When the user didn't specify "intremap" for the IOMMU device, we turn
it on by default if it is supported. This will turn IR on for the
default Q35 platform as long as the IOMMU device is specified on new
kernels.

Switch the intr_supported variable from a boolean to OnOffAuto type so
that we can know whether the user specified it or not. With that
we'll have a chance to help the user to choose more wisely where
possible. Introduce x86_iommu_ir_supported() to mask these changes.
No functional change at all.

Starting from QEMU 4.0, let's specify "split" as the default value for
kernel-irqchip.
So for QEMU>=4.0 we'll have: allowed=Y,required=N,split=Y
for QEMU<=3.1 we'll have: allowed=Y,required=N,split=N
(omitting all the "kernel_irqchip_" prefix)
Note that this will let the default q35 machine type to depend on
Linux version 4.4 or newer because that's where split irqchip is
introduced in kernel. But it's fine since we're boosting supported
Linux version for QEMU 4.0 to around Linux 4.5. For more information

AcpiRsdpDescriptor describes revision 2 RSDP table so using sizeof(*rsdp)
for checksum calculation isn't correct since we are adding extra 16 bytes.
But acpi_data_push() zeroes out table, so just by luck we are summing up
exta zeros which still yelds correct checksum.
Fix it up by explicitly stating table size instead of using
pointer arithmetics on stucture.
PS:
Extra 16 bytes are still wasted, but droping them will break migration
for machines older than 2.3 due to size mismatch, for 2.3 and older it's
not an issue since they are using resizable memory regions (a1666142d)
for ACPI blobs. So keep wasting memory to avoid breaking old machines.

Support DMA read/write draining should be easy for existing VT-d
emulation since the emulation itself does not have any request queue
there so we don't need to do anything to flush the un-commited queue.
What we need to do is to declare the support.
These capabilities are required to pass Windows SVVP test program. It
is verified that when with parameters "x-aw-bits=48,caching-mode=off"
we can pass the Windows SVVP test with this patch applied. Otherwise
we'll fail with:
IOMMU[0] - DWD (DMA write draining) not supported
IOMMU[0] - DWD (DMA read draining) not supported
Segment 0 has no DMA remapping capable IOMMU units
However since these bits are not declared support for QEMU<=3.1, we'll
need a compatibility bit for it and we turn this on by default only
for QEMU>=4.0.

When issuing the qmp/hmp 'system_wakeup' command, what happens in a
nutshell is:
- qmp_system_wakeup_request set runstate to RUNNING, sets a wakeup_reason
and notify the event
- in the main_loop, all vcpus are paused, a system reset is issued, all
subscribers of wakeup_notifiers receives a notification, vcpus are then
resumed and the wake up QAPI event is fired
Note that this procedure alone doesn't ensure that the guest will awake
from SUSPENDED state - the subscribers of the wake up event must take
action to resume the guest, otherwise the guest will simply reboot. At
this moment, only the ACPI machines via acpi_pm1_cnt_init and xen_hvm_init
have wake-up from suspend support.
However, only the presence of 'system_wakeup' is required for QGA to
support 'guest-suspend-ram' and 'guest-suspend-hybrid' at this moment.
This means that the user/management will expect to suspend the guest using
one of those suspend commands and then resume execution using system_wakeup,
regardless of the support offered in system_wakeup in the first place.
This patch creates a new API called query-current-machine [1], that holds
a new flag called 'wakeup-suspend-support' that indicates if the guest
supports wake up from suspend via system_wakeup. The machine is considered
to implement wake-up support if a call to a new 'qemu_register_wakeup_support'
is made during its init, as it is now being done inside acpi_pm1_cnt_init
and xen_hvm_init. This allows for any other machine type to declare wake-up
support regardless of ACPI state or wakeup_notifiers subscription, making easier
for newer implementations that might have their own mechanisms in the future.
This is the expected output of query-current-machine when running a x86
guest:

With this extra tool, management can avoid situations where a guest
that does not have proper suspend/wake capabilities ends up in
inconsistent state (e.g.
https://github.com/open-power-host-os/qemu/issues/31).
[1] the decision of creating the query-current-machine API is based
on discussions in the QEMU mailing list where it was decided that
query-target wasn't a proper place to store the wake-up flag, neither
was query-machines because this isn't a static property of the
machine object. This new API can then be used to store other
dynamic machine properties that are scattered around the code

Now that amd-iommu support interrupt remapping, enable the GASup in IVRS
table and GASup in extended feature register to indicate that IOMMU
support guest virtual APIC mode. GASup provides option to guest OS to
make use of 128-bit IRTE.
Note that the GAMSup is set to zero to indicate that amd-iommu does not
support guest virtual APIC mode (aka AVIC) which would be used for the
nested VMs.

Currently, the amdvi_validate_dte() assumes that a valid DTE will
always have V=1. This is not true. The V=1 means that bit[127:1] are
valid. A valid DTE can have IV=1 and V=0 (i.e address translation
disabled and interrupt remapping enabled)
Remove the V=1 check from amdvi_validate_dte(), make the caller
responsible to check for V or IV bits.
This also fixes a bug in existing code that when error is
detected during the translation we'll fail the translation
instead of assuming a passthrough mode.

The vtd_generate_msi_message() in intel-iommu is used to construct a MSI
Message from IRQ. A similar function will be needed when we add interrupt
remapping support in amd-iommu. Moving the function in common file to
avoid the code duplication. Rename it to x86_iommu_irq_to_msi_message().
There is no logic changes in the code flow.

We should handle VTD_FR_CONTEXT_ENTRY_P properly when synchronizing
shadow page tables. Having invalid context entry there is perfectly
valid when we move a device out of an existing domain. When that
happens, instead of posting an error we invalidate the whole region.
Without this patch, QEMU will crash if we do these steps:
(1) start QEMU with VT-d IOMMU and two 10G NICs (ixgbe)
(2) bind the NICs with vfio-pci in the guest
(3) start testpmd with the NICs applied
(4) stop testpmd
(5) rebind the NIC back to ixgbe kernel driver
The patch should fix it.

There are two callers for vtd_sync_shadow_page_table_range(): one
provided a valid context entry and one not. Move that fetching
operation into the caller vtd_sync_shadow_page_table() where we need to
fetch the context entry.
Meanwhile, remove the error_report_once() directly since we're already
tracing all the error cases in the previous call. Instead, return error
number back to caller. This will not change anything functional since
callers are dropping it after all.
We do this move majorly because we want to do something more later in
vtd_sync_shadow_page_table().

QEMU is not handling the global DMAR switch well, especially when from
"on" to "off".
Let's first take the example of system reset.
Assuming that a guest has IOMMU enabled. When it reboots, we will drop
all the existing DMAR mappings to handle the system reset, however we'll
still keep the existing memory layouts which has the IOMMU memory region
enabled. So after the reboot and before the kernel reloads again, there
will be no mapping at all for the host device. That's problematic since
any software (for example, SeaBIOS) that runs earlier than the kernel
after the reboot will assume the IOMMU is disabled, so any DMA from the
software will fail.
For example, a guest that boots on an assigned NVMe device might fail to
find the boot device after a system reboot/reset and we'll be able to
observe SeaBIOS errors if we capture the debugging log:
WARNING - Timeout at nvme_wait:144!
Meanwhile, we should see DMAR errors on the host of that NVMe device.
It's the DMA fault that caused a NVMe driver timeout.
The correct fix should be that we do proper switching of device DMA
address spaces when system resets, which will setup correct memory
regions and notify the backend of the devices. This might not affect
much on non-assigned devices since QEMU VT-d emulation will assume a
default passthrough mapping if DMAR is not enabled in the GCMD
register (please refer to vtd_iommu_translate). However that's required
for an assigned devices, since that'll rebuild the correct GPA to HPA
mapping that is needed for any DMA operation during guest bootstrap.
Besides the system reset, we have some other places that might change
the global DMAR status and we'd better do the same thing there. For
example, when we change the state of GCMD register, or the DMAR root
pointer. Do the same refresh for all these places. For these two
places we'll also need to explicitly invalidate the context entry cache
and iotlb cache.

