util/vfio-helpers: Remove unreachable code in qemu_vfio_dma_map()

qemu_vfio_add_mapping() returns a pointer to an indexed entry
in pre-allocated QEMUVFIOState::mappings[], thus can not be NULL.
Remove the pointless check.
