cpu_model() or check_compatibility()
fail. That's wrong; see the big comment in error.h. All three
introduced in commit 80560137cf "s390x/cpumodel: check and apply the
CPU model".

If kvm does not support VMX feature by nested=0, the kvm_vmx_basic
can't get the right value from MSR_IA32_VMX_BASIC register, which
make qemu coredump when qemu do KVM_SET_MSRS.

Previous implementation in hvf_inject_interrupts() would always inject
VMCS_INTR_T_SWINTR even when VMCS_INTR_T_HWINTR was required. Now
correctly determine when VMCS_INTR_T_HWINTR is appropriate versus
VMCS_INTR_T_SWINTR.
Make sure to clear ins_len and has_error_code when ins_len isn't
valid and error_code isn't set.

HCR_EL2.TID3 mandates that access from EL1 to a long list of id
registers traps to EL2, and QEMU has so far ignored this requirement.
This breaks (among other things) KVM guests that have PtrAuth enabled,
while the hypervisor doesn't want to expose the feature to its guest.
To achieve this, KVM traps the ID registers (ID_AA64ISAR1_EL1 in this
case), and masks out the unsupported feature.
QEMU not honoring the trap request means that the guest observes
that the feature is present in the HW, starts using it, and dies
a horrible death when KVM injects an UNDEF, because the feature
*really* isn't supported.
Do the right thing by trapping to EL2 if HCR_EL2.TID3 is set.
Note that this change does not include trapping of the MVFR
registers from AArch32 (they are accessed via the VMRS
instruction and need to be handled in a different way).

The ARMv8 ARM states when executing at EL2, EL3 or Secure EL1,
ISR_EL1 shows the pending status of the physical IRQ, FIQ, or
SError interrupts.
Unfortunately, QEMU's implementation only considers the HCR_EL2
bits, and ignores the current exception level. This means a hypervisor
trying to look at its own interrupt state actually sees the guest
state, which is unexpected and breaks KVM as of Linux 5.3.
Instead, check for the running EL and return the physical bits
if not running in a virtualized context.

According to the PushStack() pseudocode in the armv7m RM,
bit 4 of the LR should be set to NOT(CONTROL.PFCA) when
an FPU is present. Current implementation is doing it for
armv8, but not for armv7. This patch makes the existing
logic applicable to both code paths.

In real x86 processors, the REX prefix must come after legacy prefixes.
REX before legacy is ignored. Update the HVF emulation code to properly
handle this. Fix some spelling errors in constants. Fix some decoder
table initialization issues found by Coverity.

The existing code in QEMU's HVF support to attempt to synchronize TSC
across multiple cores is not sufficient. TSC value on other cores
can go backwards. Until implementation is fixed, remove calls to
hv_vm_sync_tsc(). Pass through TSC to guest OS.

If an area is non-RAM and non-ROMD, then remove mappings so accesses
will trap and can be emulated. Change hvf_find_overlap_slot() to take
a size instead of an end address: it wouldn't return a slot because
callers would pass the same address for start and end. Don't always
map area as read/write/execute, respect area flags.

We have been trying to avoid adding new aliases for CPU model
versions, but in the case of changes in defaults introduced by
the TAA mitigation patches, the aliases might help avoid user
confusion when applying host software updates.

The MSR_IA32_TSX_CTRL MSR can be used to hide TSX (also known as the
Trusty Side-channel Extension). By virtualizing the MSR, KVM guests
can disable TSX and avoid paying the price of mitigating TSX-based
attacks on microarchitectural side channels.

Simply moving the non-stub helper_v7m_mrs/msr outside of
!CONFIG_USER_ONLY is not an option, because of all of the
other system-mode helpers that are called.
But we can split out a few subroutines to handle the few
EL0 accessible registers without duplicating code.

Coverity reports, in sve_zcr_get_valid_len,
"Subtract operation overflows on operands
arm_cpu_vq_map_next_smaller(cpu, start_vq + 1U) and 1U"
First, the aarch32 stub version of arm_cpu_vq_map_next_smaller,
returning 0, does exactly what Coverity reports. Remove it.
Second, the aarch64 version of arm_cpu_vq_map_next_smaller has
a set of asserts, but they don't cover the case in question.
Further, there is a fair amount of extra arithmetic needed to
convert from the 0-based zcr register, to the 1-base vq form,
to the 0-based bitmap, and back again. This can be simplified
by leaving the value in the 0-based form.
Finally, use test_bit to simplify the common case, where the
length in the zcr registers is in fact a supported length.

The access to the machine class returned by MACHINE_GET_CLASS() in
tsc_khz_needed() is crashing as it is trying to dereference a different
type of machine class object (TYPE_PC_MACHINE) to that of this microVM.
This can be resolved by extending the changes in the following commit
f0bb276bf8 ("hw/i386: split PCMachineState deriving X86MachineState from it")
and moving the save_tsc_khz field in PCMachineClass to X86MachineClass.

TSX Async Abort (TAA) is a side channel attack on internal buffers in
some Intel processors similar to Microachitectural Data Sampling (MDS).
Some future Intel processors will use the ARCH_CAP_TAA_NO bit in the
IA32_ARCH_CAPABILITIES MSR to report that they are not vulnerable to
TAA. Make this bit available to guests.

