29a9a3d9b3 Work around yet more LKM system braindamage tickled by rev 1.418 by
including layer_*.c if "options LKM". This is not the right way to fix
it, but we have as yet no standardized mechanism to add a "LKM code
library" to fill in gaps in the monolithic kernel's code.

Problem noted by <tron@netbsd.org>.

5706160195 fix several bugs:
- in the cases where we skip over the i/o loop, increment npages by ridx
so that when the cleanup code starts processing the pgs array at index 0
it'll actually process all of the pages.
 - process the PG_RELEASED flag when unbusying pages.
 - add some missing MP locking.
 - use MIN() and MAX() instead of min() and max() since the latter are
functions which take arguments of type "int" but we call them with
values of type "off_t", so the values could be truncated.

end of the requested range of pages as the file size for this request.
this fixes some problems with sparsing writes to large offsets.

4d5451090e in uvm_map_clean(), fix the case where the start offset is within the last
entry in the map. the old code would walk around the end of the linked list,
through the header entry, and keep going from the first map entry until it
found a gap in the map, at which point it would return an error. if the map
had no gaps then it would loop forever. reported by k-abe@cs.utah.edu.
while I'm here, clean up this function a bit.

also, use MIN() instead of min(), since the latter takes arguments of
type "int" but we're passing it values of type "vaddr_t", which can be
a larger size.

modified attribute checking actually work. Previously, a

"clean" pages of read-only files.

Only lip-services is paid to referenced attribute emulation.

The main advantage of this version is that is uses rasops (which
is faster than the rcons stuff) and wsfont, so you can select the
console font in the kernel config file. Using the BOLD8x16 font,
you get an 80x30 console (vast improvement compared to the Gallant12x22

5ce651390d Fix problem where tsleep() is used not on the cardbus thread.
This change makes PCMCIA cards work again on my machine.

Unfortunately, some of the DELAY_MS() are called when the current
process is not the cardbus thread, that is, on interrupt contexts.
I confirmed these are actually used in interrupt context and replaced
them with delay()s.  However, I haven't confirmed other DELAY_MS()s
are always on the cardbus thread.

This shall be a temporary fix. The driver needs being rewritten
not to use such delay()s, of course.

1779f8f71b Page scanner improvements, behavior is actually a bit more like
Mach VM's now. Specific changes:
- Pages now need not have all of their mappings removed before being
put on the inactive list. They only need to have the "referenced"
attribute cleared. This makes putting pages onto the inactive list
much more efficient. In order to eliminate redundant clearings of
"refrenced", callers of uvm_pagedeactivate() must now do this
  themselves.
- When checking the "modified" attribute for a page (for clearing
PG_CLEAN), make sure to only do it if PG_CLEAN is currently set on
the page (saves a potentially expensive pmap operation).
- When scanning the inactive list, if a page is referenced, reactivate
it (this part was actually added in uvm_pdaemon.c,v 1.27). This
now works properly now that pages on the inactive list are allowed to
  have mappings.
- When scanning the inactive list and considering a page for freeing,
remove all mappings, and then check the "modified" attribute if the
  page is marked PG_CLEAN.
- When scanning the active list, if the page was referenced since its
last sweep by the scanner, don't deactivate it. (This part was
  actually added in uvm_pdaemon.c,v 1.28.)

These changes greatly improve interactive performance during
moderate to high memory and I/O load.

a7206004fc Faster error handling for iommu_dmamap_load_raw; avoid loading the map
and unloading on error when we clearly can't get out without an error.

75ea8b428e Having moved some of the bits needed by raidctl into rf_configure.c, we
no longer need those bits here. In particular, this file is now only
used in the kernel, and that allows us to clean up a lot of cruft.

02adaaf197 cleanup cloned route when parent route (RTF_CLONING) goes away.
adds rt_parent to link parent from child (like NRL did, ours do refcnt
rt_refcnt properly).

bsdi rt_walkbranch would speedup the processing, but since the code will not
be visited too frequently, the current code (with rt_walktree) should be okay.
