This adds GRAPH_RDLOCK annotations to declare that callers of
bdrv_(un)freeze_backing_chain() need to hold a reader lock for the
graph because it calls bdrv_filter_or_cow_child(), which accesses
bs->file/backing.
Use the opportunity to make bdrv_is_backing_chain_frozen() static, it
has no external callers.

Leading underscores are ill-advised because such identifiers are
reserved. Trailing underscores are merely ugly. Strip both.
Our header guards commonly end in _H. Normalize the exceptions.
Macros should be ALL_CAPS. Normalize the exception.
Done with scripts/clean-header-guards.pl.
include/hw/xen/interface/ and tools/virtiofsd/ left alone, because
these were imported from Xen and libfuse respectively.

Provide API for the COR-filter removal. Also, drop the filter child
permissions for an inactive state when the filter node is being
removed.
To insert the filter, the block generic layer function
bdrv_insert_node() can be used.
The new function bdrv_cor_filter_drop() may be considered as an
intermediate solution before the QEMU permission update system has
overhauled. Then we are able to implement the API function
bdrv_remove_node() on the block generic layer.
