MorphOS writes to PCI_CLASS_PROG during IDE initialisation to place the
controller in native mode, but thinks the initialisation has failed
because the native mode bits aren't set when reading the register back.

According to both the VT82C686B and VT8231 datasheets the VIA Southbridge IDE
controller is initialised in legacy mode.
This allows Linux to correctly determine that legacy rather than PCI IRQ routing
should be used since the boot console text in the fulong2e test image changes from:

Some firmwares accidentally write to PCI_INTERRUPT_LINE on startup which has
no effect on real hardware since it is hard-wired to its default value, but
causes the guest OS to become confused trying to initialise IDE devices
when running under QEMU.

Provide a temporary device_legacy_reset function doing what
device_reset does to prepare for the transition with Resettable
API.
All occurrence of device_reset in the code tree are also replaced
by device_legacy_reset.
The new resettable API has different prototype and semantics
(resetting child buses as well as the specified device). Subsequent
commits will make the changeover for each call site individually; once
that is complete device_legacy_reset() will be removed.

The commit a718978ed5 from July 2015 introduced the assertion which
implies that the size of successful DMA transfers handled in ide_dma_cb()
should be multiple of 512 (the size of a sector). But guest systems can
initiate DMA transfers that don't fit this requirement.
For fixing that let's check the number of bytes prepared for the transfer
by the prepare_buf() handler. The code in ide_dma_cb() must behave
according to the Programming Interface for Bus Master IDE Controller
(Revision 1.0 5/16/94):
1. If PRDs specified a smaller size than the IDE transfer
size, then the Interrupt and Active bits in the Controller
status register are not set (Error Condition).
2. If the size of the physical memory regions was equal to
the IDE device transfer size, the Interrupt bit in the
Controller status register is set to 1, Active bit is set to 0.
3. If PRDs specified a larger size than the IDE transfer size,
the Interrupt and Active bits in the Controller status register
are both set to 1.

It's not correct to just ignore an error code in a callback; we need to
handle that error and possible report failure to the guest so that they
don't wait indefinitely for an operation that will now never finish.
This ought to help cases reported by Nutanix where iSCSI returns a
legitimate -ECANCELED for certain operations which should be propagated
normally.

sysemu/sysemu.h is a rather unfocused dumping ground for stuff related
to the system-emulator. Evidence:
* It's included widely: in my "build everything" tree, changing
sysemu/sysemu.h still triggers a recompile of some 1100 out of 6600
objects (not counting tests and objects that don't depend on
qemu/osdep.h, down from 5400 due to the previous two commits).
* It pulls in more than a dozen additional headers.
Split stuff related to run state management into its own header
sysemu/runstate.h.
Touching sysemu/sysemu.h now recompiles some 850 objects. qemu/uuid.h
also drops from 1100 to 850, and qapi/qapi-types-run-state.h from 4400
to 4200. Touching new sysemu/runstate.h recompiles some 500 objects.
Since I'm touching MAINTAINERS to add sysemu/runstate.h anyway, also
add qemu/main-loop.h.

In my "build everything" tree, changing sysemu/sysemu.h triggers a
recompile of some 5400 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).
Almost a third of its inclusions are actually superfluous. Delete
them. Downgrade two more to qapi/qapi-types-run-state.h, and move one
from char/serial.h to char/serial.c.
hw/semihosting/config.c, monitor/monitor.c, qdev-monitor.c, and
stubs/semihost.c define variables declared in sysemu/sysemu.h without
including it. The compiler is cool with that, but include it anyway.
This doesn't reduce actual use much, as it's still included into
widely included headers. The next commit will tackle that.

In my "build everything" tree, changing hw/qdev-properties.h triggers
a recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).
Many places including hw/qdev-properties.h (directly or via hw/qdev.h)
actually need only hw/qdev-core.h. Include hw/qdev-core.h there
instead.
hw/qdev.h is actually pointless: all it does is include hw/qdev-core.h
and hw/qdev-properties.h, which in turn includes hw/qdev-core.h.
Replace the remaining uses of hw/qdev.h by hw/qdev-properties.h.
While there, delete a few superfluous inclusions of hw/qdev-core.h.
Touching hw/qdev-properties.h now recompiles some 1200 objects.