We have to set the default model of all machine classes, not just for
the active one. Otherwise, "query-machines" will indicate the wrong
CPU model (e.g. "power9_v2.0-powerpc64-cpu" instead of
"host-powerpc64-cpu") as "default-cpu-type".
s390x already fixed this in de60a92e "s390x/kvm: Set default cpu model for
all machine classes". This patch applies a similar fix for the pseries-*
machine types on ppc64.

Instead of relying on atomics to access the MIP register let's update
our helper function to instead just lock the IO mutex thread before
writing. This follows the same concept as used in PPC for handling

Allow cpu 'host' to enable SVE when it's available, unless the
user chooses to disable it with the added 'sve=off' cpu property.
Also give the user the ability to select vector lengths with the
sve<N> properties. We don't adopt 'max' cpu's other sve property,
sve-max-vq, because that property is difficult to use with KVM.
That property assumes all vector lengths in the range from 1 up
to and including the specified maximum length are supported, but
there may be optional lengths not supported by the host in that
range. With KVM one must be more specific when enabling vector
lengths.

Extend the SVE vq map initialization and validation with KVM's
supported vector lengths when KVM is enabled. In order to determine
and select supported lengths we add two new KVM functions for getting
and setting the KVM_REG_ARM64_SVE_VLS pseudo-register.
This patch has been co-authored with Richard Henderson, who reworked
the target/arm/cpu64.c changes in order to push all the validation and
auto-enabling/disabling steps into the finalizer, resulting in a nice
LOC reduction.

kvm_arm_create_scratch_host_vcpu() takes a struct kvm_vcpu_init
parameter. Rather than just using it as an output parameter to
pass back the preferred target, use it also as an input parameter,
allowing a caller to pass a selected target if they wish and to
also pass cpu features. If the caller doesn't want to select a
target they can pass -1 for the target which indicates they want
to use the preferred target and have it passed back like before.

Enable SVE in the KVM guest when the 'max' cpu type is configured
and KVM supports it. KVM SVE requires use of the new finalize
vcpu ioctl, so we add that now too. For starters SVE can only be
turned on or off, getting all vector lengths the host CPU supports
when on. We'll add the other SVE CPU properties in later patches.

Introduce cpu properties to give fine control over SVE vector lengths.
We introduce a property for each valid length up to the current
maximum supported, which is 2048-bits. The properties are named, e.g.
sve128, sve256, sve384, sve512, ..., where the number is the number of
bits. See the updates to docs/arm-cpu-features.rst for a description
of the semantics and for example uses.
Note, as sve-max-vq is still present and we'd like to be able to
support qmp_query_cpu_model_expansion with guests launched with e.g.
-cpu max,sve-max-vq=8 on their command lines, then we do allow
sve-max-vq and sve<N> properties to be provided at the same time, but
this is not recommended, and is why sve-max-vq is not mentioned in the
document. If sve-max-vq is provided then it enables all lengths smaller
than and including the max and disables all lengths larger. It also has
the side-effect that no larger lengths may be enabled and that the max
itself cannot be disabled. Smaller non-power-of-two lengths may,
however, be disabled, e.g. -cpu max,sve-max-vq=4,sve384=off provides a
guest the vector lengths 128, 256, and 512 bits.

Since 97a28b0eea ("target/arm: Allow VFP and Neon to be disabled via
a CPU property") we can disable the 'max' cpu model's VFP and neon
features, but there's no way to disable SVE. Add the 'sve=on|off'
property to give it that flexibility. We also rename
cpu_max_get/set_sve_vq to cpu_max_get/set_sve_max_vq in order for them
to follow the typical *_get/set_<property-name> pattern.

Add support for the query-cpu-model-expansion QMP command to Arm. We
do this selectively, only exposing CPU properties which represent
optional CPU features which the user may want to enable/disable.
Additionally we restrict the list of queryable cpu models to 'max',
'host', or the current type when KVM is in use. And, finally, we only
implement expansion type 'full', as Arm does not yet have a "base"
CPU type. More details and example queries are described in a new
document (docs/arm-cpu-features.rst).
Note, certainly more features may be added to the list of advertised
features, e.g. 'vfp' and 'neon'. The only requirement is that we can
detect invalid configurations and emit failures at QMP query time.
For 'vfp' and 'neon' this will require some refactoring to share a
validation function between the QMP query and the CPU realize
functions.

riscv_cpu_tlb_fill() uses the `size` parameter to check PMP violation
using pmp_hart_has_privs().
However, if the size is unknown (=0), the ending address will be
`addr - 1` as it is `addr + size - 1` in `pmp_hart_has_privs()`.
This always causes a false PMP violation on the starting address of the
range, as `addr - 1` is not in the range.
In order to fix, we just assume that all bytes from addr to the end of
the page will be accessed if the size is unknown.

There is a small wrinkle with the gUSA instruction. The translator
effectively treats a (known) gUSA sequence as a single instruction.
For the purposes of the plugin we end up with a long multi-instruction
qemu_plugin_insn.
If the known sequence isn't detected we shall never run this
translation anyway.

