The PL011 UART has six interrupt lines:
* RX (receive data)
* TX (transmit data)
* RT (receive timeout)
* MS (modem status)
* E (errors)
* combined (logical OR of all the above)
So far we have only emulated the combined interrupt line;
add support for the others, so that boards that wire them
up to different interrupt controller inputs can do so.

Create a new include file for the pl011's device struct,
type macros, etc, so that it can be instantiated using
the "embedded struct" coding style.

Header guard symbols should match their file name to make guard
collisions less likely. Offenders found with
scripts/clean-header-guards.pl -vn.
Cleaned up with scripts/clean-header-guards.pl, followed by some
renaming of new guard symbols picked by the script to better ones.