According to hw/ide/internal's file comment, only files in hw/ide/ are
supposed to include it. Drag reality slightly closer to supposition.
Three includes outside hw/ide remain: hw/arm/sbsa-ref.c,
include/hw/ide/pci.h, and include/hw/misc/macio/macio.h. Turns out
board code needs ide-internal.h to wire up IDE stuff. More cleanup is
needed. Left for another day.

definition macros such as DEFINE_PROP_BLOCKDEV_ON_ERROR() and
DEFINE_PROP_OFF_AUTO(). Moving their inclusion from
hw/qdev-properties.h to the users of these macros avoids pointless
recompiles. This is how other property definition macros, such as
DEFINE_PROP_NETDEV(), already work.

This adds a new parameter to blk_new() which requires its callers to
declare from which AioContext this BlockBackend is going to be used (or
the locks of which AioContext need to be taken anyway).
The given context is only stored and kept up to date when changing

Some of our machines (like the ARM cubieboard) use CONFIG_AHCI for an AHCI
sysbus device, but do not use CONFIG_PCI since they do not feature a PCI
bus. With CONFIG_AHCI but without CONFIG_PCI, currently linking fails:

Some trace points are attributed to the wrong source file. Happens
when we neglect to update trace-events for code motion, or add events
in the wrong place, or misspell the file name.
Clean up with help of cleanup-trace-events.pl. Same funnies as in the
previous commit, of course. Manually shorten its change to
linux-user/trace-events to */signal.c.

Instead of including the same list of devices for each target,
set CONFIG_PCI to true, and make the devices default to present
whenever PCI is available. However, s390x does not want all the
PCI devices, so there is a separate symbol to enable them.
Done mostly with the following script:

This device only implemented ISA compatibility mode and native PCI IDE
mode was missing but no clients actually need ISA mode but to the
contrary, they usually want to switch to and use device in native
PCI IDE mode. Therefore implement native PCI mode and switch default
to that.

Parts of the SiI3112 mmio are identical to PCI IDE registers so we can
use the corresponding functions that were factored out into ide/pci.c.
This removes code duplication and simplifies the SiI3112 model which
also helped to spot a copy paste error where reading status of the
2nd channel read the 1st channel instead. This is also fixed here.

Now that no CMD646 specific parts are left in CMD646BAR (all remaining
members are really PCI IDE specific) this struct can be deleted moving
the memory regions for PCI IDE BARs to PCIIDEState where they better
belong. The CMD646 PCI IDE model is adjusted accordingly.

The cmd646 io mem ops callbacks only need the IDEBus which is
currently passed via a CMD646BAR struct. No need to wrap it up like
that, we can pass it directly to these callbacks which then allows to
drop the IDEBus from the CMD646BAR.

them to spaces so that we don't confuse people.
disas, standard-headers, linux-headers and libdecnumber are imported

This reinstates commit b008326744,
which was temporarily reverted for the 3.0 release so that libvirt gets
some extra time to update their command lines.
The -drive option serial was deprecated in QEMU 2.10. It's time to
remove it.
Tests need to be updated to set the serial number with -global instead
of using the -drive option.

The "I" bit in PIO Setup and D2H FISes is exclusively a device concept
and the irqstatus register in the controller does not matter. The SATA
spec says when it should be one; for D2H FISes in practice it is always
set, while the PIO Setup FIS has several subcases that are documented in
the patch.
Also, the PIO Setup FIS interrupt is actually generated _after_ data
has been received.
Someone should probably spend some time reading the SATA specification and
figuring out the more obscure fields in the PIO Setup FIS, but this is enough
to fix SeaBIOS booting from ATAPI CD-ROMs over an AHCI controller.

For the case where the end_transfer_func is also the caller of
ide_transfer_start, the mutual recursion can lead to unlimited
stack usage. Introduce a new version that can be used to change
tail recursion into a loop, and use it in trace_ide_atapi_cmd_reply_end.