32 bit and 64 bit access functions have historically not included the
sign so we leave those as is. We also introduce some signed helpers
which are used for loading immediate values in the translator.

Currently only PRV_U, PRV_S and PRV_M are supported, so this patch ensures that
the privilege mode is set to one of them. Once support for the H-extension is
added, this code will also need to properly update the virtualization status
when switching between VU/VS-modes and M-mode.

This patch enables a debugger to read the current privilege level via a virtual
"priv" register. When compiled with CONFIG_USER_ONLY the register is still
visible but always reports the value zero.

If the number of registers reported to the gdbstub code does not match the

The CPU loop tagged all the queued signals as QEMU_SI_KILL while it was
filling the `_sigfault` part of `siginfo`: this caused QEMU to copy the
wrong fields over to the userspace program.
Make sure the fault address recorded by the MMU is is stored in the CPU
environment structure.
In case of memory faults store the exception address into `siginfo`.

We directly access physical memory while walking the page tables on
RISC-V, but while doing so we were using cpu_ld*() which does not report
bus errors. This patch converts the page table walker over to use
address_space_ld*(), which allows bus errors to be detected.

If we are in debugger mode, skip the CSR privilege level checking
so that we can read/write all CSRs. Otherwise we get:

Change the handling of port F0h writes and FPU exceptions to implement IGNNE.
The implementation mixes a bit what the chipset and processor do in real
hardware, but the effect is the same as what happens with actual FERR#
and IGNNE# pins: writing to port F0h asserts IGNNE# in addition to lowering
FP_IRQ; while clearing the SE bit in the FPU status word deasserts IGNNE#.

There are three page size in qemu:
real host page size
host page size
target page size
All of them have dedicate variable to represent. For the last two, we
use the same form in the whole qemu project, while for the first one we
use two forms: qemu_real_host_page_size and getpagesize().
qemu_real_host_page_size is defined to be a replacement of
getpagesize(), so let it serve the role.
[Note] Not fully tested for some arch or device.

application, consuming almost a quarter of the runtime. Within the
entire execution, cpu_get_tb_cpu_state consumes about 12%.
After, helper_lookup_tb_ptr has dropped to the fourth hottest function,
with consumption dropping to a sixth of the runtime. Within the
entire execution, cpu_get_tb_cpu_state has dropped below 1%, and the
supporting function to rebuild hflags also consumes about 1%.
Assertions are retained for --enable-debug-tcg.

There are 3 conditions that each enable this flag. M-profile always
enables; A-profile with EL1 as AA64 always enables. Both of these
conditions can easily be cached. The final condition relies on the
FPEXC register which we are not prepared to cache.

used, and the results are fed back to cpu_get_tb_cpu_state.
Note that not all BTI related flags are cached, so we have to
test the BTI feature twice -- once for those bits moved out to
rebuild_hflags_a64 and once for those bits that remain in
cpu_get_tb_cpu_state.

In previous implementation, invocation of TCG shift function could request
shift of TCG variable by 64 bits when variable 'sh' is 0, which is not
supported in TCG (values can be shifted by 0 to 63 bits). This patch fixes
this by using two separate invocation of TCG shift functions, with maximum
shift amount of 32.
Name of variable 'shifted' is changed to 'carry' so variable naming
is similar to old helper implementation.
Variables 'avrA' and 'avrB' are replaced with variable 'avr'.

UMWAIT and TPAUSE instructions use 32bits IA32_UMWAIT_CONTROL at MSR
index E1H to determines the maximum time in TSC-quanta that the processor
can reside in either C0.1 or C0.2.
This patch is to Add support for save/load IA32_UMWAIT_CONTROL MSR in
guest.

UMONITOR, UMWAIT and TPAUSE are a set of user wait instructions.
This patch adds support for user wait instructions in KVM. Availability
of the user wait instructions is indicated by the presence of the CPUID
feature flag WAITPKG CPUID.0x07.0x0:ECX[5]. User wait instructions may
be executed at any privilege level, and use IA32_UMWAIT_CONTROL MSR to
set the maximum time.
The patch enable the umonitor, umwait and tpause features in KVM.
Because umwait and tpause can put a (psysical) CPU into a power saving
state, by default we dont't expose it to kvm and enable it only when
guest CPUID has it. And use QEMU command-line "-overcommit cpu-pm=on"
(enable_cpu_pm is enabled), a VM can use UMONITOR, UMWAIT and TPAUSE
instructions. If the instruction causes a delay, the amount of time
delayed is called here the physical delay. The physical delay is first
computed by determining the virtual delay (the time to delay relative to
the VM’s timestamp counter). Otherwise, UMONITOR, UMWAIT and TPAUSE cause
an invalid-opcode exception(#UD).

