It seems that the url changed a bit, and it triggers an error. Fix the URLs so
the documentation can be reached again.

A number of headers neglect to include everything they need. They
compile only if the headers they need are already included from
elsewhere. Fix that.

Header guard symbols should match their file name to make guard
collisions less likely.
Cleaned up with scripts/clean-header-guards.pl, followed by some
renaming of new guard symbols picked by the script to better ones.