attemped to fix hotplug regression introduced by
848a1cc1e "hw/acpi-build: build SRAT memory affinity structures for DIMM devices"
fixed issue for Windows/3.0+ linux kernels, however it regressed 2.6 based
kernels (RHEL6) to the point where guest might crash at boot.
Reason is that 2.6 kernel discards SRAT table due too small last entry
which down the road leads to crashes. Hack I've tried in 10efd7e108 is also
not ACPI spec compliant according to which whole possible RAM should be
described in SRAT. Revert 10efd7e108 to fix regression for 2.6 based kernels.
With 10efd7e108 reverted, I've also tried splitting SRAT table statically
in different ways %/node and %/slot but Windows still fails to online
2nd pc-dimm hot-plugged into node 0 (as described in 10efd7e108) and
sometimes even coldplugged pc-dimms where affected with static SRAT
partitioning.
The only known so far way where Windows stays happy is when we have 1
SRAT entry in the last node covering all hotplug area.
Revert 848a1cc1e until we come up with a way to avoid regression
on Windows with hotplug area split in several entries.
Tested this with 2.6/3.0 based kernels (RHEL6/7) and WS20[08/12/12R2/16]).

Replace existing trace_vtd_err() with error_report_once() then stderr
will capture something if any of the error happens, meanwhile we don't
suffer from any DDOS. Then remove the trace point. Since at it,
provide more information where proper (now we can pass parameters into
the report function).

We can assign and verify the address before realizing and trying to plug.
reading/writing the address property should never fail for DIMMs, so let's
reduce error handling a bit by using &error_abort. Getting access to the
memory region now might however fail. So forward errors from
get_memory_region() properly.
As all memory devices should use the alignment of the underlying memory
region for guest physical address asignment, do detection of the
alignment in pc_dimm_pre_plug(), but allow pc.c to overwrite the
alignment for compatibility handling.

All applicable memory regions always have an alignment > 0. All memory
backends result in file_ram_alloc() or qemu_anon_ram_alloc() getting
called, setting the alignment to > 0.
So a PCDIMM memory region always has an alignment > 0. NVDIMM copy the
alignment of the original memory memory region into the handcrafted memory
region that will be used at this place.
So the check for 0 can be dropped and we can reduce the special
handling.
Dropping this check makes factoring out of alignment handling easier as
compat handling only has to look at pcmc->enforce_aligned_dimm and not
care about the alignment of the memory region.

The multiboot code parses the initrd_filename twice, first to count how
many entries there are, and second to process each entry. This changes
the first loop to store the parse module names in a list, and the second
loop can now use these names. This avoids having to pass NULL to the
get_opt_value() method which means it can safely assume a non-NULL param.

Hyper-V identifies vCPUs by Virtual Processor (VP) index which can be
queried by the guest via HV_X64_MSR_VP_INDEX msr. It is defined by the
spec as a sequential number which can't exceed the maximum number of
vCPUs per VM.
It has to be owned by QEMU in order to preserve it across migration.
However, the initial implementation in KVM didn't allow to set this
msr, and KVM used its own notion of VP index. Fortunately, the way
vCPUs are created in QEMU/KVM makes it likely that the KVM value is
equal to QEMU cpu_index.
So choose cpu_index as the value for vp_index, and push that to KVM on
kernels that support setting the msr. On older ones that don't, query
the kernel value and assert that it's in sync with QEMU.
Besides, since handling errors from vCPU init at hotplug time is
impossible, disable vCPU hotplug.
This patch also introduces accessor functions to encapsulate the mapping
between a vCPU and its vp_index.

This include both userspace and in-kernel ioapic. Note that the numbers
can be inaccurate for kvm-ioapic. One reason is the same with
kvm-i8259, that when irqfd is used, irqs can be delivered all inside
kernel without our notice. Meanwhile, kvm-ioapic is specially treated
when irq numbers <ISA_NUM_IRQS, those irqs will be delivered in kernel
too via kvm-i8259 (please refer to kvm_pc_gsi_handler).

