Implement pmap_kenter_pa() and pmap_kremove() properly.

This should fix `kernel diagnostic assertion "rv" failed' panic
and the problem was tracked down by tsubai.
Also add small optimization in pmap_enter() and pmap_remove(), from tsubai.