Hyper-V TLFS specifies this enlightenment as:
"NoNonArchitecturalCoreSharing - Indicates that a virtual processor will never
share a physical core with another virtual processor, except for virtual
processors that are reported as sibling SMT threads. This can be used as an
optimization to avoid the performance overhead of STIBP".
However, STIBP is not the only implication. It was found that Hyper-V on
KVM doesn't pass MD_CLEAR bit to its guests if it doesn't see
NoNonArchitecturalCoreSharing bit.
KVM reports NoNonArchitecturalCoreSharing in KVM_GET_SUPPORTED_HV_CPUID to
indicate that SMT on the host is impossible (not supported of forcefully
disabled).
Implement NoNonArchitecturalCoreSharing support in QEMU as tristate:
'off' - the feature is disabled (default)
'on' - the feature is enabled. This is only safe if vCPUS are properly
pinned and correct topology is exposed. As CPU pinning is done outside
of QEMU the enablement decision will be made on a higher level.
'auto' - copy KVM setting. As during live migration SMT settings on the
source and destination host may differ this requires us to add a migration
blocker.

Patch logs MCE AO, AR messages injected to guest or taken by QEMU itself.
We print the QEMU address for guest MCEs, helps on hypervisors that have
another source of MCE logging like mce log, and when they go missing.
For example we found these QEMU logs:

We have to set the default model of all machine classes, not just for
the active one. Otherwise, "query-machines" will indicate the wrong
CPU model ("qemu-s390x-cpu" instead of "host-s390x-cpu") as
"default-cpu-type".

Looks like my idea of what a "borrow" is was wrong. The PoP says:
"If the resulting subtraction results in a carry out of bit zero, a value
of one is placed in the corresponding element of the first operand;
otherwise, a value of zero is placed in the corresponding element"
As clarified by Richard, all we have to do is invert the result.

KVM has a 80-entry limit at KVM_SET_CPUID2. With the
introduction of CPUID[0x1F], it is now possible to hit this limit
with unusual CPU configurations, e.g.:

qemu-system-x86_64: kvm_init_vcpu failed: Argument list too long
This happens because QEMU adds a lot of all-zeroes CPUID entries
for unused CPUID leaves. In the example above, we end up
creating 48 all-zeroes CPUID entries.
KVM already returns all-zeroes when emulating the CPUID
instruction if an entry is missing, so the all-zeroes entries are
redundant. Skip those entries. This reduces the CPUID table
size by half while keeping CPUID output unchanged.

SH_EXT_STDOUT_STDERR is a v2.0 semihosting extension: the guest
can open ":tt" with a file mode requesting append access in
order to open stderr, in addition to the existing "open for
read for stdin or write for stdout". Implement this and

SH_EXT_EXIT_EXTENDED is a v2.0 semihosting extension: it
indicates that the implementation supports the SYS_EXIT_EXTENDED
function. This function allows both A64 and A32/T32 guests to
exit with a specified exit status, unlike the older SYS_EXIT
function which only allowed this for A64 guests. Implement
this extension.

Version 2.0 of the semihosting specification added support for
allowing a guest to detect whether the implementation supported
particular features. This works by the guest opening a magic
file ":semihosting-features", which contains a fixed set of
data with some magic numbers followed by a sequence of bytes
with feature flags. The file is expected to behave sensibly
for the various semihosting calls which operate on files
(SYS_FLEN, SYS_SEEK, etc).
Implement this as another kind of guest FD using our function
table dispatch mechanism. Initially we report no extended
features, so we have just one feature flag byte which is zero.

Currently for the semihosting calls which take a file descriptor
(SYS_CLOSE, SYS_WRITE, SYS_READ, SYS_ISTTY, SYS_SEEK, SYS_FLEN)
we have effectively two implementations, one for real host files
and one for when we indirect via the gdbstub. We want to add a
third one to deal with the magic :semihosting-features file.
Instead of having a three-way if statement in each of these
cases, factor out the implementation of the calls to separate
functions which we dispatch to via function pointers selected
via the GuestFDType for the guest fd.
In this commit, we set up the framework for the dispatch,
and convert the SYS_CLOSE call to use it.

When we are routing semihosting operations through the gdbstub, the
work of sorting out the return value and setting errno if necessary
is done by callback functions which are invoked by the gdbstub code.
Clean up some ifdeffery in those functions by having them call
set_swi_errno() to set the semihosting errno.

The semihosting code needs accuss to the linux-user only
TaskState pointer so it can set the semihosting errno per-thread
for linux-user mode. At the moment we do this by having some
ifdefs so that we define a 'ts' local in do_arm_semihosting()
which is either a real TaskState * or just a CPUARMState *,
depending on which mode we're compiling for.
This is awkward if we want to refactor do_arm_semihosting()
into other functions which might need to be passed the TaskState.
Restrict usage of the TaskState local by:
* making set_swi_errno() always take the CPUARMState pointer
and (for the linux-user version) get TaskState from that
* creating a new get_swi_errno() which reads the errno
* having the two semihosting calls which need the TaskState
for other purposes (SYS_GET_CMDLINE and SYS_HEAPINFO)
define a variable with scope restricted to just that code

Currently the Arm semihosting code returns the guest file descriptors
(handles) which are simply the fd values from the host OS or the
remote gdbstub. Part of the semihosting 2.0 specification requires
that we implement special handling of opening a ":semihosting-features"
filename. Guest fds which result from opening the special file
won't correspond to host fds, so to ensure that we don't end up
with duplicate fds we need to have QEMU code control the allocation
of the fd values we give the guest.
Add in an abstraction layer which lets us allocate new guest FD
values, and translate from a guest FD value back to the host one.
This also fixes an odd hole where a semihosting guest could
use the semihosting API to read, write or close file descriptors
that it had never allocated but which were being used by QEMU itself.
(This isn't a security hole, because enabling semihosting permits
the guest to do arbitrary file access to the whole host filesystem,
and so should only be done if the guest is completely trusted.)
Currently the only kind of guest fd is one which maps to a
host fd, but in a following commit we will add one which maps
to the :semihosting-features magic data.
If the guest is migrated with an open semihosting file descriptor
then subsequent attempts to use the fd will all fail; this is
not a change from the previous situation (where the host fd
being used on the source end would not be re-opened on the
destination end).

