The rtc-reset-reinjection QMP command is specific to x86, other boards do not
have the ACK tracking functionality that is needed for RTC interrupt
reinjection. Therefore the QMP command is only included in x86, but
qmp_rtc_reset_reinjection() is implemented by hw/rtc/mc146818rtc.c
and requires tracking of all created RTC devices. Move the implementation
to hw/i386, so that 1) it is available even if no RTC device exist
2) the only RTC that exists is easily found in x86ms->rtc.

vAPIC isn't KVM specific, so having its name prefixed 'kvm'

This commit adds XTSup configuration to let user choose to whether enable
this feature or not. When XTSup is enabled, additional bytes in IRTE with
enabled guest virtual VAPIC are used to support 32-bit destination id.
Additionally, this commit exports IVHD type 0x11 besides the old IVHD type
0x10 in ACPI table. IVHD type 0x10 does not report full set of IOMMU
features only the legacy ones, so operating system (e.g. Linux) may only
detects x2APIC support if IVHD type 0x11 is available. The IVHD type 0x10
is kept so that old operating system that only parses type 0x10 can detect
the IOMMU device.
Besides, an amd_iommu-stub.c file is created to provide the definition for
amdvi_extended_feature_register when CONFIG_AMD_IOMMU=n. This function is
used by acpi-build.c to get the extended feature register value for
building the ACPI table. When CONFIG_AMD_IOMMU=y, this function is defined
in amd_iommu.c.

xen_ss is added unconditionally to arm_ss and i386_ss (the only
targets that can have CONFIG_XEN enabled) and its contents are gated by
CONFIG_XEN; xen_specific_ss has no condition for its constituent files
but is gated on CONFIG_XEN when its added to specific_ss.
So xen_ss is a duplicate of xen_specific_ss, though defined in a
different way. Merge the two by eliminating xen_ss.

Expose SGX to the guest if and only if KVM is enabled and supports
virtualization of SGX. While the majority of ENCLS can be emulated to
some degree, because SGX uses a hardware-based root of trust, the
attestation aspects of SGX cannot be emulated in software, i.e.
ultimately emulation will fail as software cannot generate a valid
quote/report. The complexity of partially emulating SGX in Qemu far
outweighs the value added, e.g. an SGX specific simulator for userspace
applications can emulate SGX for development and testing purposes.
Note, access to the PROVISIONKEY is not yet advertised to the guest as
KVM blocks access to the PROVISIONKEY by default and requires userspace
to provide additional credentials (via ioctl()) to expose PROVISIONKEY.

SGX EPC is enumerated through CPUID, i.e. EPC "devices" need to be
realized prior to realizing the vCPUs themselves, which occurs long
before generic devices are parsed and realized. Because of this,
do not allow 'sgx-epc' devices to be instantiated after vCPUS have
been created.
The 'sgx-epc' device is essentially a placholder at this time, it will
be fully implemented in a future patch along with a dedicated command
to create 'sgx-epc' devices.
