oster 77ab4d1598 - update RF_CREATE_THREAD to handle a 'process name' argument.
- fire up a new thread for parity re-writes, copybacks, and reconstructs.
  The ioctl's which trigger these actions now return immediately.
- add progress accounting for the above actions.
- minor rototillage of rf_netbsdkintf.c to deal with all of the above.

mjacob c763ef5b5d - If the user application issues an INITIALIZE ELEMENT STATUS which
succeeds, note that we now are valid.

- Don't attempt to try and run initialize element status from interrupt level-
  we don't really support that yet. Also, key more correctly off of ASC/ASCQ
  instead of just the sense key.

- Make the practice of doing an INITIALIZE ELEMENT STATUS automatically when
  we get params (from chopen time even) a policy decision that is not the
  default for now- this can be a dangerous practice as well as time consuming.
  It's dangerous in that you can have a hung open when all you really want
  to do is do a read of parameters- and parameters, including slot status,
  are perfectly fine to read even before an INITIALIZE ELEMENT STATUS is
  done- all the elements whos status your read are going to be marked with
  an exception- so leave it up to the application to decide how important
  this is.

chopps d8a4f43d0f convert to support non-dma IO (for the isa Riscom/N2).
allow the user to set and use the internal baud rate generator
fix the transmission ring logic to support more than 1 frame per interrupt
add autodetection of the base clock frequency.
cleanup the receive ring logic
support dynamically resizing the low-water mark on the fifo in response
	to buffer underruns on transmit.

sommerfeld b38548c58d On a cold suspend to disk, the Sony VAIO Z505 BIOS appears to not
preserve enough of the state of the PCIC to keep it interrupting on
card insertion/removal..

Add a power hook to notice if the CSC_INTR device register is zeroed
on resume, and if so, reset it to a sane value.

itohy 38e8851905 - Unmuting an external input source (CD playback, line input, and speaker)
now prevents power down/save the device.
  The external input sources are muted by default at initialization.

pk f037f3be7e Shuffle some code around to allow two PHYs to attach. Only one PHY can
be used at a time, but it's now possible to select one by using ifconfig(8)'s
`instance' parameter.

thorpej caae0e1aa1 - Don't try to run the chip in big-endian mode. Not all chips support
this consistently, and it doesn't always work even when the chip
  supports it.
- Make sure things DMA'd to the chip that the chip interprets are in
  little-endian mode.

sommerfeld d4cc45ba08 Fix hot-swapping for controllers which grab two ranges of port space.
(if the mapping of the second range succeeded, it would forget about
the window of the second address range, causing a subsequent insertion
event to fail..).

My Sony PCGA-CD51 now hot-swaps a lot better now.

thorpej f773270d0d If, on the 21143, we find that we don't have an ISV SROM, try reading
it again, with 8 address bits.  21143 Pass 4.1 has a larger SROM,
for storing the CIS when used in a CardBus application.

drochner 162086b1cd do the screen switch asynchronously (via timeout(0)) if possible,
this hopefully helps for the (very rare) display corruption reported

drochner 8996199a85 -allow the "show_screen" call to the display driver to complete
asynchronously, in the same style like the process attach/detach functions
-intercept the "cnpollc" call which originally went directly to the
 keyboard driver and keep track whether the console is in "polling" state
 (DDB!)
-pass a NULL callback to the screen switcher and the process attach/detach
 functions if the console is "polling", to tell them that asynchronous
 completion is forbidden

thorpej 9eea13a674 Mark exactly 3 uses of the SCR_BO() macro as being bogus; these 3 uses
already involve byte swapping on big-endian systems due to bus_space_*().

However, the use is self-consistent, and the value is not interpreted
by the chip, so it probably does not matter.  Leave them in for now; we
can always look at their removal later.

mjacob 53ecdaee6c Some queue related functions now use a u_int16_t. Turn some messages into
CFGPRINTF messages. Clear up the f/w download slightly. If less than
rev 5 2100 chip, turn off loop fairness. Clean up a botch or two.

Clean up some isp_attach time stuff- if ISP2100_FABRIC is defined try *really*
hard to make sure that we get the firmware state to FW_READY and see the
loop state where the Port Database is ready to be gathered- if we don't do
this it's unlikely we will be able to correctly query the nameserver because
we won't see that we're on a fabric.

Clean up the completely broken and stupid attempts to hot switch the
'slow' start routine out of the way. Sigh.

Turn speed announcements into CFGPRINTF functions (available only if DEBUG
defined).

mjacob e363023daf Rearrange defines to make the file diff cleaner with OpenBSD. Change CFGPRINTF
and default verbose levels. Add in a forced compile in f/w define unless

mhitch 901e4b0219 Fix IOASIC DMA alignment problems. Buffers not aligned on doubleword (8
byte) boundaries don't work correctly.  Make use of the SDR0/SDR1 registers
to adjust the buffer alignment when starting the DMA transfer.  Block device
I/O done by the filesystems will usually be aligned correctly, but character
device I/O may not be aligned correctly.  This should fix the problems with
dump(8) failing randomly on DECstations.

-don't use the fg/bg colors in ...alloc_attr() if the WSATTR_WSCOLORS
 flag was not given - use reasonable defaults instead
-add an opaque "ri_hw" member to "rasops_info", for driver use

drochner a7ee57948b -support 3c905B-COMBO - somewhat preliminary: the chip has both an internal
MII PHY capable of NWAY (according to the Linux driver) and plain BNC/
 transceiver connectors. The shared code can't handle this yet, so leave
 out MII/NWAY for now.
-print out revision number from PCI config header, this has been useful to
 identify buggy chips

itojun 1b6504a175 improve cnw driver, looking at bsdi/freebsd-pao cnw driver by wide+prb@bsdi.
- implement ioctls compatible with bsdi/freebsd-pao.  stat type is
  changed from u_int to u_quad_t to avoid overflows.  now it is possible
  to switch domain/encryption key from the userland, using cnwctl(8).
- implement multicast/promiscuous mode.  tested with IPv6.

scw 54f42f17fc Don't bother spinning in the console 'putc' routine for the transmitter
to become idle; it actually doesn't matter as the chip takes care of
this for us. It would also spin endlessly in certain circumstances
during shutdown.

While I'm here, (finally) add code to allow sending BREAK characters.
