Set the NaN propagation rule explicitly in xtensa_use_first_nan().
(When we convert the softfloat pickNaNMulAdd routine to also
select a NaN propagation rule at runtime, we will be able to
remove the use_first_nan flag because the propagation rules
will handle everything.)

In xtensa we currently call set_use_first_nan() in a lot of
places where we want to switch the NaN-propagation handling.
We're about to change the softfloat API we use to do that,
so start by factoring all the calls out into a single
xtensa_use_first_nan() function.
The bulk of this change was done with
sed -i -e 's/set_use_first_nan(\([^,]*\),[^)]*)/xtensa_use_first_nan(env, \1)/' target/xtensa/fpu_helper.c

In commit ad18376b90 we added an assert that the level value was
in-bounds for the array we're about to index into. However, the
assert condition is wrong -- env->config->interrupt_vector is an
array of uint32_t, so we should bounds check the index against
ARRAY_SIZE(...), not against sizeof().

Coverity gets confused about the use of the 'segment' variable in the
pptlb helper function: it thinks that we can take a code path where
we first initialize it:
unsigned segment = XTENSA_MPU_PROBE_B; // 0x40000000
and then use that value as a shift count:
} else if (nhits == 1 && (env->sregs[MPUENB] & (1u << segment))) {
In fact this isn't possible, beacuse xtensa_mpu_lookup() is passed
'&segment', and it uses that as an output value, which it will always
set if it returns nonzero. But the way the code is currently written
is confusing to a human reader as well as to Coverity.
Instead of initializing 'segment' at the top of the function with a
value that's only used in the "nhits == 0" code path, use the
constant value directly in that code path, and don't initialize
segment. This matches the way we use xtensa_mpu_lookup() in its
other callsites in get_physical_addr_mpu().

The semihosting feature depends on TCG (due to the probe_access
API access). Although TCG is the single accelerator currently
available for the xtensa target, use the Kconfig "imply" directive
which is more correct (if we were to support a different accel).

Currently the TCGCPUOps::cpu_exec_halt method is optional, and if it
is not set then the default is to call the CPUClass::has_work
method (which has an identical function signature).
We would like to make the cpu_exec_halt method mandatory so we can
remove the runtime check and fallback handling. In preparation for
that, make all the targets which don't need special handling in their
cpu_exec_halt set it to their cpu_has_work implementation instead of
leaving it unset. (This is every target except for arm and i386.)
In the riscv case this requires us to make the function not
be local to the source file it's defined in.

accel/tcg/ files requires the following definitions:
- TARGET_LONG_BITS
- TARGET_PAGE_BITS
- TARGET_PHYS_ADDR_SPACE_BITS
- TCG_GUEST_DEFAULT_MO
The first 3 are defined in "cpu-param.h". The last one
in "cpu.h", with a bunch of definitions irrelevant for
TCG. By moving the TCG_GUEST_DEFAULT_MO definition to
"cpu-param.h", we can simplify various accel/tcg includes.

We pass a ResetType argument to the Resettable class enter
phase method, but we don't pass it to hold and exit, even though
the callsites have it readily available. This means that if
a device cared about the ResetType it would need to record it
in the enter phase method to use later on. Pass the type to
all three of the phase methods to avoid having to do that.

Since CPU() macro is a simple cast, the following are equivalent:

In order to ease static analysis when running
scripts/coccinelle/cpu_env.cocci from the previous commit,
replace:

+ CPU_GET_CLASS(obj);
Most code use the 'cs' variable name for CPUState handle.
Replace few 's' -> 'cs' to unify cpu_reset_hold() style.
No logical change in this patch.

r[id]tlb[01], [iw][id]tlb opcodes use TLB way index passed in a register
by the guest. The host uses 3 bits of the index for ITLB indexing and 4
bits for DTLB, but there's only 7 entries in the ITLB array and 10 in
the DTLB array, so a malicious guest may trigger out-of-bound access to
these arrays.
Change split_tlb_entry_spec return type to bool to indicate whether TLB
way passed to it is valid. Change get_tlb_entry to return NULL in case
invalid TLB way is requested. Add assertion to xtensa_tlb_get_entry that
requested TLB way and entry indices are valid. Add checks to the
[rwi]tlb helpers that requested TLB way is valid and return 0 or do
nothing when it's not.