The oldest machine type which is still used in a still maintained distro
is a pc-0.12 based machine type in RHEL6, so everything that is older
than pc-0.12 should not be used anymore. Thus let's deprecate pc-0.10
and pc-0.11 so that we can finally remove them in a future release.

Add support for multiple IOMMU indexes to the IOMMU notifier APIs.
When initializing a notifier with iommu_notifier_init(), the caller
must pass the IOMMU index that it is interested in. When a change
happens, the IOMMU implementation must pass
memory_region_notify_iommu() the IOMMU index that has changed and
that notifiers must be called for.
IOMMUs which support only a single index don't need to change.
Callers which only really support working with IOMMUs with a single
index can use the result of passing MEMTXATTRS_UNSPECIFIED to
memory_region_iommu_attrs_to_index().

A link property can be set during creation, with
object_property_add_link() and later with object_property_set_link().
add_link() doesn't add a reference to the target object, while
set_link() does.
Furthemore, OBJ_PROP_LINK_UNREF_ON_RELEASE flags, set during add_link,
says whether a reference must be released when the property is destroyed.
This can lead to leaks if the property was later set_link(), as the
added reference is never released.
Instead, rename OBJ_PROP_LINK_UNREF_ON_RELEASE to OBJ_PROP_LINK_STRONG
and use that has an indication on how the link handle reference
management in set_link().

QEMU 3.0 enables strict check for compression & decompression to
make the migration more robust, that depends on the source to fix
the internal design which triggers the unexpected error conditions
To make it work for migrating old version QEMU to 2.13 QEMU, we
introduce this parameter to disable the error check on the
destination which is the default behavior of the machine type
which is older than 2.13, alternately, the strict check can be
enabled explicitly as followings:

This patch removes the current hackery where IOREQ_TYPE_PCI_CONFIG
requests are handled by faking PIO to 0xcf8 and 0xcfc and replaces it
with direct calls to pci_host_config_read/write_common().
Doing so necessitates mapping BDFs to PCIDevices but maintaining a simple
QLIST in xen_device_realize/unrealize() will suffice.
NOTE: whilst config space accesses are currently limited to
PCI_CONFIG_SPACE_SIZE, this patch paves the way to increasing the
limit to PCIE_CONFIG_SPACE_SIZE when Xen gains the ability to
emulate MCFG table accesses.

Xen 4.11 has a new API to directly map guest resources. Among the resources
that can be mapped using this API are ioreq pages.
This patch modifies QEMU to attempt to use the new API should it exist,
falling back to the previous mechanism if it is unavailable.

When global_log_dirty is enabled VRAM modification tracking never
worked correctly. The address that is passed to xen_hvm_modified_memory()
is not the effective PFN but RAM block address which is not the same
for VRAM.
We need to make a translation for this address into PFN using
physmap. Since there is no way to access physmap properly inside
xen_hvm_modified_memory() let's make it a global structure.

This patch fixes a potential small window that the DMA page table might
be incomplete or invalid when the guest sends domain/context
invalidations to a device. This can cause random DMA errors for
assigned devices.
This is a major change to the VT-d shadow page walking logic. It
includes but is not limited to:
- For each VTDAddressSpace, now we maintain what IOVA ranges we have
mapped and what we have not. With that information, now we only send
MAP or UNMAP when necessary. Say, we don't send MAP notifies if we
know we have already mapped the range, meanwhile we don't send UNMAP
notifies if we know we never mapped the range at all.
- Introduce vtd_sync_shadow_page_table[_range] APIs so that we can call
in any places to resync the shadow page table for a device.
- When we receive domain/context invalidation, we should not really run
the replay logic, instead we use the new sync shadow page table API to
resync the whole shadow page table without unmapping the whole
region. After this change, we'll only do the page walk once for each
domain invalidations (before this, it can be multiple, depending on
number of notifiers per address space).
While at it, the page walking logic is also refactored to be simpler.

