Use decode.c's support for intercepts, doing the check in TCG-generated
code rather than the helper. This is cleaner because it allows removing
the eip_addend argument to helper_pause(), even though it adds a bit of
bloat for opcode 0x90's new decoding function.

ICEBP generates a trap-like exception, while gen_exception() produces
a fault. Resurrect gen_update_eip_next() to implement the desired
semantics.

The TCGCPUOps::cpu_exec_halt method is called from cpu_handle_halt()
when the CPU is halted, so that a target CPU emulation can do
anything target-specific it needs to do. (At the moment we only use
this on i386.)
The current specification of the method doesn't allow the target
specific code to do something different if the CPU is about to come
out of the halt state, because cpu_handle_halt() only determines this
after the method has returned. (If the method called cpu_has_work()
itself this would introduce a potential race if an interrupt arrived
between the target's method implementation checking and
cpu_handle_halt() repeating the check.)
Change the definition of the method so that it returns a bool to
tell cpu_handle_halt() whether to stay in halt or not.
We will want this for the Arm target, where FEAT_WFxT wants to do
some work only for the case where the CPU is in halt but about to
leave it.

Many instructions which load/store 128-bit values are supposed to
raise #GP when the memory operand isn't 16-byte aligned. This includes:
- Instructions explicitly requiring memory alignment (Exceptions Type 1
in the "AVX and SSE Instruction Exception Specification" section of
the SDM)
- Legacy SSE instructions that load/store 128-bit values (Exceptions
Types 2 and 4).
This change sets MO_ALIGN_16 on 128-bit memory accesses that require
16-byte alignment. It adds cpu_record_sigbus and cpu_do_unaligned_access
hooks that simulate a #GP exception in qemu-user and qemu-system,
respectively.

glib-compat.

env->error_code is only 32-bits wide, so the high 32 bits of EXITINFO1
are being lost. However, even though saving guest state and restoring
host state must be delayed to do_vmexit, because they might take tb_lock,
it is always possible to write to the VMCB. So do this for the exit
code and EXITINFO1, just like it is already being done for EXITINFO2.

to do this, we need to take code out of cpu.c and helper.c,
and also move some prototypes from cpu.h, for code that is
needed in tcg/xxx_helper.c, and which in turn is part of the
callbacks registered by the class initialization.
Therefore, do some shuffling of the parts of cpu.h that
are only relevant for tcg/, and put them in tcg/helper-tcg.h
For FT0 and similar macros, put them in tcg/fpu-helper.c
since they are used only there.