The ATAPI_INT_REASON_IO interrupt is raised when I/O starts, but in the
AHCI case ide_set_irq was actually called at the end of a mutual recursion.
Move it early, with the side effect that ide_transfer_start becomes a tail
call in ide_atapi_cmd_reply_end.

There is code checking s->end_transfer_func and it was not taught about
ide_transfer_cancel. We can just use ide_transfer_stop because
s->end_transfer_func is only ever called in the DRQ phase.
ide_transfer_cancel can then be removed, since it would just be
calling ide_transfer_halt.

Now that end_transfer_func is a tail call in ahci_start_transfer,
formalize the fact that the callback (of which ahci_start_transfer is
the sole implementation) takes care of the transfer too: rename it to
pio_transfer and, if it is present, call the end_transfer_func as soon
as it returns.

The PIO Setup FIS is written in the PIO:Entry state, which comes before
the ATA and ATAPI data transfer states. As a result, the PIO Setup FIS
interrupt is now raised before DMA ends for ATAPI commands, and tests have
to be adjusted.
This is also hinted by the description of the command header in the AHCI
specification, where the "A" bit is described as
When ‘1’, indicates that a PIO setup FIS shall be sent by the device
indicating a transfer for the ATAPI command.
and also by the description of the ACMD (ATAPI command region):
The ATAPI command must be either 12 or 16 bytes in length. The length
transmitted by the HBA is determined by the PIO setup FIS that is sent
by the device requesting the ATAPI command.
QEMU, which conflates the "generator" and the "receiver" of the FIS into
one device, always uses ATAPI_PACKET_SIZE, aka 12, for the length.

A trace is added to let us watch unimplemented registers specifically,
as these are more likely to cause us trouble. Otherwise, the port read
traces now tell us what register is getting hit, which is nicer.

AHCI presently signals completion prior to the PxCI register being
cleared to indicate completion. If a guest driver attempts to issue
a new command in its IRQ handler, it might be surprised to learn there
is still a command pending.
In the case of Windows 10's boot driver, it will actually poll the IRQ
register hoping to find out when the command is done running -- which
will never happen, as there isn't a command running.
Fix this: clear PxCI in ahci_cmd_done and not in the asynchronous BH.
Because it now runs synchronously, we don't need to check if the command
is actually done by spying on the ATA registers. We know it's done.

Commit ef0e64a983 "ide: pass IDEState to trim AIO callback" changed the
IDE trim callback from using a BlockBackend to an IDEState but forgot to update
the dma_blk_io() call in hw/ide/macio.c accordingly.
Without this fix qemu-system-ppc segfaults when issuing an IDE trim command on
any of the PPC Mac machines (easily triggered by running the Debian installer).

commit 947858b0 "ide: abort TRIM operation for invalid range"
is incorrect for macio; just ide_dma_error() without doing a callback
is not enough for that errorpath.
Instead, pass -EINVAL to the callback and handle it there
(see related motivation for read/write in 58ac32113).
It will however catch possible EINVAL from the block layer too.

An exit function was mistakenly left here but it's not needed because
the PCI bars are organised differently in this device. Calling this
exit function during device_del was causing an abort with
memory_region_del_subregion: `Assertion subregion->container == mr' failed.

buggy Perl script. Previous iteration was commit a9c94277f0.

to <string.h> and <strings.h>, because we always include these via
osdep.h.
Put the cleaned up system header includes first.

If the Trim bit is set to one and:
a) the device detects an invalid LBA Range Entry; or
b) count is greater than IDENTIFY DEVICE data word 105
(see 7.16.7.55),
then the device shall return command aborted.
A device may trim one or more LBA Range Entries before it returns
command aborted. See table 209.
This check is not in the common ide_dma_cb() as the range for TRIM
is harder to reach: it is not in LBA/count registers and the buffer has
to be parsed first.

This is a common generic PCI SATA controller that is also used in PCs
but more importantly guests running on the Sam460ex board prefer this
card and have a driver for it (unlike for other SATA controllers
already emulated).