Don't embed ibreak exception generation into TB and don't invalidate TB
on ibreak address change. Add CPUBreakpoint pointers to xtensa
CPUArchState, use cpu_breakpoint_insert/cpu_breakpoint_remove_by_ref to
manage ibreak breakpoints and provide TCGCPUOps::debug_check_breakpoint
callback that recognizes valid instruction breakpoints.

The Big QEMU Lock (BQL) has many names and they are confusing. The
actual QemuMutex variable is called qemu_global_mutex but it's commonly
referred to as the BQL in discussions and some code comments. The
locking APIs, however, are called qemu_mutex_lock_iothread() and
qemu_mutex_unlock_iothread().
The "iothread" name is historic and comes from when the main thread was
split into into KVM vcpu threads and the "iothread" (now called the main
loop thread). I have contributed to the confusion myself by introducing
a separate --object iothread, a separate concept unrelated to the BQL.
The "iothread" name is no longer appropriate for the BQL. Rename the
locking APIs to:
- void bql_lock(void)
- void bql_unlock(void)
- bool bql_locked(void)
There are more APIs with "iothread" in their names. Subsequent patches
will rename them. There are also comments and documentation that will be
updated in later patches.

For all targets, the CPU class returned from CPUClass::class_by_name()
and object_class_dynamic_cast(oc, CPU_RESOLVING_TYPE) need to be
compatible. Lets apply the check in cpu_class_by_name() for once,
instead of having the check in CPUClass::class_by_name() for individual
target.

The OBJECT_DECLARE_CPU_TYPE() macro forward-declares each
ArchCPUClass type. These forward declarations are sufficient
for code in hw/ to use the QOM definitions. No need to expose
these structure definitions. Keep each local to their target/
by moving them to the corresponding "cpu.h" header.

When CPUArchState* is available (here CPUXtensaState*), we
can use the fast env_archcpu() macro to get ArchCPU* (here
XtensaCPU*). The QOM cast XTENSA_CPU() macro will be slower
when building with --enable-qom-cast-debug.

Hegerogeneous code needs access to the FOO_CPU_TYPE_NAME()
macro to resolve target CPU types. Move the declaration
(along with the required FOO_CPU_TYPE_SUFFIX) to "cpu-qom.h".
"target/foo/cpu-qom.h" is supposed to be target agnostic
(include-able by any target). Add such mention in the
header.

Enforce the style described by commit 067109a11c ("docs/devel:
mention the spacing requirement for QOM"):
The first declaration of a storage or class structure should
always be the parent and leave a visual space between that
declaration and the new code. It is also useful to separate
backing for properties (options driven by the user) and internal
state to make navigation easier.

In handle_interrupt() we use level as an index into the interrupt_vector[]
array. This is safe because we have checked it against env->config->nlevel,
but Coverity can't see that (and it is only true because each CPU config
sets its XCHAL_NUM_INTLEVELS to something less than MAX_NLEVELS), so it
complains about a possible array overrun (CID 1507131)
Add an assert() which will make Coverity happy and catch the unlikely
case of a mis-set XCHAL_NUM_INTLEVELS in future.

We use the user_ss[] array to hold the user emulation sources,
and the softmmu_ss[] array to hold the system emulation ones.
Hold the latter in the 'system_ss[]' array for parity with user
emulation.

In preparation for compiling tcg/ only once, eliminate
the all_helpers array. Instantiate the info structs for
the generic helpers in accel/tcg/, and the structs for
the target-specific helpers in each translate.c.
Since we don't see all of the info structs at startup,
initialize at first use, using g_once_init_* to make
sure we don't race while doing so.

These inline helpers are all used by target specific code so move them
out of the general header so we don't needlessly pollute the rest of
the API with target specific stuff.
Note we have to include cpu.h in semihosting as it was relying on a
side effect before.