In arm_gdb_syscall() we have a comment suggesting a race
because the syscall completion callback might not happen
before the gdb_do_syscallv() call returns. The comment is
correct that the callback may not happen but incorrect about
the effects. Correct it and note the important caveat that
callers must never do any work of any kind after return from
arm_gdb_syscall() that depends on its return value.

The set_swi_errno() function is called to capture the errno
from a host system call, so that we can return -1 from the
semihosting function and later allow the guest to get a more
specific error code with the SYS_ERRNO function. It comes in
two versions, one for user-only and one for softmmu. We forgot
to capture the errno in the softmmu version; fix the error.
(Semihosting calls directed to gdb are unaffected because
they go through a different code path that captures the
error return from the gdbstub call in arm_semi_cb() or
arm_semi_flen_cb().)

Host kernel within [4.18, 5.3] report an erroneous KVM_MAX_VCPUS=512
for ARM. The actual capability to instantiate more than 256 vcpus
was fixed in 5.4 with the upgrade of the KVM_IRQ_LINE ABI to support
vcpu id encoded on 12 bits instead of 8 and a redistributor consuming
a single KVM IO device instead of 2.
So let's check this capability when attempting to use more than 256
vcpus within any ARM kvm accelerated machine.

Host kernels that expose the KVM_CAP_ARM_IRQ_LINE_LAYOUT_2 capability
allow injection of interrupts along with vcpu ids larger than 255.
Let's encode the vpcu id on 12 bits according to the upgraded KVM_IRQ_LINE
ABI when needed.
Given that we have two callsites that need to assemble
the value for kvm_set_irq(), a new helper routine, kvm_arm_set_irq
is introduced.
Without that patch qemu exits with "kvm_set_irq: Invalid argument"
message.

MVCL is interruptible and we should check for interrupts and process
them after writing back the variables to the registers. Let's check
for any exit requests and exit to the main loop. Introduce a new helper
function for that: cpu_loop_exit_requested().
When booting Fedora 30, I can see a handful of these exits and it seems
to work reliable. Also, Richard explained why this works correctly even
when MVCL is called via EXECUTE:
(1) TB with EXECUTE runs, at address Ae
- env->psw_addr stored with Ae.
- helper_ex() runs, memory address Am computed
from D2a(X2a,B2a) or from psw.addr+RI2.
- env->ex_value stored with memory value modified by R1a
(2) TB of executee runs,
- env->ex_value stored with 0.
- helper_mvcl() runs, using and updating R1b, R1b+1, R2b, R2b+1.
(3a) helper_mvcl() completes,
- TB of executee continues, psw.addr += ilen.
- Next instruction is the one following EXECUTE.
(3b) helper_mvcl() exits to main loop,
- cpu_loop_exit_restore() unwinds psw.addr = Ae.
- Next instruction is the EXECUTE itself...
- goto 1.
As the PoP mentiones that an interruptible instruction called via EXECUTE
should avoid modifying storage/registers that are used by EXECUTE itself,
it is fine to retrigger EXECUTE.

We currently set ilen to AUTO, then overwrite that during
unwinding, then overwrite that for the code access case.
This can be simplified to setting ilen to our arbitrary
value for the (undefined) code access case, then rely on
unwinding to overwrite that with the correct value for
the data access case.

We currently call trigger_pgm_exception to set cs->exception_index
and env->int_pgm_code and then read the values back and then
reset cs->exception_index so that the exception is not delivered.
Instead, use the exception type that we already have directly
without ever triggering an exception that must be suppressed.

We now implement a bunch of new facilities we can properly indicate.
ESOP-1/ESOP-2 handling is discussed in the PoP Chafter 3-15
("Suppression on Protection"). The "Basic suppression-on-protection (SOP)
facility" is a core part of z/Architecture without a facility
indication. ESOP-2 is indicated by ESOP-1 + Side-effect facility
("ESOP-2"). Besides ESOP-2, the side-effect facility is only relevant for
the guarded-storage facility (we don't implement).
S390_ESOP:
- We indicate DAT exeptions by setting bit 61 of the TEID (TEC) to 1 and
bit 60 to zero. We don't trigger ALCP exceptions yet. Also, we set
bit 0-51 and bit 62/63 to the right values.
S390_ACCESS_EXCEPTION_FS_INDICATION:
- The TEID (TEC) properly indicates in bit 52/53 on any access if it was
a fetch or a store
S390_SIDE_EFFECT_ACCESS_ESOP2:
- We have no side-effect accesses (esp., we don't implement the
guarded-storage faciliy), we correctly set bit 64 of the TEID (TEC) to
0 (no side-effect).
- ESOP2: We properly set bit 56, 60, 61 in the TEID (TEC) to indicate the
type of protection. We don't trigger KCP/ALCP exceptions yet.
S390_INSTRUCTION_EXEC_PROT:
- The MMU properly detects and indicates the exception on instruction fetches
- Protected TLB entries will never get PAGE_EXEC set.
There is no need to fake the abscence of any of the facilities - without
the facilities, some bits of the TEID (TEC) are simply unpredictable.
As IEP was added with z14 and we currently implement a z13, add it to
the MAX model instead.

