Change sector-based blk_discard(), blk_co_discard(), and
blk_aio_discard() to instead be byte-based blk_pdiscard(),
blk_co_pdiscard(), and blk_aio_pdiscard(). NBD gets a lot
simpler now that ignoring the unaligned portion of a
byte-based discard request is handled under the hood by
the block layer.

The rerror/werror policies are implemented in the devices, so that's
where they should be configured. In comparison to the old options in
-drive, the qdev properties are only added to those devices that
actually support them.
If the option isn't given (or "auto" is specified), the setting of the
BlockBackend is used for compatibility with the old options. For block
jobs, "auto" is the same as "enospc".

As cache.writeback is a BlockBackend property and as such more related
to the guest device than the BlockDriverState, we already removed it
from the blockdev-add interface. This patch adds the new way to set it,
as a qdev property of the corresponding guest device.

collisions less likely. Offenders found with
scripts/clean-header-guards.pl -vn.

Sector-based limits are awkward to think about; in our on-going
quest to move to byte-based interfaces, convert max_transfer_length
and opt_transfer_length. Rename them (dropping the _length suffix)
so that the compiler will help us catch the change in semantics
across any rebased code, and improve the documentation. Use unsigned
values, so that we don't have to worry about negative values and
so that bit-twiddling is easier; however, we are still constrained
by 2^31 of signed int in most APIs.
When a value comes from an external source (iscsi and raw-posix),
sanitize the results to ensure that opt_transfer is a power of 2.

Making all callers special-case 0 as unlimited is awkward,
and we DO have a hard maximum of BDRV_REQUEST_MAX_SECTORS given
our current block layer API limits.
In the case of scsi, this means that we now always advertise a
limit to the guest, even in cases where the underlying layers
previously use 0 for no inherent limit beyond the block layer.

msi_init() reports errors with error_report(), which is wrong
when it's used in realize().
Fix by converting it to Error.
Fix its callers to handle failure instead of ignoring it.
For those callers who don't handle the failure, it might happen:
when user want msi on, but he doesn't get what he want because of
msi_init fails silently.

Dataplane has used the same virtqueue code as non-dataplane since
commits e24a47c5b7 ("virtio-scsi: do not
use vring in dataplane") and 03de2f5274
("virtio-blk: do not use vring in dataplane"). It is no longer
necessary to stop dataplane in order to sync state since there is no
duplicated virtqueue state.

While doing DMA read into ESP command buffer 's->cmdbuf', it could
write past the 's->cmdbuf' area, if it was transferring more than 16
bytes. Increase the command buffer size to 32, which is maximum when
's->do_cmd' is set, and add a check on 'len' to avoid OOB access.

Avoid duplicated code between esp_do_dma and handle_ti. esp_do_dma
has the same code that handle_ti contains after the call to esp_do_dma;
but the code in handle_ti is never reached because it is in an "else if".
Remove the else and also the pointless return.
esp_do_dma also has a partially dead assignment of the to_device
variable. Sink it to the point where it's actually used.
Finally, assert that the other caller of esp_do_dma (esp_transfer_data)
only transfers data and not a command. This is true because get_cmd
cancels the old request synchronously before its caller handle_satn_stop
sets do_cmd to 1.

The 53C9X Fast SCSI Controller(FSC) comes with an internal 16-byte
FIFO buffer. It is used to handle command and data transfer.
Routine get_cmd() in non-DMA mode, uses 'ti_size' to read scsi
command into a buffer. Add check to validate command length against
buffer size to avoid any overrun.

The 53C9X Fast SCSI Controller(FSC) comes with internal 16-byte
FIFO buffers. One is used to handle commands and other is for
information transfer. Three control variables 'ti_rptr',
'ti_wptr' and 'ti_size' are used to control r/w access to the
information transfer buffer ti_buf[TI_BUFSZ=16]. In that,
'ti_rptr' is used as read index, where read occurs.
'ti_wptr' is a write index, where write would occur.
'ti_size' indicates total bytes to be read from the buffer.
While reading/writing to this buffer, index could exceed its
size. Add check to avoid OOB r/w access.

The rationale is similar to the above mode sense response interception:
this is practically the only channel to communicate restraints from
elsewhere such as host and block driver.
The scsi bus we attach onto can have a larger max xfer len than what is
accepted by the host file system (guarding between the host scsi LUN and
QEMU), in which case the SG_IO we generate would get -EINVAL.

Using pread/pwrite or io_submit has the advantage of eliminating the
bounce buffer, but drops the SCSI status. This keeps the guest from
seeing unit attention codes, as well as statuses such as RESERVATION
CONFLICT. Because we know scsi-block operates on an SBC device we can
still use the DMA helpers with SG_IO; just remember to patch the CDBs
if the transfer is split into multiple segments.
This means that scsi-block will always use the thread-pool unfortunately,
instead of respecting aio=native.

Commonize all the checks for canceled requests and errors. The next patch
will add another case to check for, in order to handle passthrough commands.
There is no semantic change here; the only nontrivial modification is in
scsi_write_do_fua, where cancellation has been checked earlier by both
callers. Thus, the check is replaced with an assertion.

scsi-block will be able to do FUA just by passing the request through
to the LUN (which is also more efficient); there is no need to emulate
it like we do for scsi-disk.

These are replacements for blk_aio_readv and blk_aio_writev that allow
customization of the data path. They reuse the DMA helpers' DMAIOFunc
callback type, so that the same function can be used in either the
QEMUSGList or the bounce-buffered case.
This customization will be needed in the next patch to do zero-copy
SG_IO on scsi-block.

While doing MegaRAID SAS controller command frame lookup, routine
'megasas_lookup_frame' uses 'read_queue_head' value as an index
into 'frames[MEGASAS_MAX_FRAMES=2048]' array. Limit its value
within array bounds to avoid any OOB access.