The allwinner code is only needed for the allwinner board (for which
we also have a separate CONFIG_ALLWINNER_A10 config switch), so it
does not make sense that we compile this for all the other boards
that need AHCI, too. Let's move it to a separate file that is only
compiled when CONFIG_ALLWINNER_A10 is set.

Commit 4f7265f "ppc/ide/macio: Add missing registers" added two extra macio
registers but forgot to add them to the corresponding VMStateDescription.
The version number is bumped accordingly, although this will have little
effect given that the Mac machines are practically unmigratable.

Not all PCI bridges are Conventional PCI devices, so
INTERFACE_CONVENTIONAL_PCI_DEVICE is added only to the subtypes
that are actually Conventional PCI:
* dec-21154-p2p-bridge
* i82801b11-bridge
* pbm-bridge
* pci-bridge
The direct subtypes of base-pci-bridge not touched by this patch
are:
* xilinx-pcie-root: Already marked as PCIe-only.
* pcie-pci-bridge: Already marked as PCIe-only.
* pcie-port: all non-abstract subtypes of pcie-port are already
marked as PCIe-only devices.
3) megasas-base
Not all megasas devices are Conventional PCI devices, so the
interface names are added to the subclasses registered by
megasas_register_types(), according to information in the
megasas_devices[] array.
"megasas-gen2" already implements INTERFACE_PCIE_DEVICE, so add
INTERFACE_CONVENTIONAL_PCI_DEVICE only to "megasas".

The Linux kernel will query the ATA IDENTITY DEVICE data, word 217
to determine the rotations per minute of the disk. If this has
the value 1, it is taken to be an SSD and so Linux sets the
'rotational' flag to 0 for the I/O queue and will stop using that
disk as a source of random entropy. Other operating systems may
also take into account rotation rate when setting up default
behaviour.
Mgmt apps should be able to set the rotation rate for virtualized
block devices, based on characteristics of the host storage in use,
so that the guest OS gets sensible behaviour out of the box. This
patch thus adds a 'rotation-rate' parameter for 'ide-hd' device
types.

Modify the pre_save method on VMStateDescription to return an int
rather than void so that it potentially can fail.
Changed zillions of devices to make them return 0; the only
case I've made it return non-0 is hw/intc/s390_flic_kvm.c that already
had an error_report/return case.
Note: If you add an error exit in your pre_save you must emit
an error_report to say why.

One of the reasons macio_ide_register_dma() needs to exist is because the
channel id isn't passed into the MACIO_IDE object. Pass in the channel id

The timing register exists on all variants of MacIO IDE, we just
store and return its value.
The interrupts register only exists on KeyLargo but it doesn't
hurt to have it. The lack of this register causes MacOS X to
hangs under some circumstances.
Both are 32-bit only. The HW might support smaller access sizes
but no known OS uses them.
Because the core IDE subsystem doesn't provide us with a way
to query the main (level) interrupt state, nor do we have a way
to know that DBDMA issued a (edge) interrupt, we reflect both
through a private pair of qirq's in order to maintain the
register state.

Create a new enum so that we can name the IRQ bits, which will make debugging
them a little nicer if we can print them out. Not handled in this patch, but
this will make it possible to get a nice debug printf detailing exactly which
status bits are set, as it can be multiple at any given time.
As a consequence of this patch, it is no longer possible to set multiple IRQ
codes at once, but nothing was utilizing this ability anyway.

ich9-ahci device creates ide buses and attaches them as QOM children
at realize time, however it forgets to properly clean them up
at unrealize time and frees memory containing these children,

If no drive=... option is passed (for an empty drive), we don't only
lack the BlockBackend normally created by parse_drive(), but we also
need to manually call blk_attach_dev().
IDE does not support hot unplug, but if it did, qdev would take care to
call the matching blk_detach_dev() on unplug.
This fixes at least the bug that such devices didn't show up in
query-block, and probably some more problems.