IEP support in the mmu is fairly easy. Set the right permissions for TLB
entries and properly report an exception.
Make sure to handle EDAT-2 by setting bit 56/60/61 of the TEID (TEC) to
the right values.
Let's keep s390_cpu_get_phys_page_debug() working even if IEP is
active. Switch MMU_DATA_LOAD - this has no other effects any more as the
ASC to be used is now fully selected outside of mmu_translate().

We already implement ESOP-1. For ESOP-2, we only have to indicate all
protection exceptions properly. Due to EDAT-1, we already indicate DAT
exceptions properly. We don't trigger KCP/ALCP/IEP exceptions yet.
So all we have to do is set the TEID (TEC) to the right values
(bit 56, 60, 61) in case of LAP.
We don't have any side-effects (e.g., no guarded-storage facility),
therefore, bit 64 of the TEID (TEC) is always 0.
We always have to indicate whether it is a fetch or a store for all access
exceptions. This is only missing for LAP exceptions.

A non-recursive implementation allows to make better use of the
branch predictor, avoids function calls, and makes the implementation of
new features only for a subset of region table levels easier.
We can now directly compare our implementation to the KVM gaccess
implementation in arch/s390/kvm/gaccess.c:guest_translate().

Let's use consistent names for the region/section/page table entries and
for the macros to extract relevant parts from virtual address. Make them
match the definitions in the PoP - e.g., how the relevant bits are actually
called.
Introduce defines for all bits declared in the PoP. This will come in
handy in follow-up patches.
Add a note where additional information about s390x and the used
definitions can be found.

Personally, I've never used these debug statements at all. And if I had
to debug issues, I used plain GDB instead (debug prints are just way too
much noise in the MMU). We might want to introduce tracing at some point
instead, so we can able selected events on demand.

Some secondary controls are automatically enabled/disabled based on the CPUID
values that are set for the guest. However, they are still available at a
global level and therefore should be present when KVM_GET_MSRS is sent to
/dev/kvm.
Unfortunately KVM forgot to include those, so fix that.

Add code to convert the VMX feature words back into MSR values,
allowing the user to enable/disable VMX features as they wish. The same
infrastructure enables support for limiting VMX features in named
CPU models.

VMX requires 64-bit feature words for the IA32_VMX_EPT_VPID_CAP
and IA32_VMX_BASIC MSRs. (The VMX control MSRs are 64-bit wide but
actually have only 32 bits of information).

Sometimes a CPU feature does not make sense unless another is
present. In the case of VMX features, KVM does not even allow
setting the VMX controls to some invalid combinations.
Therefore, this patch adds a generic mechanism that looks for bits
that the user explicitly cleared, and uses them to remove other bits
from the expanded CPU definition. If these dependent bits were also
explicitly *set* by the user, this will be a warning for "-cpu check"
and an error for "-cpu enforce". If not, then the dependent bits are
cleared silently, for convenience.
With VMX features, this will be used so that for example
"-cpu host,-rdrand" will also hide support for RDRAND exiting.

There is a problem, that you don't have access to the data using cpu_memory_rw_debug() function when in SMM. You can't remotely debug SMM mode program because of that for example.
Likely attrs version of get_phys_page_debug should be used to get correct asidx at the end to handle access properly.
Here the patch to fix it.

Now that the parameters to both set_dfp64() and set_dfp128() are exactly the
same, there is no need for an explicit if() statement to determine which
function should be called based upon size. Instead we can simply use the
preprocessor to generate the call to set_dfp##size() directly.

Switch over all accesses to the decimal numbers held in struct PPC_DFP from
using HI_IDX and LO_IDX to using the VsrD() macro instead. Not only does this
allow the compiler to ensure that the various dfp_* functions are being passed
a ppc_vsr_t rather than an arbitrary uint64_t pointer, but also allows the
host endian-specific HI_IDX and LO_IDX to be completely removed from
dfp_helper.c.

There are several places in dfp_helper.c that access the decimal number
representations in struct PPC_DFP via HI_IDX and LO_IDX defines which are set
at the top of dfp_helper.c according to the host endian.
However we can instead switch to using ppc_vsr_t for decimal numbers and then
make subsequent use of the existing VsrD() macros to access the correct
element regardless of host endian. Note that 64-bit decimals are stored in the
LSB of ppc_vsr_t (equivalent to VsrD(1)).

Most of the DFP helper functions call decimal{64,128}FromNumber() just before
returning in order to convert the decNumber stored in dfp.t64 back to a
Decimal{64,128} to write back to the FP registers.
Introduce new dfp_finalize_decimal{64,128}() helper functions which both enable
the parameter list to be reduced considerably, and also help minimise the
changes required in the next patch.