The LSI SAS1068 Host Bus Adapter emulator in Qemu, periodically
looks for requests and fetches them. A loop doing that in
mptsas_fetch_requests() could run infinitely if 's->state' was
not operational. Move check to avoid such a loop.

Vmware Paravirtual SCSI emulation uses command descriptors to
process SCSI commands. These descriptors come with their ring
buffers. A guest could set the ring buffer size to an arbitrary
value leading to OOB access issue. Add check to avoid it.

Commit 983a1600 changed the semantics of blk_write_zeroes() to
be byte-based rather than sector-based, but did not change the
name, which is an open invitation for other code to misuse the
function. Renaming to pwrite_zeroes() makes it more in line
with other byte-based interfaces, and will help make it easier
to track which remaining write_zeroes interfaces still need
conversion.

FIFO buffer. It is used to handle command and data transfer.
Routine get_cmd() uses DMA to read scsi commands into this buffer.
Add check to validate DMA length against buffer size to avoid any
overrun.
Fixes CVE-2016-4441.

Eliminating the reentrancy is actually a nice thing that we can do
with the API that Michael proposed, so let's make it first class.
This also hides the complex assign/set_handler conventions from
callers of virtio_queue_aio_set_host_notifier_handler, which in
fact was always called with assign=true.

In addition to handling IO in vcpu thread and in io thread, dataplane
introduces yet another mode: handling it by AioContext.
This reuses the same handler as previous modes, which triggers races as
these were not designed to be reentrant. Use a separate handler just
for aio, and disable regular handlers when dataplane is active.

Add two missing checks for s->dataplane_fenced. In one case, QEMU
would skip injecting an IRQ due to a write to an uninitialized
EventNotifier's file descriptor.
In the second case, the dataplane_disabled field was used by mistake;
in fact after fixing this occurrence it is completely unused.

Commit 57cb38b included qapi/error.h into qemu/osdep.h to get the
Error typedef. Since then, we've moved to include qemu/osdep.h
everywhere. Its file comment explains: "To avoid getting into
possible circular include dependencies, this file should not include
any other QEMU headers, with the exceptions of config-host.h,
compiler.h, os-posix.h and os-win32.h, all of which are doing a
similar job to this file and are under similar constraints."
qapi/error.h doesn't do a similar job, and it doesn't adhere to
similar constraints: it includes qapi-types.h. That's in excess of
100KiB of crap most .c files don't actually need.
Add the typedef to qemu/typedefs.h, and include that instead of
qapi/error.h. Include qapi/error.h in .c files that need it and don't
get it now. Include qapi-types.h in qom/object.h for uint16List.
Update scripts/clean-includes accordingly. Update it further to match
reality: replace config.h by config-target.h, add sysemu/os-posix.h,
sysemu/os-win32.h. Update the list of includes in the qemu/osdep.h
comment quoted above similarly.
This reduces the number of objects depending on qapi/error.h from "all
of them" to less than a third. Unfortunately, the number depending on
qapi-types.h shrinks only a little. More work is needed for that one.

This adds the SAS1068 device, a SAS disk controller used in VMware that
is oldish but widely supported and has decent performance. Unlike
megasas, it presents itself as a SAS controller and not as a RAID
controller. The device corresponds to the mptsas kernel driver in
Linux.
A few small things in the device setup are based on Don Slutz's old
patch, but the device emulation was written from scratch based on Don's
SeaBIOS patch and on the FreeBSD and Linux drivers. It is 2400 lines
shorter than Don's patch (and roughly the same size as MegaSAS---also
because it doesn't support the similar SPI controller), implements SCSI
task management functions (with asynchronous cancellation), supports
big-endian hosts, has complete support for migration and follows the
QEMU coding standards much more closely.
To write the driver, I first split Don's patch in two parts, with
the configuration bits in one file and the rest in a separate file.
I first left mptconfig.c in place and rewrote the rest, then deleted
mptconfig.c as well. The configuration pages are still based mostly on
VirtualBox's, though not exactly the same. However, the implementation
is completely different. The contents of the pages themselves should
not be copyrightable.

The return code of virtqueue_pop/vring_pop is unused except to check for
errors or 0. We can thus easily move allocation inside the functions
and just return a pointer to the VirtQueueElement.
The advantage is that we will be able to allocate only the space that
is needed for the actual size of the s/g list instead of the full
VIRTQUEUE_MAX_SIZE items. Currently VirtQueueElement takes about 48K
of memory, and this kind of allocation puts a lot of stress on malloc.
By cutting the size by two or three orders of magnitude, malloc can
use much more efficient algorithms.
The patch is pretty large, but changes to each device are testable
more or less independently. Splitting it would mostly add churn.

The next patch will make virtqueue_pop/vring_pop allocate memory for
the VirtQueueElement. In some cases (blk, scsi, gpu) the device wants
to extend VirtQueueElement with device-specific fields and, until now,
the place of the VirtQueueElement within the containing struct didn't
matter. When allocating the entire block in virtqueue_pop/vring_pop,
however, the containing struct must basically be a "subclass" of
VirtQueueElement, with the VirtQueueElement as the first field. Make
that the case for blk and scsi; gpu is already doing it.

Fam Zheng noticed that the change in commit 36896bf ("scsi: always call
notifier on async cancellation", 2015-12-16) could cause a leak of
the request; scsi_req_cancel_async now calls scsi_req_ref
multiple times for multiple cancellations, but there is only
one call to scsi_req_cancel_complete.
So revert the patch and instead assert that the problematic case (a call
to scsi_req_cancel_async after the aiocb has been completed) cannot
happen.

This was found by code inspection. If the request is cancelled twice,
the notifier is never called on the second cancellation request,
and hence for example a TMF might never finish.
All the calls in scsi_req_cancel_async are idempotent, so the change
is safe.