This patch only modifies the trace points.
Previously we were tracing page walk levels. They are redundant since
we have page mask (size) already. Now we trace something much more
useful which is the domain ID of the page walking. That can be very
useful when we trace more than one devices on the same system, so that
we can know which map is for which domain.

During the recursive page walking of IOVA page tables, some stack
variables are constant variables and never changed during the whole page
walking procedure. Isolate them into a struct so that we don't need to
pass those contants down the stack every time and multiple times.

For UNMAP-only IOMMU notifiers, we don't need to walk the page tables.
Fasten that procedure by skipping the page table walk. That should
boost performance for UNMAP-only notifiers like vhost.

SECURITY IMPLICATION: this patch fixes a potential race when multiple
threads access the IOMMU IOTLB cache.
Add a per-iommu big lock to protect IOMMU status. Currently the only
thing to be protected is the IOTLB/context cache, since that can be
accessed even without BQL, e.g., in IO dataplane.
Note that we don't need to protect device page tables since that's fully
controlled by the guest kernel. However there is still possibility that
malicious drivers will program the device to not obey the rule. In that
case QEMU can't really do anything useful, instead the guest itself will
be responsible for all uncertainties.

SECURITY IMPLICATION: without this patch, any guest with both assigned
device and a vIOMMU might encounter stale IO page mappings even if guest
has already unmapped the page, which may lead to guest memory
corruption. The stale mappings will only be limited to the guest's own
memory range, so it should not affect the host memory or other guests on
the host.
During IOVA page table walking, there is a special case when the PSI
covers one whole PDE (Page Directory Entry, which contains 512 Page
Table Entries) or more. In the past, we skip that entry and we don't
notify the IOMMU notifiers. This is not correct. We should send UNMAP
notification to registered UNMAP notifiers in this case.
For UNMAP only notifiers, this might cause IOTLBs cached in the devices
even if they were already invalid. For MAP/UNMAP notifiers like
vfio-pci, this will cause stale page mappings.
This special case doesn't trigger often, but it is very easy to be
triggered by nested device assignments, since in that case we'll
possibly map the whole L2 guest RAM region into the device's IOVA
address space (several GBs at least), which is far bigger than normal
kernel driver usages of the device (tens of MBs normally).
Without this patch applied to L1 QEMU, nested device assignment to L2
guests will dump some errors like:

This should help to avoid problems with accessing the device after
migration/resume without PV drivers by migrating its PCI configuration
space state. Without an explicitly defined state record it resets
every time a VM migrates which confuses the OS and makes every
access to xen-pvdevice MMIO region to fail. PV tools enable some
logic to save and restore PCI configuration state from within the VM
every time it migrates which basically hides the issue.
Older systems will acquire the new record when migrated which should
not change their state for worse.

By default MachineClass::get_hotplug_handler is NULL and concrete board
should set it to it's own handler.
Considering there isn't any default handler, drop saving empty
MachineClass::get_hotplug_handler in child class and make PC code
consistent with spapr/s390x boards.
We can bring this back when actual usecase surfaces and do it
consistently across boards that use get_hotplug_handler().

Let's allow to query the MemoryHotplugState directly from the machine.
If the pointer is NULL, the machine does not support memory devices. If
the pointer is !NULL, the machine supports memory devices and the
data structure contains information about the applicable physical
guest address space region.
This allows us to generically detect if a certain machine has support
for memory devices, and to generically manage it (find free address
range, plug/unplug a memory region).
We will rename "MemoryHotplugState" to something more meaningful
("DeviceMemory") after we completed factoring out the pc-dimm code into
MemoryDevice code.

On the qmp level, we already have the concept of memory devices:
"query-memory-devices"
Right now, we only support NVDIMM and PCDIMM.
We want to map other devices later into the address space of the guest.
Such device could e.g. be virtio devices. These devices will have a
guest memory range assigned but won't be exposed via e.g. ACPI. We want
to make them look like memory device, but not glued to pc-dimm.
Especially, it will not always be possible to have TYPE_PC_DIMM as a parent
class (e.g. virtio devices). Let's use an interface instead. As a first
part, convert handling of
- qmp_pc_dimm_device_list
- get_plugged_memory_size
to our new model. plug/unplug stuff etc. will follow later.
A memory device will have to provide the following functions:
- get_addr(): Necessary, as the property "addr" can e.g. not be used for
virtio devices (already defined).
- get_plugged_size(): The amount this device offers to the guest as of
now.
- get_region_size(): Because this can later on be bigger than the
plugged size.
- fill_device_info(): Fill MemoryDeviceInfo, e.g. for qmp.