Since commit ef96e3ae96 "target/ppc: move FP and VMX registers into aligned vsr
register array" FP registers are no longer stored consecutively in memory and so
the current method of combining FP register pairs into DFP numbers is incorrect.
Firstly update the definition of the dh_*_fprp defines in helper.h to reflect
that FP registers are now stored as part of an array of ppc_vsr_t elements
rather than plain uint64_t elements, and then introduce a new ppc_fprp_t type
which conceptually represents a DFP even-odd register pair to be consumed by the
DFP helper functions.
Finally update the new DFP {get,set}_dfp{64,128}() helper functions to convert
between DFP numbers and DFP even-odd register pairs correctly, making use of the
existing VsrD() macro to access the correct elements regardless of host endian.

On POWER8 systems the Directed Privileged Door-bell Exception State
register (DPDES) stores doorbell pending status, one bit per a thread
of a core, set by "msgsndp" instruction. The register is shared among
threads of the same core and KVM on POWER9 emulates it in a similar way
(POWER9 does not have DPDES).
DPDES is shared but QEMU assumes all SPRs are per thread so the only safe
way to write DPDES back to VCPU before running a guest is doing so
while all threads are pulled out of the guest so DPDES cannot change.
There is only one situation when this condition is met: incoming migration
when all threads are stopped. Otherwise any QEMU HMP/QMP command causing
kvm_arch_put_registers() (for example printing registers or dumping memory)
can clobber DPDES in a race with other vcpu threads.
This changes DPDES handling so it is not written to KVM at runtime.

instructions: mffsce, mffscdrn, mffscdrni, mffscrn, mffscrni, mffsl.
This patch adds support for 'mffsce' instruction.
'mffsce' is identical to 'mffs', except that it also clears the exception
enable bits in the FPSCR.
On CPUs without support for 'mffsce' (below ISA 3.0), the
instruction will execute identically to 'mffs'.

instructions: mffsce, mffscdrn, mffscdrni, mffscrn, mffscrni, mffsl.
This patch adds support for 'mffscrn' and 'mffscrni' instructions.
'mffscrn' and 'mffscrni' are similar to 'mffsl', except they do not return
the status bits (FI, FR, FPRF) and they also set the rounding mode in the
FPSCR.
On CPUs without support for 'mffscrn'/'mffscrni' (below ISA 3.0), the

s390 was trying to solve limited KVM memslot size issue by abusing
memory_region_allocate_system_memory(), which breaks API contract
where the function might be called only once.
Beside an invalid use of API, the approach also introduced migration
issue, since RAM chunks for each KVM_SLOT_MAX_BYTES are transferred in
migration stream as separate RAMBlocks.
After discussion [1], it was agreed to break migration from older
QEMU for guest with RAM >8Tb (as it was relatively new (since 2.12)
and considered to be not actually used downstream).
Migration should keep working for guests with less than 8TB and for
more than 8TB with QEMU 4.2 and newer binary.
In case user tries to migrate more than 8TB guest, between incompatible
QEMU versions, migration should fail gracefully due to non-exiting
RAMBlock ID or RAMBlock size mismatch.
Taking in account above and that now KVM code is able to split too
big MemorySection into several memslots, partially revert commit
(bb223055b s390-ccw-virtio: allow for systems larger that 7.999TB)
and use kvm_set_max_memslot_size() to set KVMSlot size to
KVM_SLOT_MAX_BYTES.

For AArch64 CPUs with a CBAR register, we have two views for it:
- in AArch64 state, the CBAR_EL1 register (S3_1_C15_C3_0), returns the
full 64 bits CBAR value
- in AArch32 state, the CBAR register (cp15, opc1=1, CRn=15, CRm=3, opc2=0)
returns a 32 bits view such that:
CBAR = CBAR_EL1[31:18] 0..0 CBAR_EL1[43:32]
This commit fixes the current implementation where:
- CBAR_EL1 was returning the 32 bits view instead of the full 64 bits
value,
- CBAR was returning a truncated 32 bits version of the full 64 bits
one, instead of the 32 bits view
- CBAR was declared as cp15, opc1=4, CRn=15, CRm=0, opc2=0, which is
the CBAR register found in the ARMv7 Cortex-Ax CPUs, but not in
ARMv8 CPUs.

The CONFIG_USER_ONLY adjustment blindly mashed the swcr
exception enable bits into the fpcr exception disable bits.
However, fpcr_exc_enable has already converted the exception
disable bits into the exception status bits in order to make
it easier to mask status bits at runtime.
Instead, merge the swcr enable bits with the fpcr before we
convert to status bits.

We were setting the wrong bit. The fp_status.flush_to_zero
setting is overwritten by either the constant 1 or the value
of fpcr_flush_to_zero depending on bits within an fp insn.

As we are moving between address spaces, we can use access_memmove()
without checking for destructive overlaps (especially of real storage
locations):
"Each storage operand is processed left to right. The
storage-operand-consistency rules are the same as
for MOVE (MVC), except that when the operands
overlap in real storage, the use of the common real-
storage locations is not necessarily recognized."