Following the previous patch which changed pvscsi to be a pci express
device, this patch introduces a boolean property 'x-disable-pcie'.
Its default value is false, exposing pvscsi as a pcie device.
Setting 'x-disable-pcie' to 'on' preserves the old 'pci device' (non
express) behavior. This allows migration to older versions.

Following the previous patches, which introduced various changes in
pvscsi's pci configuration space (device subsystem id and revision, msi
offset), this patch introduces a boolean property
'x-old-pci-configuration' to pvscsi.
Its default value is false, exposing the above changes in the pci config
space.
Setting 'x-old-pci-configuration' to 'on' preserves the old behavior,
which allows migration to older versions.

When creating snapshot with the dataplane enabled, the snapshot file gets
not the actual state of virtqueue, because the current state is stored in
VirtIOBlockDataPlane. Therefore, before saving snapshot need to sync
the dataplane vring state to the virtqueue. The dataplane will resume its
work at the next notify virtqueue.
When snapshot loads with loadvm we get a message:

Replace the generic vhost_call() by specific functions for each
function call to help with type safety and changing arguments.
While doing this, I found that "unsigned long long" and "uint64_t" were
used interchangeably and causing compilation warnings, using uint64_t
instead, as the vhost & protocol specifies.

There are two bugs here. First, the 16-bit id loses the high 8 bits
when shifted left by 24. Second, the address must be combined with
an "or" or we just get zero.

Passed-through SCSI devices can be opened with the readonly=on option.
When this happens, Linux filters away write commands so that the guest
cannot overwrite the contents of the device.
However, the guest does not know that the device is read-only, and
accepts writes. The writes only fail later when the page cache is
flushed.
This patch modifies scsi-generic to modify the MODE SENSE data and
set the read-only bit in the device-specific parameters, so that
the guest OS treats the disk as write protected.

vhost_virtqueue_mask takes an "absolute" virtqueue index, while the
code looks like it's passing an index that is relative to
s->dev.vq_index. In reality, s->dev.vq_index is always zero, so
this patch does not make any difference, but the code is clearer.

vhost-scsi calls vhost_dev_init() at realize() time
but forgets to call it's counterpart vhost_dev_cleanup()
at unrealize() time.
Calling it should fix leaking of memory table and
mem_sections table in vhost device. And also unregister
vhost's memory listerner to prevent access from
memory core to freed memory.

The last portion of an unaligned WRITE SAME command could fail the
assertion in bdrv_aligned_pwritev:
assert(!qiov || bytes == qiov->size);
Because we updated data->iov.iov_len right above this if block, but
data->qiov still has the old size.
Reinitialize the qiov to make them equal and keep block layer happy.

The VirtQueueElement struct is serialized during migration but the
in_sg[]/out_sg[] iovec arrays are not usable on the destination host
because the pointers are meaningless.
Use virtqueue_map_sg() to refresh in_sg[]/out_sg[] to valid pointers
based on in_addr[]/out_addr[] hwaddrs.

This patch fixes a QEMU SEGFAULT when a write operation is performed on
the memory region of the PCI BAR 3 (base address space).
When a writeb(0xe0000000) is performed the .write function is invoked to
handle the write access, however, since the .write is not initialised,
the call to 0, causes QEMU to SEGFAULT.

Currently, scsi_get_configuration always returns a current
profile (DVD or CD), even when there is actually no media present.
By comparison, ide/atapi uses a default profile of 0 (MMC_PROFILE_NONE)
for this case and checks for tray_open, so let's do the same for scsi.
This fixes a problem I'm seeing with Fedora 22 guests where systemd
cdrom_id fails to unmount after a QEMU-initiated eject against a
scsi cdrom device because it believes the media is still present
(but unreadable).

This is a guest-triggerable buffer overflow present in QEMU 2.2.0
and newer. scsi_cdb_length returns -1 as an error value, but the
caller does not check it.
Luckily, the massive overflow means that QEMU will just SIGSEGV,
making the impact much smaller.

We use vhostforce to enable vhost even if Guests don't have MSI-X
support and we fall back to QEMU virtio-net.
This gives a very small performance gain, but the disadvantage
is that guest now controls which virtio code is running
(qemu or vhost) so our attack surface is doubled.
This patch will enable vhost unconditionally whenever it's requested.
For compatibility, enable vhost when vhostforce is set, as well.

Few devices have their specialized 'config_write' methods which simply
call 'pci_default_write_config' followed by a 'msix_write_config' or
'msi_write_config' calls, using exact same arguments.
This is unnecessary as 'pci_default_write_config' already invokes
'msi_write_config' and 'msix_write_config'.
Also, since 'pci_default_write_config' is the default 'config_write'
handler, we can simply avoid the registration of these specialized
versions.

We create optional sections with this patch. But we already have
optional subsections. Instead of having two mechanism that do the
same, we can just generalize it.
For subsections we just change:
- Add a needed function to VMStateDescription
- Remove VMStateSubsection (after removal of the needed function
it is just a VMStateDescription)
- Adjust the whole tree, moving the needed function to the corresponding
VMStateDescription

Make features 64bit wide everywhere.
On migration a full 64bit guest_features field is sent if one of the
high bits is set, in addition to the lower 32bit guest_features field
which must stay for compatibility reasons. That way we send the lower
32 feature bits twice, but the code is simpler because we don't have
to split and compose the 64bit features into two 32bit fields.

So far virtio-scsi-device can't expose host features to guest while
using virtio-mmio because it doesn't set DEFINE_VIRTIO_SCSI_FEATURES on
backend or transport.
The host features belong to the backends while virtio-scsi-pci,
virtio-scsi-s390 and virtio-scsi-ccw set the DEFINE_VIRTIO_SCSI_FEATURES
on transports. But they already have the ability to forward property
accesses to the backend child. So if we move the host features to
backends, it doesn't break the backwards compatibility for them and
make host features work while using virtio-mmio.
Move DEFINE_VIRTIO_SCSI_FEATURES to the backend virtio-scsi. The
transports just sync the host features from backends.

