Define a new qdev input gpio for handling incoming PS2 IRQs, and then wire up the
PS2 keyboard and mouse devices to use it. At the same time set update_irq() and
update_arg to NULL in ps2_kbd_init() and ps2_mouse_init() to ensure that any
accidental attempt to use the legacy update_irq() function will cause a NULL
pointer dereference.

Currently i8042_mm_init() creates a new KBDState directly which is used by the MIPS
magnum machine. Introduce a new I8042_MMIO QOM type that will soon be used to
allow the MIPS magnum machine to be wired up using standard qdev GPIOs.

g_new(T, n) is neater than g_malloc(sizeof(T) * n). It's also safer,
for two reasons. One, it catches multiplication overflowing size_t.
Two, it returns T * rather than void *, which lets the compiler catch
more type errors.
This commit only touches allocations with size arguments of the form
sizeof(T).

The variable outport_present is a flag to show if the outport
subsection was loaded. Clear the outport_present flag in the
outer pre_load() function. This method is recommended in the QEMU
manual for developers in the chapter "Migration, Subsections".

In 2005 the author of commit daa579632d "PS2 mouse and keyboard
separation (Paul Brook)" and 0e43e99c04 "PS2 mouse and keyboard
separation (Paul Brook)" separated the PS/2 controller code and
the PS/2 keyboard and mouse code. It seems he forgot to remove
a few defines. Remove them now.

Currently the PS/2 controller command KBD_CCMD_MOUSE_DISABLE
doesn't disable the PS/2 mouse communication at all, and the
PS/2 controller commands KBD_CCMD_KBD_DISABLE and
KBD_CCMD_KBD_ENABLE only disable and enable the keyboard
interrupt, which is very different from what a real PS/2
controller does. A guest may notice the difference.
Mask out pending data on disabled queues to correctly disable
the PS/2 controller communication.

Add a separate queue for PS/2 controller responses. The
responses no longer get queued in the keyboard or mouse queues.
The advantage of this can be seen after the next two patches,
where the guest can disable the PS/2 communication with keyboard
and mouse and still talk to the PS/2 controller.

Currently there is only one flag to distinguish between two
interrupt sources and there are no available flags for more
sources. Add an internal state variable to store the interrupt
source. The next patch will introduce an additional interrupt
source. There is no functional change.

Limit the keyboard data rate to the serial link speed. Some old
DOS software relies on being able to read an incoming scan-code
more than once. After reading keyboard data from the i8042
controller, the guest software has 1ms to read the same data
again.
Use -global i8042.kbd-throttle=on to enable this feature.
To see how this patch works, start a FreeDOS 1.2 guest with the
qemu option -global i8042.kbd-throttle=on and open a text file
with the vim 7.3 32 bit for DOS executable. Then use the cursor
keys (not the cursor keys on the numeric keypad) to move through
the text. Without the kbd-throttle option enabled each keystroke
will move the cursor two positions.

sysemu/sysemu.h is a rather unfocused dumping ground for stuff related
to the system-emulator. Evidence:
* It's included widely: in my "build everything" tree, changing
sysemu/sysemu.h still triggers a recompile of some 1100 out of 6600
objects (not counting tests and objects that don't depend on
qemu/osdep.h, down from 5400 due to the previous two commits).
* It pulls in more than a dozen additional headers.
Split stuff related to run state management into its own header
sysemu/runstate.h.
Touching sysemu/sysemu.h now recompiles some 850 objects. qemu/uuid.h
also drops from 1100 to 850, and qapi/qapi-types-run-state.h from 4400
to 4200. Touching new sysemu/runstate.h recompiles some 500 objects.
Since I'm touching MAINTAINERS to add sysemu/runstate.h anyway, also
add qemu/main-loop.h.

The previous commits have left only the declaration of hw_error() in

now recompiles less than 200 objects.

Convert the pckbd device away from using the old_mmio field
of MemoryRegionOps. This change only affects the memory-mapped
variant of the i8042, which is used by the Unicore32 'puv3'
board and the MIPS Jazz boards 'magnum' and 'pica61'.

Time to wire up all the call sites that request a shutdown or
reset to use the enum added in the previous patch.
It would have been less churn to keep the common case with no
arguments as meaning guest-triggered, and only modified the
host-triggered code paths, via a wrapper function, but then we'd
still have to audit that I didn't miss any host-triggered spots;
changing the signature forces us to double-check that I correctly
categorized all callers.
Since command line options can change whether a guest reset request
causes an actual reset vs. a shutdown, it's easy to also add the
information to reset requests.

The i8042 device has outgouing IRQ line A20. Currently the IRQ is referenced
by a pointer which normally is set during machine initialization. The pointer
is never changed at runtime. So common GPIO model can be applied to A20 IRQ
line. Note that checking for IRQ to be connected as in previous version
of code is not required because qemu_set_irq will do it.

We create optional sections with this patch. But we already have
optional subsections. Instead of having two mechanism that do the
same, we can just generalize it.
For subsections we just change:
- Add a needed function to VMStateDescription
- Remove VMStateSubsection (after removal of the needed function
it is just a VMStateDescription)
- Adjust the whole tree, moving the needed function to the corresponding
VMStateDescription

OSes typically write 0xdd/0xdf to turn the A20 line off and on. This
has bits 2-3-6-7 on, so that the output port subsection is migrated.
Change the reset value and migration default to include those four
bits, thus avoiding that the subsection is migrated.
This strictly speaking changes guest ABI, but the long time during which
we have not migrated the value means that the guests really do not care
much; so the change is for all machine types.