In some cases a failing VMSTATE_*_EQUAL does not mean we detected a bug,
but it's actually the best we can do. Especially in these cases a verbose
error message is required.
Let's introduce infrastructure for specifying a error hint to be used if
equal check fails. Let's do this by adding a parameter to the _EQUAL
macros called _err_hint. Also change all current users to pass NULL as
last parameter so nothing changes for them.

As the pci ahci can be hotplug and unplug, in the ahci unrealize

we have an idebus unrealize function, but it was being
registered as the unrealize function for the IDE Device,
so it was not getting invoked on device teardown because
nothing is "unrealizing" the IDE devices themselves.

is resizing images for some devices that can't support it.

Some devices allow a media change between read-only and read-write
media. They need to adapt the permissions in their .change_media_cb()
implementation, which can fail. So add an Error parameter to the
function.

We want every user to be specific about the permissions it needs, so
we'll pass the initial permissions as parameters to blk_new(). A user
only needs to call blk_set_perm() if it wants to change the permissions
after the fact.
The permissions are stored in the BlockBackend and applied whenever a
BlockDriverState should be attached in blk_insert_bs().
This does not include actually choosing the right set of permissions
everywhere yet. Instead, the usual FIXME comment is added to each place
and will be addressed in individual patches.

We've traditionally rejected orphans here and there, but not
systematically. For instance, the sun4m machines have an onboard SCSI
HBA (bus=0), and have always rejected bus>0. Other machines with an
onboard SCSI HBA don't.
Commit a66c9dc made all orphans trigger a warning, and the previous
commit turned this into an error. The checks "here and there" are now
redundant. Drop them.
Note that the one in mips_jazz.c was wrong: it rejected bus > MAX_FD,
but MAX_FD is the number of floppy drives per bus.

The AHCI emulation code supports 64-bit addressing and should advertise this
fact in the Host Capabilities register. Both Linux and Windows drivers test
this bit to decide if the upper 32 bits of various registers may be written
to, and at least some versions of Windows have a bug where DMA is attempted
with an address above 4GB but, in the absence of HOST_CAP_64, the upper 32
bits are left unititialized which leads to a memory corruption.

The Xen HVM unplug protocol [1] specifies a mechanism to allow guests to
request unplug of 'aux' disks (which is stated to mean all IDE disks,
except the primary master). This patch adds support for that unplug request.
NOTE: The semantics of what happens if unplug of all disks and 'aux' disks
is simultaneously requests is not clear. The patch makes that
assumption that an 'all' request overrides an 'aux' request.

The hard-coded default alignment is BDRV_SECTOR_SIZE, however this is not

Fix a memory leak in ide_register_restart_cb() in hw/ide/core.c and add
idebus_unrealize() in hw/ide/qdev.c to have calls to
qemu_del_vm_change_state_handler() to deal with the dangling change

ATA8-APT defines the state transitions for both a host controller and
for the hardware device during the lifecycle of a DMA transfer, in
section 9.7 "DMA command protocol."
One of the interesting tidbits here is that when a device transitions
from DDMA0 ("Prepare state") to DDMA1 ("Data_Transfer State"), it can
choose to set either BSY or DRQ to signal this transition, but not both.
as ide_sector_dma_start is the last point in our preparation process
before we begin the real data transfer process (for either AHCI or BMDMA),
this is the correct transition point for DDMA0 to DDMA1.
I have chosen !BSY && DRQ for QEMU to make the transition from DDMA0 the
most obvious.

We can teach Xen to drain and flush each device as it needs to, instead
of trying to flush ALL devices. This removes the last user of
blk_flush_all.
The function is therefore removed under the premise that any new uses
of blk_flush_all would be the wrong paradigm: either flush the single
device that requires flushing, or use an appropriate flush_all mechanism
from outside of the BlkBackend layer.

The isa_register_portio_list() function allocates ioports
data/state. Let's keep the reference to this data on some owner. This
isn't enough to fix leaks, but at least, ASAN stops complaining of
direct leaks. Further cleanup would require calling
portio_list_del/destroy().

res_count should be set to the number of outstanding bytes after a DBDMA
request. Unfortunately this wasn't being set to zero by the non-block
transfer codepath meaning drivers that checked the descriptor result for
such requests (e.g reading the CDROM TOC) would assume from a non-zero result
that the transfer had failed.