It is wrong to use address_space_memory directly, because there could be an
IOMMU in the middle. Passing the entire PVSCSIRingInfo to RS_GET_FIELD
and RS_SET_FIELD makes it easy to go back to the PVSCSIState.

Assertion `skip == 0' failed.
This is introduced by commit 55783a55 (virtio-scsi: work around bug in
old BIOSes) which didn't check out_num when accessing out_sg[0].iov_len
(the same to in sg). For virtio_scsi_push_event, looking into out_sg
doesn't make sense because 0 req_size is intended.

Commit c53659f0 ("BlockConf: Call backend functions to detect geometry
and blocksizes") causes a segmentation fault on the invalid
configuration of a scsi device without a drive.
Let's check for conf.blk before calling blkconf_blocksizes. The error
will be handled later on in scsi_realize anyway.

geometry: hd_geometry_guess function autodetects the drive geometry.
This patch adds a block backend call, that probes the backing device
geometry. If the inner driver method is implemented and succeeds
(currently only for DASDs), the blkconf_geometry will pass-through
the backing device geometry. Otherwise will fallback to old logic.
blocksize: This patch initializes blocksize properties to 0.
In order to set the property a blkconf_blocksizes was introduced.
If user didn't set physical or logical blocksize, it will
retrieve its value from a driver (only succeeds for DASD), otherwise
it will set default 512 value.
The blkconf_blocksizes call was added to all users of BlkConf.

When setting "realized" fails, scsi_bus_legacy_add_drive() passes the
error to qerror_report_err(), then returns an unspecific "Setting
drive property failed" error, which is reported further up the call
chain.

Three kinds of callers:
1. On failure, report the error and abort
Passing &error_abort does the job. No functional change.
2. On failure, report the error and exit()
This is qdev_prop_set_drive_nofail(). Error reporting moves from
qdev_prop_set_drive() to its caller. Because hiding away the error
in the monitor right before exit() isn't helpful, replace
qerror_report_err() by error_report_err(). Shouldn't make a
difference, because qdev_prop_set_drive_nofail() should never be
used in QMP context.
3. On failure, report the error and recover
This is usb_msd_init() and scsi_bus_legacy_add_drive(). Error
reporting and freeing the error object moves from
qdev_prop_set_drive() to its callers.
Because usb_msd_init() can't run in QMP context, replace
qerror_report_err() by error_report_err() there.
No functional change.
scsi_bus_legacy_add_drive() calling qerror_report_err() is of
course inappropriate, but this commit merely makes it more obvious.
The next one will clean it up.

Commit a818a4b changed scsi_bus_legacy_handle_cmdline() to report
errors from scsi_bus_legacy_add_drive() with error_report() in
addition to returning them. That's inappropriate.
Two kinds of callers:
1. realize methods (devices "esp", "virtio-scsi-device" and
"spapr-vscsi")
The error object gets passed up the call chain until it gets
reported again and freed.
Example:

The second message in this error cascade comes from
scsi_bus_legacy_handle_cmdline(). The error object then gets
passed up to the qdev_init() called from
virtio_scsi_pci_init_pci(), which reports it again.
2. init methods (devices "am53c974", "dc390", "lsi53c895a",
"lsi53c810", "megasas", "megasas-gen2")
init methods need to report their errors with qerror_report().
These don't. The inappropriate error_report() papers over the bug.
error_report() isn't the same as qerror_report() in QMP context,
but this can't actually happen: QMP can still only hot-plug, and
callers call scsi_bus_legacy_handle_cmdline() only on cold-plug.
Except for sysbus_esp_realize(), but that can't be hot-plugged at
all, as far as I can tell.
Fix the init methods and drop the inappropriate error_report() in
scsi_bus_legacy_handle_cmdline().

s->blocker is really only used in hw/scsi/virtio-scsi.c; the only places
where it is used in hw/scsi/virtio-scsi-dataplane.c is when it is
allocated and when it is freed. That does not make a whole lot of sense
(and is actually wrong because this leads to s->blocker potentially
being NULL when blk_op_block_all() is called in virtio-scsi.c), so move
the allocation and destruction of s->blocker to the device realization
and unrealization in virtio-scsi.c, respectively.

It's not safe to call blk_set_aio_context from outside BQL because of
the bdrv_drain_all there. Let's put it in the hotplug callback which
will be called by qdev device realization for each scsi device attached
to the bus.

This mimics what is done in qdev_device_add, and lets the device be
freed in case something goes wrong. Otherwise, object_unparent returns
immediately without freeing the device, which is on the other hand left
in the parent bus's list of children.
scsi_bus_legacy_handle_cmdline then returns an error, and the HBA is
destroyed as well with object_unparent. But the lingering device that
was not removed in scsi_bus_legacy_add_drive cannot be removed now either,
and bus_unparent gets stuck in an infinite loop trying to empty the list
of children.
The right fix of course would be to assert in bus_add_child that the
device already has a bus, and remove the "safety net" that adds the
drive to the QOM tree in device_set_realized. I am not yet sure whether
that would entail changing all callers to qdev_create (as well as
isa_create and usb_create and the corresponding _try_create versions).

monitor_handle_fd_param() is a wrapper around
monitor_handle_fd_param2() that feeds errors to qerror_report_err()
instead of returning them. qerror_report_err() is inappropriate in
many contexts. monitor_handle_fd_param() looks simpler than
monitor_handle_fd_param2(), which tempts use. Remove the temptation:
drop the wrapper and open-code the (trivial) error handling instead.
Replace the open-coded qerror_report_err() by error_report_err() in
places that already use error_report(). Turns out that's everywhere.
While there, rename monitor_handle_fd_param2() to monitor_fd_param().