Now that we can safely call QOBJECT() on QObject * as well as its
subtypes, we can have macros qobject_ref() / qobject_unref() that work
everywhere instead of having to use QINCREF() / QDECREF() for QObject
and qobject_incref() / qobject_decref() for its subtypes.
The replacement is mechanical, except I broke a long line, and added a
cast in monitor_qmp_cleanup_req_queue_locked(). Unlike
qobject_decref(), qobject_unref() doesn't accept void *.
Note that the new macros evaluate their argument exactly once, thus no
need to shout them.

Xen unstable (to be in 4.11) has two new dmops, relocate_memory and
pin_memory_cacheattr. Use these to set up the VGA memory, replacing the
previous calls to libxc. This allows the VGA console to work properly
when QEMU is running restricted (-xen-domid-restrict).
Wrapper functions are provided to allow QEMU to work with older versions
of Xen.
Tweak the error handling while making this change:
* Report pin_memory_cacheattr errors.
* Report errors even when DEBUG_HVM is not set. This is useful for
trying to understand why VGA is not working, since otherwise it just
fails silently.
* Fix the return values when an error occurs. The functions now
consistently return -1 and set errno.

xc_interface_open etc. is not going to work if we have dropped
privilege, but xendevicemodel_shutdown will if everything is new
enough.
xendevicemodel_shutdown is only availabe in Xen 4.10 and later, so
provide a stub for earlier versions.

We need to restrict *all* the control fds that qemu opens. Looking in
/proc/PID/fd shows there are many; their allocation seems scattered
throughout Xen support code in qemu.
We must postpone the restrict call until roughly the same time as qemu
changes its uid, chroots (if applicable), and so on.
There doesn't seem to be an appropriate hook already. The RunState
change hook fires at different times depending on exactly what mode
qemu is operating in.
And it appears that no-one but the Xen code wants a hook at this phase
of execution. So, introduce a bare call to a new function
xen_setup_post, just before os_setup_post. Also provide the
appropriate stub for when Xen compilation is disabled.
We do the restriction before rather than after os_setup_post, because
xen_restrict may need to open /dev/null, and os_setup_post might have
called chroot.
Currently this does not work with migration, because when running as
the Xen device model qemu needs to signal to the toolstack that it is
ready. It currently does this using xenstore, and for incoming
migration (but not for ordinary startup) that happens after
os_setup_post.
It is correct that this happens late: we want the incoming migration
stream to be processed by a restricted qemu. The fix for this will be
to do the startup notification a different way, without using
xenstore. (QMP is probably a reasonable choice.)
So for now this restriction feature cannot be used in conjunction with
migration. (Note that this is not a regression in this patch, because
previously the -xen-restrict-domid call was, in fact, simply
ineffective!) We will revisit this in the Xen 4.11 release cycle.

The ISA serial port handling in serial-isa.c imposes a limit
of 4 serial ports. This is because we only know of 4 IO port
and IRQ settings for them, and is unrelated to the generic
MAX_SERIAL_PORTS limit, though they happen to both be set at
4 currently.
Use a new MAX_ISA_SERIAL_PORTS wherever that is the correct
limit to be checking against.

When migrating from a pre-2.9 QEMU, no clock_is_reliable flag is
transferred. We should assume that the source host has an unreliable
KVM_GET_CLOCK, rather than using whatever was determined locally, to
ensure that any drift from the TSC-based value calculated by the guest
is corrected.

