Before this patch, blk_new() automatically assigned a name to the new
BlockBackend and considered it referenced by the monitor. This patch
removes the implicit monitor_add_blk() call from blk_new() (and
consequently the monitor_remove_blk() call from blk_delete(), too) and
thus blk_new() (and related functions) no longer take a BB name
argument.
In fact, there is only a single point where blk_new()/blk_new_open() is
called and the new BB is monitor-owned, and that is in blockdev_init().
Besides thus relieving us from having to invent names for all of the BBs
we use in qemu-img, this fixes a bug where qemu cannot create a new
image if there already is a monitor-owned BB named "image".
If a BB and its BDS tree are created in a single operation, as of this
patch the BDS tree will be created before the BB is given a name
(whereas it was the other way around before). This results in minor
change to the output of iotest 087, whose reference output is amended
accordingly.

Introduce separate functions (monitor_add_blk() and
monitor_remove_blk()) which set or unset a BB name. Since the name is
equivalent to the monitor's reference to a BB, adding a name the same as
declaring the BB to be monitor-owned and removing it revokes this
status, hence the function names.

ones which are referenced by the monitor, and that is not necessarily

When bdrv_close_all() is called, instead of force-closing all root
BlockDriverStates, it is better to just drop the reference from all
BlockBackends and let them be closed automatically. This prevents BDS
from getting closed that are still referenced by other BDS, which may
result in loss of cached data.
This patch adds a function for doing that, but does not yet incorporate
it in bdrv_close_all().

bdrv_close() no longer signifies ejection of a medium, this is now done
by removing the BDS from the BB. Therefore, we want to have a notifier
for that in the BB instead of a close notifier in the BDS. The former is
added now, the latter is removed later.
Symmetrically, another notifier list is added that is invoked whenever a
BDS is inserted. We will need that for virtio-blk and virtio-scsi, which
can then remove their op blockers on BDS ejection and set them up on
insertion.

The amount of memory allocated in nbd_co_receive_request is driven by the
NBD client (possibly a virtual machine). Parallel I/O can cause the
server to allocate a large amount of memory; check for failures and
return ENOMEM in that case.

This patch keeps track of the minimum, maximum and average latencies
of I/O operations during a certain interval of time.
The values are exposed in the BlockDeviceTimedStats structure.
An option to define the intervals to collect these statistics will be
added in a separate patch.

blk_dev_change_media_cb() is called for all potential tray movements;
however, it is possible to request closing the tray but nothing actually
happening (on a floppy disk drive without a medium).
Thus, the actual tray status should be inquired before sending a
tray-moved event (and an event should be sent whenever the status
changed).
Checking @load is now superfluous; it was necessary because it was
possible to change a medium without having explicitly opened the tray
and closed it again (or it might have been possible, at least). This is
no longer possible, though.

In order to open a BDS which inherits a BB's root state,
blk_get_open_flags_from_root_state() is used to inquire the flags to be
passed to bdrv_open(), and blk_apply_root_state() is used to apply the
remaining state after the BDS has been opened.

There are several BlockBackend functions which, in theory, cannot fail.
This patch makes them cope with the BlockDriverState pointer being NULL
by making them fall back to some default action like ignoring the value
in setters and returning the default in getters.

If there is no BlockDriverState in a BlockBackend or if the tray of the
guest device is open, fail all requests (where that is possible) with
-ENOMEDIUM.
The reason the status of the guest device is taken into account is
because once the guest device's tray is opened, any request on the same
BlockBackend as the guest uses should fail. If the BDS tree is supposed
to be usable even after ejecting it from the guest, a different
BlockBackend must be used.

Fix the BlockBackend's AIOCB AioContext for aborting AIO in case there
is no BDS. If there is no implementation of AIOCBInfo::get_aio_context()
the AioContext is derived from the BDS the AIOCB belongs to. If that BDS
is NULL (because it has been removed from the BB) this will not work.
This patch makes blk_get_aio_context() fall back to the main loop
context if the BDS pointer is NULL and implements
AIOCBInfo::get_aio_context() (blk_aiocb_get_aio_context()) which invokes
blk_get_aio_context().

blk_bs() is not NULL and bdrv_is_inserted() returns true) and if the
tray of the guest device is closed.

Some are called do_COMMAND() (old ones, usually), some hmp_COMMAND(),
and sometimes COMMAND pointlessly differs in spelling.
Normalize to hmp_COMMAND(), where COMMAND is exactly the command name
with '-' replaced by '_'.
Exceptions:
* do_device_add() and client_migrate_info() *not* renamed to
hmp_device_add(), hmp_client_migrate_info(), because they're also
QMP handlers. They still need to be converted to QAPI.
* do_memory_dump(), do_physical_memory_dump(), do_ioport_read(),
do_ioport_write() renamed do hmp_* instead of hmp_x(), hmp_xp(),
hmp_i(), hmp_o(), because those names are too cryptic for my taste.
* do_info_help() renamed to hmp_info_help() instead of hmp_info(),
because it only covers help.

blk_new_with_bs() creates a BlockBackend with an empty BlockDriverState
attached to it. Empty BDSs are not nice, therefore add an alternative
function which combines blk_new_with_bs() with bdrv_open().
Note: In contrast to bdrv_open() which takes a BlockDriver parameter,
blk_new_open() does not take such a parameter. This is because
bdrv_open() opens a BlockDriverState, therefore it is natural to be able
to set the BlockDriver for that BDS. The fact that bdrv_open() can open
more than a single BDS is merely some form of a byproduct.
blk_new_open() on the other hand is intended to be used to create a
whole tree of BlockDriverStates. Therefore, setting a single BlockDriver
does not make much sense. Instead, the drivers to be used for each of
the nodes must be configured through the "options" QDict; including the
driver of the root BDS.

Adding something like a "delete notifier" to a BlockBackend would not
make much sense, because whoever is interested in registering there will
probably hold a reference to that BlockBackend; therefore, the notifier
will never be called (or only when the notifiee already relinquished its
reference and thus most probably is no longer interested in that
notification).
Therefore, this patch just passes through the close notifier interface
of the root BDS. This will be called when the device is ejected, for
instance, and therefore does make sense.

No change, because both bdrv_dev_change_media_cb() and
