e919735e35 Add in (enabled by SCSIVERBOSE option since it adds a fair amount of string
gup to the kernel) more detailed SCSI error printing, based off of final
SCSI-2 specification. This will also print out things like progress indicator
errors, and will also dump out additional sense bytes that haven't been decoded.

Also, turned the error message 'DELAYED' to 'DEFERRED' since that's more
accurate.

332ad95e76 Change the MI audio driver so it attaches to the MD driver in the
normal way.  This requires adding a line to the config files to
get audio to work again.

e5b75f05d4 Make a slight change to when blocksize, hi- and lo-water marks are
recalculated.  The new semantics is more intuitive(?) and makes Doom
work :-)

23d0cae1eb It helps streaming performance substantially to enable bursts and more up hose
packets. It's not clear whether latency will be affected much. It could be.
However, for the time being, getting full rates out of HIPPI or SCSI are what
interests us at NASA, so we'll hew to this bias.

24add65376 Major rework to support Qlogic ISP2100 (Fibre Channel PCI card). Also now
able to read SXP registers (instead of panicing). Probably have done a bit
of setup overkill- we now alloc some scratch memory for the 2100 that I thought
I would need for the Port database, but so far I haven't needed to retrieve
the port database. Well, early days yet.

14a7730c16 *Major* differences to support the Qlogic 2100 (Fibre Channel PCI), as well

d540ff911c wrap bus_space I/O tag checks with #ifdef DIAGNOSTIC.
wrap actual read port map/unmap with #ifdef _KERNEL, so userland probe
  code will still work.

efecf08fef Sync with bus_space(9):
* make map and alloc take 'flags' rather than 'cacheable,' for
  more flexibility.
* rename BUS_BARRIER_* to BUS_SPACE_BARRIER_*, for consistency.
* rename bus_space_copy_* to bus_space_copy_region_* and make their
  defns match the updated spec.
Backward compatibility is provided by defining __BUS_SPACE_COMPAT_OLDDEFS,
which is currently defined by default.

c7079260e5 More machine support:
- Add entries for the LC520, Color Classic, and PB150 to intvid_info[].
 - Make the PB150 a Duo-class machine, as it seems to work better this way.

Now that we're doing the latter, it points out something I missed a long
time ago when I added Duo support:  CLASSDUO was being ignored in
mac68k_set_io_offsets().

in PR 3970.

473b4f1222 Initialize system console from a central place (don't use cninit()
anymore). This makes the code cleaner (no more bus tag guessing in the
serial console driver) and it allows to share code with KGDB port
initialization.
The console device is now given as string ("pc", "com0".."com3").
Modelled after alpha port.

40db8a98f5 1. Allow to set a netmask (option NFS_BOOT_NETMASK) for the booting
interface. Without this, NFS_BOOT_NETMASK could be useless in
   subnetting envirinment.
2. Comment out unneeded NFS swap related stuff.
   Closes PR kern/3918.

2092d92ef2 New, optional, method to get size of extended memory.
It will be used for getextmem() if EXTENDED_MEMDETECT is defined.

945beb8d63 Move procfs_checkioperm() from procvs_subr.c to procfs_mem.c, since _subr is
not included in a kernel without procfs, and it seems wrong to pull
all of procfs_subr.c in for just that one function.  Perhaps this
should go into a new file instead?

f9777e4a1c Fix the procfs hole described on current-users, similar to a fix for
FreeBSD by Sean Eric Fagan, but a bit different.  This makes the checks
in the same places as sef's FreeBSD patch, but does not hardcode the
"kmem" group into the kernel, and also does a check identical to the
(3) and (4) checks in the NetBSD ptrace(2):

	(1) it's not owned by you, or is set-id on exec (unless
	    you're root), or

	(2) it's init, which controls the security level of the
	    entire system, and the system was not compiled with
	    permanently insecure mode turned on.

d5998780c0 KGDB improvements:
-Separate KGDB port initialization and softc related stuff to allow
 KGDB to be attached in early system startup, before autoconfiguration.
-Export the variables needed by md code to hand-craft bus tag/handle.
-Fix initialization to enable interrupt by line break.
-Call DDB/KGDB at line break (move DDB call from the softirq handler
 to the hard handler because it should work without a tty attached too).

95a12ee943 MIPS cpu-speed detection using mc146818 clock.
Compute CPU speed(MHz) and loop multiplier for DELAY() based on
counting empty loop between mcclock ticks.  New global: cpu_mhz.
Change pmax/pmax/machdep.c to build baseboard model names from cpu_mhz.
Set  'cpuspeed' for more realistic DELAY() on mips3 models.

003ccf3b1c mips pmap_activate:
* prototype and definition for pmap_activate(p). Updates the segtab,
   and changes the active ASID if p == curproc.
 * Make reserved fixed-address (UADDR) kernelstack PTEs global,
   so we still have a kernel stack after pmap_activate() on curproc.
 * make KSEG2 mappings for p_addr global (see above.)

Seems to detune contextswitch and NTP resolution (by 60 ms), but
thepmap_activate() interface is mandatory.  Needs more thought.
