fbfc0872d8 New PowerVR display driver, based on the TURBOchannel "cfb"
driver by Tohru Nisimura (a very good framework for a FB driver,
BTW!), using bits of Marcus's original driver for the hardware
access.

The main advantage of this version is that is uses rasops (which
is faster than the rcons stuff) and wsfont, so you can select the
console font in the kernel config file. Using the BOLD8x16 font,
you get an 80x30 console (vast improvement compared to the Gallant12x22
font).

6c60c9ff82 initial commit of USB midi driver.
- I tested only ROLAND UM-1 and YAMAHA UX256.
These are not conforming to USB-MIDI spec, however these are
    similar to USB-MIDI.
  - Since I've not yet seen the "genuine" USB-MIDI device,
I cannot test such devices although I wrote the codes for it.
TODO:
  - clean up the codes.
  - umidi(4) manpage.
  - /dev/rmidi? is working well, but /dev/music seems not working correctly.

  - allow this function to send multiple bytes to hw_if->output()
at the same time. it's necessary for umidi.

5ce651390d Fix problem where tsleep() is used not on the cardbus thread.
This change makes PCMCIA cards work again on my machine.

Unfortunately, some of the DELAY_MS() are called when the current
process is not the cardbus thread, that is, on interrupt contexts.
I confirmed these are actually used in interrupt context and replaced
them with delay()s.  However, I haven't confirmed other DELAY_MS()s
are always on the cardbus thread.

This shall be a temporary fix. The driver needs being rewritten
not to use such delay()s, of course.

6dcc7e3de4 Don't use PR_URGENT to allocate page header. We don't want to just panic

item requested, and just cleanup and return failure if we can't allocate
page header while we aren't allowed to wait.

1779f8f71b Page scanner improvements, behavior is actually a bit more like
Mach VM's now. Specific changes:
- Pages now need not have all of their mappings removed before being
put on the inactive list. They only need to have the "referenced"
attribute cleared. This makes putting pages onto the inactive list
much more efficient. In order to eliminate redundant clearings of
"refrenced", callers of uvm_pagedeactivate() must now do this
  themselves.
- When checking the "modified" attribute for a page (for clearing
PG_CLEAN), make sure to only do it if PG_CLEAN is currently set on
the page (saves a potentially expensive pmap operation).
- When scanning the inactive list, if a page is referenced, reactivate
it (this part was actually added in uvm_pdaemon.c,v 1.27). This
now works properly now that pages on the inactive list are allowed to
  have mappings.
- When scanning the inactive list and considering a page for freeing,
remove all mappings, and then check the "modified" attribute if the
  page is marked PG_CLEAN.
- When scanning the active list, if the page was referenced since its
last sweep by the scanner, don't deactivate it. (This part was
  actually added in uvm_pdaemon.c,v 1.28.)

These changes greatly improve interactive performance during
moderate to high memory and I/O load.

f32fc0fc90 Use evcnts properly for interrupt counting.
The architecture here follows that of the vax port -- each device has its
evcnt in its softc, but defers actually incrementing it to the IRQ
dispatcher. This way, devices can attach sub-counts (e.g. Rx and Tx counts
for Ethernet interfaces), but don't all have to have code to increment the
counters.

Drivers deliberately call evcnt_attach_dynamic() before establishing their
interrupt handler so that the establish routine can attach a parent event if
that's appropriate. At present, it isn't.