We get two error messages: one from monitor_handle_fd_param2(), and
another one from vhost_scsi_realize(). The second one gets suppressed
in QMP context.
That's because monitor_handle_fd_param() calls qerror_report_err().
Calling qerror_report_err() is always inappropriate in realize
methods, because it doesn't return the Error object. It either
reports the error to stderr or the human monitor, or it stores it in
the QMP monitor, where it makes the QMP command fail even when the
realize method ignores the error and succeeds. Fortunately,
vhost_scsi_realize() doesn't do that.
Fix by switching to monitor_handle_fd_param2().

The vring.c code currently assumes that guest and host endianness match,
which is not true for a number of cases:
- emulating targets with a different endianness than the host
- bi-endian targets, where the correct endianness depends on the virtio
device
- upcoming support for the virtio-1 standard mandates little-endian
accesses even for big-endian targets and hosts
Make sure to use accessors that depend on the virtio device.
Note that dataplane now needs to be built per-target.

One of the annoyances of the current migration format is the fact that
it's not self-describing. In fact, it's not properly describing at all.
Some code randomly scattered throughout QEMU elaborates roughly how to
read and write a stream of bytes.
We discussed an idea during KVM Forum 2013 to add a JSON description of
the migration protocol itself to the migration stream. This patch
adds a section after the VM_END migration end marker that contains
description data on what the device sections of the stream are composed of.
This approach is backwards compatible with any QEMU version reading the
stream, because QEMU just stops reading after the VM_END marker and ignores
any data following it.
With an additional external program this allows us to decipher the
contents of any migration stream and hopefully make migration bugs easier
to track down.

scsi_req_cancel_complete is responsible for releasing the request, so we
shouldn't skip it in any case. This doesn't affect the only existing
caller, virtio-scsi, but is useful for other devices once they use it.

Commit d577646 (scsi: Introduce scsi_req_cancel_complete, 2014-09-25)
was supposed to have no semantic change, but it missed a case. When
r->aiocb has already been NULLed, but DMA was not complete and the
SCSI layer was waiting for scsi_req_continue, after the patch the
SCSI layer will not call the .cancel callback of SCSIBusInfo.

g_new(T, n) is neater than g_malloc(sizeof(T) * n). It's also safer,
for two reasons. One, it catches multiplication overflowing size_t.
Two, it returns T * rather than void *, which lets the compiler catch
more type errors.
This commit only touches allocations with size arguments of the form
sizeof(T).

After a reset ESP_TCHI should contain the unique ID
of the chip. This value will be overwritten with the
current tranfer count if the transfer count has
previously been set.
So we should always return the chip id if ESP_TCHI
has never been written to.

Old BIOSes left some padding by mistake after the req_size/resp_size.
New QEMU does not like it, thinking it is a bidirectional command.
As a workaround, we can check if the ANY_LAYOUT bit is set; if not, we
always consider the first buffer as the virtio-scsi request/response,
because, back when QEMU did not support ANY_LAYOUT, it expected the
payload to start at the second element of the iovec.
This can show up during migration.

A linux guest will be issuing messages:

MSI-X works slightly different than INTx; the doorbell
registers are not necessarily used as MSI-X interrupts
are directed anyway. So the head pointer on the
reply queue needs to be updated as soon as a frame
is completed, and we can set the doorbell only
when in INTx mode.

Windows requires the frames to be unmapped, otherwise we run
into a race condition where the updated frame data is not
visible to the guest.
With that we can simplify the queue algorithm and use a bitmap
for tracking free frames.

The windows driver is sending several init_firmware commands
when in MSI-X mode. It is, however, using only the first
queue. So disregard any additional init_firmware commands

Logical drives can only be addressed with the 'target_id' number;
LUN numbers cannot be selected.
Physical drives can be selected with both, target and LUN id.
So we should disallow LUN numbers not equal to 0 when in
RAID mode.

scsi_cdb_length() does not return the length of the cdb, but
the transfersize encoded in the cdb. So rename it to scsi_cdb_xfer()
and also rename all other related functions to end with _xfer.
We can then add a new scsi_cdb_length() which actually does return the
length of the cdb. With that DEBUG_SCSI can now display the correct
CDB buffer.

The local variable "type" in virtio_scsi_handle_ctl represents the tmf
command type from the guest and it has the same meaning as the
req->req.tmf.type. However, before the invoking of virtio_scsi_parse_req
the req->req.tmf.type doesn't has the correct value(just initialized to
zero). Therefore, we need to use the "type" variable to judge the case.

The dataplane code is currently doing a hard exit on various setup
failures. In practice, this may mean that a guest suddenly dies after
a dataplane device failed to come up (e.g., when a file descriptor
limit is hit for the nth device).
Let's just try to unwind the setup instead and return.

blockdev_init() always creates a DriveInfo, but only drive_new() fills
it in. qmp_blockdev_add() leaves it blank. This results in a drive
with type = IF_IDE, bus = 0, unit = 0. Screwed up in commit ee13ed1c.
Board initialization code looking for IDE drive (0,0) can pick up one
of these bogus drives. The QMP command has to execute really early to
be visible. Not sure how likely that is in practice.
Fix by creating DriveInfo in drive_new(). Block backends created by
blockdev-add don't get one.
Breaks the test for "has been created by qmp_blockdev_add()" in
blockdev_mark_auto_del() and do_drive_del(), because it changes the
value of dinfo && !dinfo->enable_auto_del from true to false. Simply
test !dinfo instead.
Leaves DriveInfo member enable_auto_del unused. Drop it.
A few places assume a block backend always has a DriveInfo. Fix them
up.

