drh b3f0276b9e Fix test cases so that they work with the new EXPLAIN QUERY PLAN output 
format.  Only some of the cases have been fixed.  This is an incremental
check-in.

drh 6a9b9527fa The push-down optimization was being too aggressive such that it sometimes 
generated incorrect results.  Reinstate the restriction (4) (with
qualifications) that was removed by check-ins

drh 2589787c5a If terms of the WHERE clause require that the right table in a LEFT JOIN 
not be a null row, then simplify the LEFT JOIN into an ordinary JOIN.

dan 75dbf68b5b Fix a problem preventing the planner from identifying scans that visit at most 
one row in cases where that property is guaranteed by a unique, not-null,
non-IPK column that is the leftmost in its table.
