include/hw/pci/pci_bus.h contains several data structures related to PCI
bridges that aren't needed by most users of pci_bus.h. We already have
a pci_bridge.h, so move them there.

Add a new slot_reserved_mask bitmask to PCIBus indicating whether or not each
PCI slot on the bus is reserved. Ensure that it is initialised to zero to
maintain the existing behaviour that all slots are available by default, and
add the additional check with appropriate error reporting to
do_pci_register_device().

Skip bus_master_enable region creation on PCI device init
in order to be sure the IOMMU device (if present) would
be created in advance. Add this memory region at machine_done time.

make qdev_unplug()/device_set_realized() to call hotplug handler's
plug/unplug methods if available and remove not needed anymore
hot(un)plug handling from PCIDevice.
In case if hotplug handler is not available, revert to the legacy
hotplug method for compatibility with not yet converted buses.

This reverts commit a53ae8e934.
The patch being reverted introduced a low-priority memory region
covering all 64 bit pci address space. This exposed the following bugs
elsewhere in the code:
1. Some memory regions have INT64_MAX size, where the
intent was all 64 bit address space.
This results in a sub-page region, should be UINT64_MAX.
2. page table rendering in exec.c ignores physical address bits
above TARGET_PHYS_ADDR_SPACE_BITS.
Access outside this range (e.g. from device DMA, or gdb stub)
ends up with a wrong region. Registering a region outside this
range leads to page table corruption.
3. Some regions overlap PCI hole and have same priority.
This only works as long as no device uses the overlapping address.
It doesn't look like we can resolve all issues in time for 1.7.
Let's fix the bugs first and apply afterwards for 1.8.

A MemoryRegion with negative priority was created and
it spans over all the pci address space.
It "intercepts" the accesses to unassigned pci
address space and will follow the pci spec:
1. returns -1 on read
2. does nothing on write
Note: setting the RECEIVED MASTER ABORT bit in the STATUS register
of the device that initiated the transaction will be
implemented in another series

Use the new iommu support in the memory core for iommu support. The only
user, spapr, is also converted, but it still provides a DMAContext
interface until the non-PCI bits switch to AddressSpace.

Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches.