Device models should access their block backends only through the
block-backend.h API. Convert them, and drop direct includes of
inappropriate headers.
Just four uses of BlockDriverState are left:
* The Xen paravirtual block device backend (xen_disk.c) opens images
itself when set up via xenbus, bypassing blockdev.c. I figure it
should go through qmp_blockdev_add() instead.
* Device model "usb-storage" prompts for keys. No other device model
does, and this one probably shouldn't do it, either.
* ide_issue_trim_cb() uses bdrv_aio_discard() instead of
blk_aio_discard() because it fishes its backend out of a BlockAIOCB,
which has only the BlockDriverState.
* PC87312State has an unused BlockDriverState[] member.
The next two commits take care of the latter two.

The replacement is repetitive, but the conversion of device models to
BlockBackend is imminent, and will shorten it to just
blk_legacy_dinfo(dinfo).
Line wrapping muddies the waters a bit. I also omit tests whether
dinfo->bdrv is null, because it never is.

For VIRTIO_SCSI_T_TMF_ABORT_TASK and VIRTIO_SCSI_T_TMF_ABORT_TASK_SET,
use scsi_req_cancel_async to start the cancellation.
Because each tmf command may cancel multiple requests, we need to use a
counter to track the number of remaining requests we still need to wait
for.

Devices will call this function to start an asynchronous cancellation. The
bus->info->cancel will be called after the request is canceled.
Devices will probably need to track a separate TMF request that triggers this
cancellation, and wait until the cancellation is done before completing it. So
we store a notifier list in SCSIRequest and in scsi_req_cancel_complete we
notify them.

Before, scsi_req_cancel will take ownership of the canceled request and unref
it. We did this because we didn't know whether AIO CB will be called or not
during the cancelling, so we set the io_canceled flag before calling it, and
skip unref in the potentially called callbacks, which is not very nice.
Now, bdrv_aio_cancel has a stricter contract that the completion callbacks are
always called, so we can remove the checks of req->io_canceled and just unref
it in callbacks.
It will also make implementing asynchronous cancellation easier.

This enables the virtio-scsi-dataplane code by setting the iothread
in virtio-scsi device, and makes any function that is called by
back from dataplane to cooperate with the caller: they need to be
vring/iothread aware when handling the requests and using scsi devices
on the bus.

This implements the core part of dataplane feature of virtio-scsi.
A few fields are added in VirtIOSCSICommon to maintain the dataplane
status. These fields are managed by a new source file:
virtio-scsi-dataplane.c.
Most code in this file will run on an iothread, unless otherwise
commented as in a global mutex context, such as those functions to
start, stop and setting the iothread property.
Upon start, we set up guest/host event notifiers, in a same way as
virtio-blk does. The handlers then pop request from vring and call into
virtio-scsi.c functions to process it. So we need to make sure make all
those called functions work with iothread, too.

The VirtQueueElement is a very big structure (>48k!), since it will be
initialzed by virtqueue_pop, we can save the expensive zeroing here.

Zeroing sense buffer for each scsi request is not efficient, we can just
leave it uninitialized because sense_len is set to 0.
Move the implicitly zeroed fields to the end of the structure and use a
partial memset.
The explicitly initialized fields (by scsi_req_alloc or scsi_req_new)
are moved to the beginning of the structure, before sense buffer, to
skip the memset.
Also change g_malloc0 to g_slice_alloc.

This is the next step for decoupling block accounting functions from
BlockDriverState.
In a future commit the BlockAcctStats structure will be moved from
BlockDriverState to the device models structures.
Note that bdrv_get_stats was introduced so device models can retrieve the
BlockAcctStats structure of a BlockDriverState without being aware of it's
layout.
This function should go away when BlockAcctStats will be embedded in the device
models structures.

As vhost core can use backend_features during init, clear it earlier to
avoid using uninitialized memory.
This use would be harmless since vhost scsi ignores the result
anyway, but initializing earlier will help prevent valgrind errors,
and make scsi and net behave similarly.

Replace "init/destroy" with "realize/unrealize" in SCSIDeviceClass,
which has errp as a parameter. So all the implementations now use
error_setg instead of error_report for reporting error.
Also in scsi_bus_legacy_handle_cmdline, report the error when
initializing the if=scsi devices, before returning it, because in the
callee, error_report is changed to error_setg. And the callers don't
have the right locations (e.g. "-drive if=scsi").

These callbacks will let devices do their own request parsing, or
defer it to the bus. If the bus does not provide an implementation,
in turn, fall back to the default parsing routine.

Virtio SCSI Events need to be byteswapped before being pushed
when host and guest have a different endianness. Not doing so
breaks hotplug of virtio scsi disks, with the following error
message being printed in the guest console:
virtio_scsi: Unsupport virtio scsi event 1000000
This issue got uncovered while testing disk hotplug with a PowerKVM
ppc64le guest. I have checked that this issue also affects a x86_64
guest run on a ppc64 host.

In order to migrate virtio subsections, they should be streamed after

the common migration code to achieve this. This patch introduces load

In order to let event defines use existing types later, instead of
redefine new ones, some old type defines for spice and vnc are changed,
and BlockErrorAction is moved from block.h to qapi schema. Note that
BlockErrorAction is not merged with BlockdevOnError.
At this point, VncInfo is not made a child of VncBasicInfo, because
VncBasicInfo has mandatory fields where VncInfo makes them optional.

Decouple vhost from the Linux kernel by introducing vhost_ops. The
intention is to provide different backends - a 'kernel' backend based on
the ioctl interface, and an 'user' backend based on a UNIX domain socket
and shared memory interface.

vhost_dev_init will replace devfd and devpath with a single opaque argument.
This is initialised with a file descriptor. When TAP is used (through
vhost_net), open /dev/vhost-net and pass the fd as an opaque parameter in
VhostNetOptions. The same applies to vhost-scsi - open /dev/vhost-scsi and
pass the fd.

vhost userspace needn't to handle vq's notification from guest,
so define dummy handle_output callback for all vqs of vhost-scsi.
In some corner cases(such as when handling vq's reset from VM), virtio-pci
still trys to handle pending virtio-scsi events, then object check failure
inside virtio_scsi_handle_event() for vhost-scsi can be triggered.
The issue can be reproduced by 'rmmod virtio-scsi', 'system sleep' or reboot
inside VM.

