Currently IOAPIC RO bits can be written. To be better aligned with
hardware, we should let them read-only.

Convert 'init' function to QOM's 'realize' for ioapic and kvm-ioapic.
Change variable 'ioapic_no' from static to global. Then we can drop
the 'instance_no' function argument.

Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches.
