729b826ecd Fix packet accounting (now netstat -i and netstat -ib show reasonable 
values).

Implement a secondary connection-reestablishement mode, which is only
entered after (1) we have successfully transfered payload data over this
connection and (2) if initial retries did not reestablish a session.
In this mode we retry (infrequently) forever, until adminstrator stops
us (by "ifconfig ppppoe0 down"). XXX - need to display this mode in
pppoectl.

It is now possible to pull the DSL modems plug for say 15 minutes, plug
it back in again and just wait. The connection will be reestablished within
three minutes.

3c28255e30 BUGFIX: it gets bus error on Vr4122 and Vr4131 attempting to access 
pull-up/pull-down registers if you turn on verbose output.
And moved softc structure definition from vrgiuvar.h to vrgiu.c, which was
needless in other parts of kernel.

eefdf030c9 Simplify the pixel-clock-rate-setting code, so it always works out the 
parameters from first principles rather than using a static table for some
rates.  This makes it work correctly on ARM7500, for which the table was
bogus (ARM7500 has a different refclk from VIDC20).

c72d5d40dd Add code to support the bugged Macronix MX98905 chip, as used in several 
podulebus Ethernet cards.  This replaces the NE2000 memory-access routines
with ones that don't try to transfer more than 255 bytes at a time.

This code should perhaps be merged into ne2000.c, but presumably most NE2000
clones won't need it.

505a7027b6 Revert back to invalidating cache during BUS_DMASYNC_POSTREAD instead of 
during BUS_DMASYNC_PREREAD.

Fixes problem where SCSI bus probe didn't read meaningful identity data.

Technically there should be no difference between invalidating before or
after the DMA, but for reasons known only to the R3000 this fixes the
problem.   Some thought needs to be given to whether there is a performace
difference between these two cases.

b697919ea7 Add bus_dmamap_sync for pre-read and pre-write case. This was previously 
left out as it was a no-op on the R3000 processor.  However, recent changes
to the Mips cache ops highlighted we should DTRT in case the MI/MD layer
choses to invalidate the cache ahead of the DMA instead of after it.

e3e7b4bd55 Set np->n_size before calling nfs_vinvalbuf, to avoid recursion 
and confusion about the actual filesize. From Matt Dillon's
similar change in FreeBSD.

XXX n_size is really redundant in -current and must die. This commit
XXX is more of a placeholder for a pullup into the 1.5 branch.

8883bd8ba3 Make the ite interface look a little more like wscons. This is done 
by allowing the hardware drivers to register keyboard and display
support with the ite framework.  Removes the dependence on the hil
keyboard and should make support for dnkbd easier.

An interim fix until wscons arrives.

4923ddfdda in sys_mincore(), check the return value of uvm_vslock() to determine 
if the vec pointer is valid rather than using uvm_useracc().
uvm_useracc() just tells you if the permissions of a user mapping allow
the desired access, not whether faulting on that mapping will succeed.

219d22404b Major restructuring for swizzling to the request queue and unswizzling from 
the response queue. Instead of the ad hoc ISP_SWIZZLE_REQUEST, we now have
a complete set of inline functions in isp_inline.h. Each platform is
responsible for providing just one of a set of ISP_IOX_{GET,PUT}{8,16,32}
macros.

The reason this needs to be done is that we need to have a single set of
functions that will work correctly on multiple architectures for both little
and big endian machines. It also needs to work correctly in the case that
we have the request or response queues in memory that has to be treated
specially (e.g., have ddi_dma_sync called on it for Solaris after we update
it or before we read from it).

One thing that falls out of this is that we no longer build requests in the
request queue itself. Instead, we build the request locally (e.g., on the
stack) and then as part of the swizzling operation, copy it to the request
queue entry we've allocated. I thought long and hard about whether this was
too expensive a change to make as it in a lot of cases requires an extra
copy. On balance, the flexbility is worth it. With any luck, the entry that
we build locally stays in a processor writeback cache (after all, it's only
64 bytes) so that the cost of actually flushing it to the memory area that is
the shared queue with the PCI device is not all that expensive. We may examine
this again and try to get clever in the future to try and avoid copies.

Another change that falls out of this is that MEMORYBARRIER should be taken
a lot more seriously. The macro ISP_ADD_REQUEST does a MEMORYBARRIER on the
entry being added. But there had been many other places this had been missing.
It's now very important that it be done.

For NetBSD, it does a ddi_dmamap_sync as appropriate. This gets us out of
the explicit ddi_dmamap_sync on the whole response queue that we did for SBus
cards at each interrupt.

Set things up so that platforms that cannot have an SBus don't get a lot of
the SBus code checks (dead coded out).

Additional changes:

Fix a longstanding buglet of sorts. When we get an entry via isp_getrqentry,
the iptr value that gets returned is the value we intend to eventually plug
into the ISP registers as the entry *one past* the last one we've written-
*not* the current entry we're updating. All along we've been calling sync
functions on the wrong index value. Argh. The 'fix' here is to rename all
'iptr' variables as 'nxti' to remember that this is the 'next' pointer-
not the current pointer.

Devote a single bit to mboxbsy- and set aside bits for output mbox registers
that we need to pick up- we can have at least one command which does not
have any defined output registers (MBOX_EXECUTE_FIRMWARE).

Explicitly decode GetAllNext SNS Response back *as* a GetAllNext response.
Otherwise, we won't unswizzle it correctly.