This makes scsi_command_name() public.
This makes use of scsi_command_name() in debug output for scsi-disk and
spapr-vscsi host bus adapter. Before this, SCSI used to print hex numbers
instead of human-friendly strings.
This adds GET_EVENT_STATUS_NOTIFICATION and READ_DISC_INFORMATION to
the list of SCSI commands supported by scsi_command_name().

This patch fixes a bug in scsi_block_new_request() that was introduced
by commit 137745c5c6. If the host cache
is used - i.e. if BDRV_O_NOCACHE is _not_ set - the 'break' statement

The ld_raw and st_raw definitions are only needed in code that
must compile for both user-mode and softmmu emulation. Device
models can use the equivalent ld_p/st_p which are simple
pointer accessors.

Using __DATE__ or __TIME__ in binary pkgs changes the checksum of
compiled binaries if they get rebuilt, even if there are no other
source changes. Replace the dynamic strings with some equally

hw/scsi/scsi-bus.c invokes load_request.
virtio_scsi_load_request does:

this probably can make elem invalid, for example,
make in_num or out_num huge, then:

mistake. This patch restores the old size, and add a subsection for the

Normally VIOSRP_OK (0) means success and non-zero value means error
except VIOSRP_OK2 (0x99) which is another success code by weird accident.
This uses 0 as success code always as some guests do not cope with
the 0x99 value well. The existing linux driver checks for both VIOSRP_OK
and VIOSRP_OK2 since 2.6.32.
This returns non-zero code (VIOSRP_ADAPTER_FAIL == 0x10) on errors which
can only happen if DMA write failed.

Intercept REPORT_LUNS commands addressed either to SRP LUN 0 or the well-known
LUN for REPORT_LUNS commands. This is required to implement the SAM and SPC
specifications.
Since SRP implements only a single SCSI target port per connection, the SRP
target is required to report all available LUNs in response to a REPORT_LUNS
command addressed either to LUN 0 or the well-known LUN. Instead, QEMU was
forwarding such requests to the first QEMU SCSI target, with the result that
initiators that relied on this feature would only see LUNs on the first QEMU
SCSI target.
Behavior for REPORT_LUNS commands addressed to any other LUN is not specified
by the standard and so is left unchanged. This preserves behavior under Linux
and SLOF, which enumerate possible LUNs by hand and so address no commands
either to LUN 0 or the well-known REPORT_LUNS LUN.

To make a VM more convincing to my application, it's useful to be able
to add a port WWN and relative target port index to the descriptors
returned for VPD page 83h. Add device properties to allow setting
these, and return them from INQUIRY commands.

The transfer length depends on field BYTCHK, which is encoded in byte
1, bits 1..2. However, the guard for for case BYTCHK=11b doesn't
work, and we get case 01b instead. Fix it.
Note that since emulated scsi-hd fails the command outright, it takes
SCSI passthrough of a device that actually implements VERIFY with
BYTCHK=11b to make the bug bite.

The alignment field is now set to the value that is promised to the
guest, rather than required by the host. The next patches will make
QEMU aware of the host-provided values, so make this clear.
The alignment is also not about memory buffers, but about the sectors on
the disk, change the documentation of the field.
At this point, the field is set by the device emulation, but completely
ignored by the block layer.

SeaBIOS waits for LUN0 to respond to the TEST UNIT READY command
in order to decide whether it should part of the boot sequence.
If LUN0 does not respond to the command, boot is delayed by up
to 5 seconds. This currently happens when there is no LUN0 on
a target. Fix that by adding a trivial implementation of the
command.

There is still a small window that occurs when a cancel I/O affects
an asynchronous I/O operation that hasn't started. In other words,
when the residual data length equals the expected data length.
Today, the routine virtio_scsi_command_complete fails because the
VirtIOSCSIReq pointer (from the hba_private field in SCSIRequest)
was cleared earlier when virtio_scsi_complete_req was called by
the virtio_scsi_request_cancelled routine. As a result, the
virtio_scsi_command_complete routine needs to simply return when
it is processing a SCSIRequest block that was marked canceled.

Linux prefers WRITE SAME to UNMAP if the limits are zero, and WRITE
SAME does not discard anything unless the device can guarantee that
the resulting block is zero.
Setting the maximum unmap block and descriptor counts to non-zero
makes Linux choose UNMAP and fixes thin provisioning on glusterfs.
While the maximum unmap block count can have some effect on performance,
the (suggested) maximum number of descriptors is not particularly
important so I didn't add a customization option. SCSI drivers are
used to online firmware updates so I'm not yet adding versioning support
for SCSI, but we're probably getting close to the point when it's worth
thinking about it.

VERIFY emulation was completely botched (and remained botched through
all the refactorings). The command must be emulated both in check-medium
mode (BYTCHK=00, which we implement by doing nothing) and in check-bytes
mode (which we do not implement yet). Unlike WRITE AND VERIFY (which we
treat simply as WRITE with FUA bit set), VERIFY cannot be handled like
READ. In fact the device is _receiving_ data for VERIFY, not _sending_
it like READ.

Fetch the data to be written from the input buffer. If it is all zeroes,
we can use the write_zeroes call (possibly with the new MAY_UNMAP flag).
Otherwise, do as many write cycles as needed, writing 512k at a time.
Strictly speaking, this is still incorrect because a zero cluster should
only be written if the MAY_UNMAP flag is set. But this is a bug in qcow2
and the other formats, not in the SCSI code.

Since we report ANC_SUP==0 in VPD page B2h, we need to return
an error (ILLEGAL REQUEST/INVALID FIELD IN CDB) for all WRITE SAME
requests with ANCHOR==1.