Everywhere else ncq_err is used, it is accompanied by a list cleanup
except for ncq_cb, which is the case you are fixing here.
Move the sglist destruction inside of ncq_err and then delete it from
the other two locations to keep it tidy.
Call dma_buf_commit in ide_dma_cb after the early return. Though, this
is also a little wonky because this routine does more than clear the
list, but it is at the moment the centralized "we're done with the
sglist" function and none of the other side effects that occur in
dma_buf_commit will interfere with the reset that occurs from
ide_restart_bh, I think

If one attempts to perform a system_reset after a failed IO request
that causes the VM to enter a paused state, QEMU will segfault trying
to free up the pending IO requests.
These requests have already been completed and freed, though, so all
we need to do is NULL them before we enter the paused state.
Existing AHCI tests verify that halted requests are still resumed
successfully after a STOP event.

Change sector-based blk_discard(), blk_co_discard(), and
blk_aio_discard() to instead be byte-based blk_pdiscard(),
blk_co_pdiscard(), and blk_aio_pdiscard(). NBD gets a lot
simpler now that ignoring the unaligned portion of a
byte-based discard request is handled under the hood by
the block layer.

The following sequence of tests discovered a problem in IDE emulation:
1. Send DMA write to IDE device 0
2. Send CMD_FLUSH_CACHE to same IDE device which will be failed by block
layer using blkdebug script in tests/ide-test:test_retry_flush
When doing DMA request ide/core.c will set s->retry_unit to s->unit in
ide_start_dma. When dma completes ide_set_inactive sets retry_unit to -1.
After that ide_flush_cache runs and fails thanks to blkdebug.
ide_flush_cb calls ide_handle_rw_error which asserts that s->retry_unit
== s->unit. But s->retry_unit is still -1 after previous DMA completion
and flush does not use anything related to retry.
This patch restricts retry unit assertion only to ops that actually use
retry logic.

The rerror/werror policies are implemented in the devices, so that's
where they should be configured. In comparison to the old options in
-drive, the qdev properties are only added to those devices that
actually support them.
If the option isn't given (or "auto" is specified), the setting of the
BlockBackend is used for compatibility with the old options. For block
jobs, "auto" is the same as "enospc".

As cache.writeback is a BlockBackend property and as such more related
to the guest device than the BlockDriverState, we already removed it
from the blockdev-add interface. This patch adds the new way to set it,

msi_init() reports errors with error_report(), which is wrong
when it's used in realize().
Fix by converting it to Error.
Fix its callers to handle failure instead of ignoring it.
For those callers who don't handle the failure, it might happen:
when user want msi on, but he doesn't get what he want because of
msi_init fails silently.

Callers of dma_blk_io have no way to pass extra data to the DMAIOFunc,
because the original callback and opaque are gone by the time DMAIOFunc
is called. On the other hand, the BlockBackend is usually derived
from those extra data that you could pass to the DMAIOFunc (in the
next patch, that would be the SCSIRequest).
So change DMAIOFunc's prototype, decoupling it from blk_aio_readv
and blk_aio_writev's. The new prototype loses the BlockBackend
and gains an extra opaque value which, in the case of dma_blk_readv
and dma_blk_writev, is of course used for the BlockBackend.

The patch had to touch multiple files at once, because dma_blk_io()
takes pointers to the functions, and ide_issue_trim() piggybacks on
the same interface (while ignoring offset under the hood).

Restart of ATAPI DMA used to be unreachable, because the request to do
so wasn't indicated in bus->error_status due to the lack of spare bits, and
ide_restart_bh() would return early doing nothing.
This patch makes use of the observation that not all bit combinations were
possible in ->error_status. In particular, IDE_RETRY_READ only made sense
together with IDE_RETRY_DMA or IDE_RETRY_PIO. This allows to re-use
IDE_RETRY_READ alone as an indicator of ATAPI DMA restart request.
To makes things more uniform, ATAPI DMA gets its own value for ->dma_cmd.
As a means against confusion, macros are added to test the state of
->error_status.
The patch fixes the restart of both in-flight and pending ATAPI DMA,
following the scheme similar to that of IDE DMA.

ide_atapi_dma_restart() used to just complete the DMA with an error,
under the assumption that there isn't enough information to restart it.
However, as the contents of the ->io_buffer is preserved, it looks safe to
just re-evaluate it and dispatch the ATAPI command again.