Nuke some additional __P macros.

2dfd15933e change the reference-counting of PT pages to start from zero instead of

ee71ea68bc microSPARC-IIep is a sun4m but with an integrated PCI controller. 
In a lot of places (like pmap &c) we want it to be treated as SUN4M.
But since various low-level things are done very differently from
normal sparcs (and since for now it requires a relocated kernel
anyway), the MSIIEP kernels are not supposed to support any other
system.  So when MSIIEP is defined - insist on SUN4M defined and SUN4
and SUN4C not defined.

01b4e2c572 When looking for a console keyboard if usb-kbd-ihandles can't be found 
then try the old name usb-kbd-ihandle.  This way my old iMac gets the
USB keyboard as console instead on the (non-existent) ADB keyboard.

d585188042 We explicitly close LCP when going to state CLOSED, so we better open 
it again when going from INITIAL to STARTING. This has been done for
passive or auto-conecting interfaces always, but not for permanent
ones.

This fixes session reestablishement for PPPoE interfaces without LINK1 set,
and probably also closes PR kern/11161.

14deefd5b1 pckbd_enable is called during autoconfig. Using pckbc_enqueue_cmd at 
that time will have bad results, such as queue corruption because
of multiple TAILQ_INSERTs, etc, because it's not designed to work
in an interrupt/context-free environment. Use pckbc_poll_cmd instead.

b45c51b1fc Roll the rest of the ports over to the new MI kernel build machinery. 
Any problems reported by testers have been fixed, and massive

The tmpx registers are now outputs, this makes them all unique.
Add the fact that cc is changed by the asm (not believed to be used but rather be correct)
Correctly specify w as an input and output register, I think this was hiding the bug below!
Allow sum to be in a different input and output register.

Correct bug in psuedo header handling for in4_cksum.  Seems that the new macros turned up a latent bug in the psuedo header handling, the code was moving a pointer forward 16 bytes twice, not found before as the ADD16 macro wasn't 100% accurate, as it didn't output w, even though it modified it.

b1554100e7 Change the way IPCP negotiation is handled. 
Collect both local and remote address and set them to the interface in
one step (the peer adress was not set at all before).

This causes the peer address now to show up on the interface and all
messages to the routing socket to be send with correct data. The latter
has been the last missing piece to complete PPPoE support.

756e7c6c1f Add the rest of the infrastructure needed to support the rest of the 
ports.  This includes cleaning out DBG, cleaning up the `clean'
target, and tweaking the warnings flags (cesfic, amigappc, and the arm
ports are a little less warning resistant).

Oh, and let's `install' the kernel into ${DESTDIR} if someone says
`make install'.  We have to think about cross-compilers here.

290cc55a28 - Implement 
uint32_t namei_hash(const char *p, const char **ep)
  which determines the equivalent MI hash32_str() hash for p.
  If *ep != NULL, calculate the hash to the character before ep.
  If *ep == NULL, calculate the has to the first / or NUL found, and
  point *ep to that location.
- Use namei_hash() to calculate cn_hash in lookup() and relookup().
  Hash distribution goes from 35-40% to 55-70%, with similar profiled
  time spent in cache_lookup() and cache_enter() on my P3-600.
- Use namei_hash() to calculate cn_hash in nfs_readdirplusrpc(),
  insetad of homegrown code (that differed from that in lookup() !)
  namei_hash() has better spread and is faster than previous code
  (which used a non-constant multiplication).

163b4fbc50 in vinvalbuf(), vtruncbuf() and vflushbuf(), don't skip calling 
VOP_PUTPAGES() just because the vnode has no pages.  layered filesystems
will want to pass these calls on through to the underlying filesystem,
and non-layered filesystems may need to remove the vnode from the
syncer queues.  fix up MP locking and add some locking assertions.

60c77bb2f1 Added support for the Symbios/NCR 53c700 SCSI controller. 
This chip is very similar to the 53c710, except that it's
missing a few registers, doesn't do relative jumps and
doesn't have the table-indirect addressing mode.

I have checked that this still generates the same output
on the existing scripts for the other chips.

03efee5585 * Allow machine-dependent code to specify hooks for ptrace(2) 
(__HAVE_PTRACE_MACHDEP) and procfs (__HAVE_PROCFS_MACHDEP).
  These changes will allow platforms like x86 (XMM) and PowerPC
  (AltiVec) to export extended register sets in a sane manner.

* Use __HAVE_PTRACE_MACHDEP to export x86 XMM registers (standard
  FP + SSE/SSE2) using PT_{GET,SET}XMMREGS (in the machdep
  ptrace request space).
* Use __HAVE_PROCFS_MACHDEP to export x86 XMM registers via
  /proc/N/xmmregs in procfs.

9dfecbefdb Fixed stack setup for argument passing. argc and **argv need to be aligned 
on a 16 bytes boundary. To get things done, we currently duplicate a lot of
code from copyargs(), this should be done in a nicer way.

c152d680d8 Add M_CANFAIL malloc(9) flag. This flag changes behaviour in M_WAITOK 
case when the requested memory size can't ever be granted - instead
of panic, malloc(9) would return failure (NULL).
Note kernel code should do proper bound checking, rather than
depend on M_CANFAIL. This flag is only supposed to be used in very
special cases, where common bound checking is not appropriate.

Discussed on tech-kern@, name ``M_CANFAIL'' suggested by Chuck Cranor.
