9f2130f58d xenfb: remove xen_init_display "temporary" hack
Initialize xenfb properly, as all other backends, from its own
"initialise" function.

Remove the dependency of vkbd on vfb: use qemu_console_lookup_by_index
to find the principal console (to get the size of the screen) instead of
relying on a vfb backend to be available (which adds a dependency
between the two).

0e39bb022b xen: use a common function for pv and hvm guest backend register calls
Instead of calling xen_be_register() for each supported backend type
for hvm and pv guests in their machine init functions use a common
function in order not to have to add new backends twice.

This at once fixes the error that hvm domains couldn't use the qusb
backend.

25f8f6b4c2 xen: move xen_sysdev to xen_backend.c
Commit 9432e53a5b added xen_sysdev as a
system device to serve as an anchor for removable virtual buses. This
introduced a build failure for non-x86 builds with CONFIG_XEN_BACKEND
set, as xen_sysdev was defined in a x86 specific file while being
consumed in an architecture independent source.

Move the xen_sysdev definition and initialization to xen_backend.c to
avoid the build failure.

816ac92ef7 xen: add pvUSB backend
Add a backend for para-virtualized USB devices for xen domains.

The backend is using host-libusb to forward USB requests from a
domain via libusb to the real device(s) passed through.

9432e53a5b xen: introduce dummy system device
Introduce a new dummy system device serving as parent for virtual
buses. This will enable new pv backends to introduce virtual buses
which are removable again opposed to system buses which are meant
to stay once added.

64a7ad6fe3 xen: make it possible to build without the Xen PV domain builder
Until the previous patch this relied on xc_fd(), which was only
implemented for Xen 4.0 and earlier.

Given this wasn't working since Xen 4.0 I have marked this as disabled
by default.

Removing this support drops the use of a bunch of symbols from
libxenctrl, specifically:

  - xc_domain_create
  - xc_domain_destroy
  - xc_domain_getinfo
  - xc_domain_max_vcpus
  - xc_domain_setmaxmem
  - xc_domain_unpause
  - xc_evtchn_alloc_unbound
  - xc_linux_build

This is another step towards only using Xen libraries which provide a
stable inteface.

4be746345f hw: Convert from BlockDriverState to BlockBackend, mostly
Device models should access their block backends only through the
block-backend.h API. Convert them, and drop direct includes of
inappropriate headers.

Just four uses of BlockDriverState are left:

* The Xen paravirtual block device backend (xen_disk.c) opens images
itself when set up via xenbus, bypassing blockdev.c. I figure it
  should go through qmp_blockdev_add() instead.

* Device model "usb-storage" prompts for keys. No other device model
does, and this one probably shouldn't do it, either.

* ide_issue_trim_cb() uses bdrv_aio_discard() instead of
blk_aio_discard() because it fishes its backend out of a BlockAIOCB,
  which has only the BlockDriverState.

* PC87312State has an unused BlockDriverState[] member.

The next two commits take care of the latter two.