Replace fast_memmove() variants by access_memmove() variants, that
first try to probe access to all affected pages (maximum is two pages).
Introduce access_get_byte()/access_set_byte(). We might be able to speed
up memmove in special cases even further (do single-byte access, use
memmove() for remaining bytes in page), however, we'll skip that for now.
In MVCOS, simply always call access_memmove_as() and drop the TODO
about LAP. LAP is already handled in the MMU.
Get rid of adj_len_to_page(), which is now unused.

Replace fast_memset() by access_memset(), that first tries to probe
access to all affected pages (maximum is two). We'll use the same
mechanism for other types of accesses soon.
Only in very rare cases (especially TLB_NOTDIRTY), we'll have to
fallback to ld/st helpers. Try to speed up that case as suggested by
Richard.
We'll rework most involved handlers soon to do all accesses via new
fault-safe helpers, especially MVC.

Process max 4k bytes at a time, writing back registers between the
accesses. The instruction is interruptible.
"For operands longer than 2K bytes, access exceptions are not
recognized for locations more than 2K bytes beyond the current location
being processed."
Note that on z/Architecture, 2k vs. 4k access cannot get differentiated as
long as pages are not crossed. This seems to be a leftover from ESA/390.
Simply stay within single pages.
MVCL handling is quite different than MVCLE/MVCLU handling, so split up
the handlers.
Defer interrupt handling, as that will require more thought, add a TODO
for that.

Since QEMU v2.10, the KVM acceleration does not work on older kernels
anymore since the code accidentally requires the KVM_CAP_DEVICE_CTRL
capability now - it should have been optional instead.
Instead of fixing the bug, we asked in the ChangeLog of QEMU 2.11 - 3.0
that people should speak up if they still need support of QEMU running
with KVM on older kernels, but seems like nobody really complained.
Thus let's make this official now and turn it into a proper error
message, telling the users to use at least kernel 3.15 now.

While debugging an application with GDB the following might happen:

This is because riscv_gdb_get_fpu calls riscv_csrrw_debug with a wrong csr
number (8). It should use the csr_register_map in order to reach the
riscv_cpu_get_fflags callback.

This adds 'info mem' command for RISC-V, to show virtual memory
mappings that aids debugging.
Rather than showing every valid PTE, the command compacts the
output by merging all contiguous physical address mappings into
one block and only shows the merged block mapping details.

For RV32, the root page table's PPN has 22 bits hence its address
bits could be larger than the maximum bits that target_ulong is
able to represent. Use hwaddr instead.

enabled. We can then protect all floating point operations based on if
they are enabled.
This patch so far doesn't change anything, it's just preparing for the

Switch the SPARC target from the old unassigned_access hook to the
new do_transaction_failed hook.
This will cause the "if transaction failed" code paths added in
the previous commits to become active if the access is to an
unassigned address. In particular we'll now handle bus errors
during page table walks correctly (generating a translation
error with the right kind of fault status).

Convert the mmu_probe() function to using address_space_ldl()
rather than ldl_phys(), so we can explicitly detect memory
transaction failures.
This makes no practical difference at the moment, because
ldl_phys() will return 0 on a transaction failure, and we
treat transaction failures and 0 PDEs identically. However
the spec says that MMU probe operations are supposed to
update the fault status registers, and if we ever implement
that we'll want to distinguish the difference. For the
moment, just add a TODO comment about the bug.

Currently we use the ldl_phys() function to read page table entries.
With the unassigned_access hook in place, if these hit an unassigned
area of memory then the hook will cause us to wrongly generate
an exception with a fault address matching the address of the
page table entry.
Change to using address_space_ldl() so we can detect and correctly
handle bus errors and give them their correct behaviour of
causing a translation error with a suitable fault status register.
Note that this won't actually take effect until we switch the
over to using the do_translation_failed hook.

ld*_phys() and st*_phys() functions for those ASIs which
imply direct accesses to physical addresses. These implicitly
rely on the unassigned_access hook to cause them to generate
an MMU fault if the access fails.
Switch to using the address_space_* functions instead, which
return a MemTxResult that we can check. This means that when
we switch SPARC over to using the do_transaction_failed hook
we'll still get the same MMU faults we did before.
This commit converts the ASIs which do MXCC stream source
and destination accesses.
It's not clear to me whether raising an MMU fault like this
is the correct behaviour if we encounter a bus error, but
we retain the same behaviour that the old unassigned_access
hook would implement.

Currently the SPARC target uses the old-style do_unassigned_access
hook. We want to switch it over to do_transaction_failed, but to do
this we must first remove all the direct calls in ldst_helper.c to
cpu_unassigned_access(). Factor out the body of the hook function's
code into a new sparc_raise_mmu_fault() and call it from the hook and
from the various places that used to call cpu_unassigned_access().
In passing, this fixes a bug where the code that raised the
MMU exception was directly calling GETPC() from a function that
was several levels deep in the callstack from the original
helper function: the new sparc_raise_mmu_fault() instead takes
the return address as an argument.
Other than the use of retaddr rather than GETPC() and a comment
format fixup, the body of the new function has no changes from
that of the old hook function.