The qdev_free() function name is misleading since all the function does
is unlink the device from its parent. The device is not necessarily
freed.
The device will be freed when its QObject refcount reaches zero. It is
usual for the parent (bus) to hold the final reference but there are
cases where something else holds a reference so "free" is a misleading
name.
Call object_unparent(obj) directly instead of having a qdev wrapper
function.

pci_set_irq and the other pci irq wrappers use
PCI_INTERRUPT_PIN config register to compute device
INTx pin to assert/deassert.
An irq is allocated using pci_allocate_irq wrapper
only if is needed by non pci devices.
Removed irq related fields from state if not used anymore.

r->buf is hardcoded to 2056 which is (256 + 1) * 8, allowing 256 luns at
most. If more than 256 luns are specified by user, we have buffer
overflow in scsi_target_emulate_report_luns.
To fix, we allocate the buffer dynamically.

Currently, treat it exactly as a 53C895A.
53C895A is a 53C810 with more capabilities, so this should work.
However, this lets us test different code paths on Linux, which
don't use lastest features if it detect a 810, or on some OSes
which only support 810 and not 895A (like very old Windows NT
versions).

53C895A datasheet says that this register is read/write, and that the value
returned on read access is dependant of DMA FIFO state. However, nothing is
said for written value.
53C810A datasheet gives more insight about this register:
"This was a general purpose read/write register in previous SYM53C8XX
family chips. Although it is still a read/write register, Symbios reserves
the right to use these bits for future 53C8XX family enhancements."
This prevents going to the default case, which prints an error message.

Use deposit32() rather than handcoded shifts/masks to update the
scratch registers. This is cleaner and incidentally avoids a clang
sanitizer complaint ("runtime error: left shift of 255 by 24 places
cannot be represented in type 'int'").

scsi_bus_legacy_add_drive() creates either a scsi-disk or a
scsi-generic device. It sets property "serial" to argument serial
unless null. Crashes with scsi-generic, because it doesn't have such
the property.
Only usb_msd_initfn_storage() passes non-null serial. Reproducer:

At the moment the guest kernel issues two types of task management
requests to the hypervisor - task about and lun reset. This adds
handling for these tasks. As spapr-vscsi starts calling scsi_req_cancel(),
free_request callback was implemented.
As virtio-vscsi, spapr-vscsi does not handle CLEAR_ACA either as CDB
control byte does not seem to be used at all so NACA bit is not
set to the guest so the guest has no good reason to call CLEAR_ACA task.

The driver calculates SOLNT bit from UCSOLNT and SCSOLNT bits from
the request. The iu pointer has a type of srp_iu* which points to a union,
so cmd and rsp overlap. As the vscsi_send_rsp function calls
memset(iu, 0, sizeof(rsp)), it clears first 36 bytes of both cmd and rsp
so cmd.sol_not is always zero at the moment of calculating rsp.sol_not.
This fixes the bug.

The patch reimplements handling of indirect requests in order to
simplify upcoming live migration support.
- all pointers (except SCSIRequest*) were replaces with integer
indexes and offsets;
- DMA'ed srp_direct_buf kept untouched (ie. BE format);
- vscsi_fetch_desc() is added, now it is the only place where
descriptors are fetched and byteswapped;
- vscsi_req struct fields converted to migration-friendly types;
- many dprintf()'s fixed.
This also removed an unused field 'lun' from the spapr_vscsi device
which is assigned, but never used. So, remove it.

The name field in a VMStateDescription is part of the migration state
versioning, so changing it will break migration. It's therefore a
bad idea to use a QOM typename macro to initialize it, because in
general we're free to rename QOM types as part of code refactoring
and cleanup. For the handful of devices that were doing this by
mistake, replace the QOM typenames with the corresponding literal
strings.

The DMAContext is a simple pointer to an AddressSpace that is now always
already available. Make everyone hold the address space directly,

This patch adds a new SCSI_DISK_F_NO_REMOVABLE_DEVOPS feature. By this
feature we can set that the scsi-block (scsi pass-through) device will still
be removable from the guest side, but from monitor it cannot be removed.

Issuing the READ CAPACITY(10) command in the guest will cause QEMU
to update its knowledge of the maximum accessible LBA in the disk.
The recorded maximum LBA will be wrong if the disk is bigger than
1TB, because ldl_be_p returns a signed int.
When this is fixed, a latent bug will be unmasked. If the READ
CAPACITY(10) command reported an overflow (0xFFFFFFFF), we must
not overwrite the previously-known maximum accessible LBA, or the guest
will fail to access the disk above the first 2TB.

Tray statuses should be also reset. Some guests may lock the tray and
right after resetting the guest it should be unlocked and closed. This
is done on power-on, reset and resume from suspend/hibernate on bare-metal.
This fix is already committed for IDE CD.
Check the commit a7f3d65b65.
Test results on bare-metal:
- on reset/power-on the CD-ROM tray is closed even before the monitor
is turned on
- on resume from suspend/hibernate the tray is also closed before
the monitor is turned on
From test results it seems that this behavior is OS and probably BIOS
independent.

Curerntly the pseries VIO device code contains quite a few explicit
uses of DO_UPCAST and plain C casts. This is (obviously) type unsafe,
and not the conventional way of doing things in the QOM model. This
patch converts the code to use the QOM convention of per-type macros
to do verified casts with OBJECT_CHECK().

The WWPN specified in configfs is passed to "-device vhost-scsi-pci".
The tgpt field of the SET_ENDPOINT ioctl is obsolete now, so it is not

This patch refactors existing virtio-scsi code into VirtIOSCSICommon
in order to allow virtio_scsi_init_common() to be used by both internal
virtio_scsi_init() and external vhost-scsi-pci code.

A verify command is not an actual read (we do not implement
compare mode) and thus does not have an AIOCB attached. Do
not crash in scsi_dma_complete.