If the migration occurs after the IDE DMA has been set up but before it
has been initiated, the state gets lost upon save/restore. Specifically,
->dma_cb callback gets cleared, so, when the guest eventually starts bus
mastering, the DMA never completes, causing the guest to time out the
operation.
OTOH all the infrastructure is already in place to restart the DMA if
the migration happens while the DMA is in progress.
So reuse that infrastructure, by setting bus->error_status based on
->dma_cmd in pre_save if ->dma_cb callback is already set but DMAING is
clear. This will indicate the need for restart and make sure ->dma_cb
is restored in ide_restart_bh(); howeover since DMAING is clear the state
upon restore will be exactly "ready for DMA" as before the save.

After commit e5e7855 (blockdev: Separate BB name management), starting a
guest with PVHVM support result in this assert:
qemu-system-i386: block/block-backend.c:173: blk_delete: Assertion `!blk->name' failed.
A backtrace show that a caller is pci_piix3_xen_ide_unplug().
This patch fix it.

piix3_ide_xen_class_init is identical to piix3_ide_class_init
except it's buggy as it does not set exit and does not disable
hotplug properly.

This patch replaces get_ticks_per_sec() calls with the macro
NANOSECONDS_PER_SECOND. Also, as there are no callers, get_ticks_per_sec()
is then removed. This replacement improves the readability and
understandability of code.
For example,
timer_mod(fdctrl->result_timer,
qemu_clock_get_ns(QEMU_CLOCK_VIRTUAL) + (get_ticks_per_sec() / 50));
NANOSECONDS_PER_SECOND makes it obvious that qemu_clock_get_ns
matches the unit of the expression on the right side of the plus.

Commit 57cb38b included qapi/error.h into qemu/osdep.h to get the
Error typedef. Since then, we've moved to include qemu/osdep.h
everywhere. Its file comment explains: "To avoid getting into
possible circular include dependencies, this file should not include
any other QEMU headers, with the exceptions of config-host.h,
compiler.h, os-posix.h and os-win32.h, all of which are doing a
similar job to this file and are under similar constraints."
qapi/error.h doesn't do a similar job, and it doesn't adhere to
similar constraints: it includes qapi-types.h. That's in excess of
100KiB of crap most .c files don't actually need.
Add the typedef to qemu/typedefs.h, and include that instead of
qapi/error.h. Include qapi/error.h in .c files that need it and don't
get it now. Include qapi-types.h in qom/object.h for uint16List.
Update scripts/clean-includes accordingly. Update it further to match
reality: replace config.h by config-target.h, add sysemu/os-posix.h,
sysemu/os-win32.h. Update the list of includes in the qemu/osdep.h
comment quoted above similarly.
This reduces the number of objects depending on qapi/error.h from "all
of them" to less than a third. Unfortunately, the number depending on
qapi-types.h shrinks only a little. More work is needed for that one.

If the FIS or DMA engines are already started, do not allow them to be
"restarted." As a side-effect of this change, the migration post-load
routine must be modified to cope. If the engines are listed as "on"
in the migrated registers, they must be cleared to allow the startup
routine to see the transition from "off" to "on".
As a second side-effect, the extra argument to ahci_cond_engine_start
is removed in favor of consistent behavior.

Currently, we let ahci_cond_start_engines reject weird configurations
where either the DMA (CLB) or FIS engines are said to be started, but
their matching on/off control bit is toggled off.
There should be no way to achieve this, since any time you toggle the
control bit off, the status bit should always follow synchronously.
Preparing for a refactor in cond_start_engines, move the rejection logic
straight up into post_load.