The code path where mh_load_end_addr is non-zero in the Multiboot
header checks that mh_load_end_addr >= mh_load_addr and so
mb_load_size is checked. However, mb_load_size is not checked when
calculated from the file size, when mh_load_end_addr is 0.
If the kernel binary size is larger than can fit in the address space
after load_addr, we ended up with a kernel_size that is smaller than
load_size, which means that we read the file into a too small buffer.
Add a check to reject kernel files with such Multiboot headers.

ACPI 6.2A Table 5-129 "SPA Range Structure" requires the proximity
domain of a NVDIMM SPA range must match with corresponding entry in
SRAT table.
The address ranges of vNVDIMM in QEMU are allocated from the
hot-pluggable address space, which is entirely covered by one SRAT
memory affinity structure. However, users can set the vNVDIMM
proximity domain in NFIT SPA range structure by the 'node' property of
'-device nvdimm' to a value different than the one in the above SRAT
memory affinity structure.
In order to solve such proximity domain mismatch, this patch builds
one SRAT memory affinity structure for each DIMM device present at
boot time, including both PC-DIMM and NVDIMM, with the proximity
domain specified in '-device pc-dimm' or '-device nvdimm'.
The remaining hot-pluggable address space is covered by one or multiple
SRAT memory affinity structures with the proximity domain of the last
node as before.

build_append_foo() API doesn't need explicit endianness
conversions which eliminates a source of errors and
it makes build_fadt() look like declarative definition of
FADT table in ACPI spec, which makes it easy to review.
Also it allows easily extending FADT to support other
revisions which will be used by follow up patches
where build_fadt() will be reused for ARM target.

move FADT data initialization out of fadt_setup() into dedicated
init_fadt_data() that will set common for pc/q35 values in
AcpiFadtData structure and acpi_get_pm_info() will complement
it with pc/q35 specific values initialization.
That will allow to get rid of fadt_setup() and generalize
build_fadt() so it could be easily extended for rev5 and
reused by ARM target.
While at it also move facs/dsdt/xdsdt offsets from build_fadt()
arg list into AcpiFadtData, as they belong to the same dataset.

The e1000 NIC is getting old and is not a very good default for a
PCIe machine type. Change it to e1000e, which should be supported
by a good number of guests.
In particular, drivers for 82574 were added first to Linux 2.6.27 (2008)
and Windows 2008 R2. This does mean that Windows 2008 will not work
anymore with Q35 machine types and a default "-net nic -net xxx" network
configuration; it did work before because it does have an AHCI driver.
However, Windows 2008 has been declared out of main stream support
in 2015. It will get out of extended support in 2020. Windows 2008
R2 has the same end of support dates and, since the two are basically
Vista vs. Windows 7, R2 probably is more popular.

Automatic creation of SCSI controllers for "-drive if=scsi" for x86
machines was quite a bad idea (see description of commit f778a82f0c
for details). This is marked as deprecated since QEMU v2.9.0, and as
far as I know, nobody complained that this is still urgently required
anymore. Time to remove this now.

The previous commit improved compile time by including less of the
generated QAPI headers. This is impossible for stuff defined directly
in qapi-schema.json, because that ends up in headers that that pull in
everything.
Move everything but include directives from qapi-schema.json to new
sub-module qapi/misc.json, then include just the "misc" shard where
possible.
It's possible everywhere, except:
* monitor.c needs qmp-command.h to get qmp_init_marshal()
* monitor.c, ui/vnc.c and the generated qapi-event-FOO.c need
qapi-event.h to get enum QAPIEvent
Perhaps we'll get rid of those some other day.
Adding a type to qapi/migration.json now recompiles some 120 instead
of 2300 out of 5100 objects.

In my "build everything" tree, a change to the types in
qapi-schema.json triggers a recompile of about 4800 out of 5100
objects.
The previous commit split up qmp-commands.h, qmp-event.h, qmp-visit.h,
qapi-types.h. Each of these headers still includes all its shards.
Reduce compile time by including just the shards we actually need.
To illustrate the benefits: adding a type to qapi/migration.json now
recompiles some 2300 instead of 4800 objects. The next commit will
improve it further.

qemu-common.h includes qemu/option.h, but most places that include the
former don't actually need the latter. Drop the include, and add it
to the places that actually need it.
While there, drop superfluous includes of both headers, and
separate #include from file comment with a blank line.
This cleanup makes the number of objects depending on qemu/option.h
drop from 4545 (out of 4743) to 284 in my "build everything" tree.

