63c915526d cpu: move exec-all.h inclusion out of cpu.h
exec-all.h contains TCG-specific definitions.  It is not needed outside
TCG-specific files such as translate.c, exec.c or *helper.c.

One generic function had snuck into include/exec/exec-all.h; move it to
include/qom/cpu.h.

b6a0aa0537 x86: Clean up includes
Clean up includes so that osdep.h is included first and headers
which it implies are not included manually.

b216aa6c0f target-i386: Use correct memory attributes for memory accesses
These include page table walks, SVM accesses and SMM state save accesses.

The bulk of the patch is obtained with

   sed -i 's/\(\<[a-z_]*_phys\(_notdirty\)\?\>(cs\)->as,/x86_\1,/'

f08b617018 softmmu: introduce cpu_ldst.h
This will collect all load and store helpers soon.  For now
it is just a replacement for softmmu_exec.h, which this patch
stops including directly, but we also include it where this will
be necessary in order to simplify the next patch.

2ef6175aa7 tcg: Invert the inclusion of helper.h
Rather than include helper.h with N values of GEN_HELPER, include a
secondary file that sets up the macros to include helper.h.  This
minimizes the files that must be rebuilt when changing the macros
for file N.

7848c8d19f target-i386: the x86 CPL is stored in CS.selector - auto update hflags accordingly.
Instead of manually calling cpu_x86_set_cpl() when the CPL changes,
check for CPL changes on calls to cpu_x86_load_seg_cache(R_CS).  Every
location that called cpu_x86_set_cpl() also called
cpu_x86_load_seg_cache(R_CS), so cpu_x86_set_cpl() is no longer
required.

This fixes the SMM handler code as it was not setting/restoring the
CPL level manually.

304520291a target-i386: set eflags prior to calling svm_load_seg_cache() in svm_helper.c
The svm_load_seg_cache() function calls cpu_x86_load_seg_cache() which
inspects env->eflags.  So, make sure all changes to eflags are done
prior to loading the segment cache.

259186a7d2 cpu: Move halted and interrupt_request fields to CPUState
Both fields are used in VMState, thus need to be moved together.
Explicitly zero them on reset since they were located before
breakpoints.

Pass PowerPCCPU to kvmppc_handle_halt().

a8170e5e97 Rename target_phys_addr_t to hwaddr
target_phys_addr_t is unwieldly, violates the C standard (_t suffixes are
reserved) and its purpose doesn't match the name (most target_phys_addr_t
addresses are not target specific).  Replace it with a finger-friendly,
standards conformant hwaddr.

Outstanding patchsets can be fixed up with the command

  git rebase -i --exec 'find -name "*.[ch]"
                        | xargs s/target_phys_addr_t/hwaddr/g' origin
