Clean up includes so that osdep.h is included first and headers
which it implies are not included manually.

qdict_array_split() should terminate if it encounters both an entry with
a key of "%u" and entries with keys prefixed "%u." for the same index.
This patch adds a test for this case.

This introduces new test reporting infrastructure based on
gtester and gtester-report.
Also, all existing tests are moved to tests/, and tests/Makefile
is reorganized to factor out the commonalities in the rules.