The macro expansions of qdict_put_TYPE() and qlist_append_TYPE() need
qbool.h, qnull.h, qnum.h and qstring.h to compile. We include qnull.h
and qnum.h in the headers, but not qbool.h and qstring.h. Works,
because we include those wherever the macros get used.
Open-coding these helpers is of dubious value. Turn them into
functions and drop the includes from the headers.
This cleanup makes the number of objects depending on qapi/qmp/qnum.h
from 4551 (out of 4743) to 46 in my "build everything" tree. For
qapi/qmp/qnull.h, the number drops from 4552 to 21.

tpm_crb is a device for TPM 2.0 Command Response Buffer (CRB)
Interface as defined in TCG PC Client Platform TPM Profile (PTP)
Specification Family “2.0” Level 00 Revision 01.03 v22.
The PTP allows device implementation to switch between TIS and CRB
model at run time, but given that CRB is a simpler device to
implement, I chose to implement it as a different device.
The device doesn't implement other locality than 0 for now (my laptop
TPM doesn't either, so I assume this isn't so bad)
Tested with some success with Linux upstream and Windows 10, seabios &
modified ovmf. The device is recognized and correctly transmit
command/response with passthrough & emu. However, we are missing PPI
ACPI part atm.

Remove dependency of possible_cpus on 1st CPU instance,
which decouples configuration data from CPU instances that
are created using that data.
Also later it would be used for enabling early cpu to numa node
configuration at runtime qmp_query_hotpluggable_cpus() should
provide a list of available cpu slots at early stage,
before machine_init() is called and the 1st cpu is created,
so that mgmt might be able to call it and use output to set
numa mapping.
Use MachineClass::possible_cpu_arch_ids() callback to set
cpu type info, along with the rest of possible cpu properties,
to let machine define which cpu type* will be used.
* for SPAPR it will be a spapr core type and for ARM/s390x/x86
a respective descendant of CPUClass.
Move parse_numa_opts() in vl.c after cpu_model is parsed into
cpu_type so that possible_cpu_arch_ids() would know which
cpu_type to use during layout initialization.

The only user-creatable sysbus devices in qemu-system-x86_64 are
amd-iommu, intel-iommu, and xen-backend. xen-backend is handled
by xen_set_dynamic_sysbus(), so we only need to add amd-iommu and
intel-iommu.

The existing has_dynamic_sysbus flag makes the machine accept
every user-creatable sysbus device type on the command-line.
Replace it with a list of allowed device types, so machines can
easily accept some sysbus devices while rejecting others.
To keep exactly the same behavior as before, the existing
has_dynamic_sysbus=true assignments are replaced with a
TYPE_SYS_BUS_DEVICE entry on the allowed list. Other patches
will replace the TYPE_SYS_BUS_DEVICE entries with more specific
lists of devices.

Starting qemu with
qemu-system-x86_64 -S -M isapc -device {amd|intel}-iommu
leads to a segfault. The code assume PCI bus is present and
tries to access the bus structure without checking.
Since Intel VT-d and AMDVI should only work with PCI, add a
check for PCI bus and return error if not present.

The current implementation of Intel IOMMU code only supports 39 bits
iova address width. This patch provides a new parameter (x-aw-bits)
for intel-iommu to extend its address width to 48 bits but keeping the
default the same (39 bits). The reason for not changing the default
is to avoid potential compatibility problems with live migration of
intel-iommu enabled QEMU guest. The only valid values for 'x-aw-bits'
parameter are 39 and 48.
After enabling larger address width (48), we should be able to map
larger iova addresses in the guest. For example, a QEMU guest that
is configured with large memory ( >=1TB ). To check whether 48 bits
aw is enabled, we can grep in the guest dmesg output with line:
"DMAR: Host address width 48".
