Move all of it into accel/tcg/monitor.c. This puts everything
about tcg that is only used by the monitor in the same place.

Unit files including it become "target tainted": they can not
be compiled as target agnostic. Rename using the '-target'
suffix to make this explicit.

On overflow of code_gen_buffer, we unlock the guest pages we had been
translating, but failed to clear gen_tb. On restart, if we cannot
allocate a TB, we exit to the main loop to perform the flush of all
TBs as soon as possible. With garbage in gen_tb, we hit an assert:

In commit f0a08b0913 we changed the type of the PC from
target_ulong to vaddr. In doing so we inadvertently dropped the
zero-padding on the PC in trace lines (the second item inside the []
in these lines). They used to look like this on AArch64, for
instance:
Trace 0: 0x7f2260000100 [00000000/0000000040000000/00000061/ff200000]
and now they look like this:
Trace 0: 0x7f4f50000100 [00000000/40000000/00000061/ff200000]
and if the PC happens to be somewhere low like 0x5000
then the field is shown as /5000/.
This is because TARGET_FMT_lx is a "%08x" or "%016x" specifier,
depending on TARGET_LONG_SIZE, whereas VADDR_PRIx is just PRIx64
with no width specifier.
Restore the zero-padding by adding an 016 width specifier to
this tracing and a couple of others that were similarly recently
changed to use VADDR_PRIx without a width specifier.
We can't unfortunately restore the "32-bit guests are padded to
8 hex digits and 64-bit guests to 16 hex digits" behaviour so
easily.

From this remove, it's no longer clear what this is attempting
to protect. The last time a use of this define was added to
the source tree, as opposed to merely moved around, was 2008.
There have been many cleanups since that time and this is
no longer required for the build to succeed.

Removes the only use of TARGET_LONG_BITS from tcg.h, which is to be
target independent. Move the symbol to a define in tcg-op.h, which
will continue to be target dependent. Rather than complicate matters
for the use in tb_gen_code(), expand the definition there.

Add ability to dump /tmp/perf-<pid>.map and jit-<pid>.dump.
The first one allows the perf tool to map samples to each individual
translation block. The second one adds the ability to resolve symbol
names, line numbers and inspect JITed code.
Example of use:

The only thing that still touches PageDesc in translate-all.c
are some locking routines related to tb-maint.c which have not
yet been moved. Do so now.
Move some code up in tb-maint.c as well, to untangle the maze
of ifdefs, and allow a sensible final ordering.
Move some declarations from exec/translate-all.h to internal.h,
as they are only used within accel/tcg/.

Now that PageDesc is not used for user-only, and for system
it is only used for tb maintenance, move the implementation
into tb-main.c appropriately ifdefed.
We have not yet eliminated all references to PageDesc for
user-only, so retain a typedef to the structure without definition.

Make bsd-user match linux-user in not marking host pages
as reserved. This isn't especially effective anyway, as

Begin weaning user-only away from PageDesc.
Since, for user-only, all TB (and page) manipulation is done with
a single mutex, and there is no virtual/physical discontinuity to
split a TB across discontinuous pages, place all of the TBs into
a single IntervalTree. This makes it trivial to find all of the
TBs intersecting a range.
Retain the existing PageDesc + linked list implementation for
system mode. Move the portion of the implementation that overlaps
the new user-only code behind the common ifdef.

Delay cpu_list_add until realize is complete, so that cross-cpu
interaction does not happen with incomplete cpu state. For this,
we must delay plugin initialization out of tcg_exec_realizefn,
because no cpu_index has been assigned.
Fixes a problem with cross-cpu jump cache flushing, when the
jump cache has not yet been allocated.
