5bbdfa8a18 Fix waits of REINDEX CONCURRENTLY for indexes with predicates or expressions
As introduced by f9900df5f94, a REINDEX CONCURRENTLY job done for an
index with predicates or expressions would set PROC_IN_SAFE_IC in its
MyProc->statusFlags, causing it to be ignored by other concurrent
operations.

Such concurrent index rebuilds should never be ignored, as a predicate
or an expression could call a user-defined function that accesses a
different table than the table where the index is rebuilt.

A test that uses injection points is added, backpatched down to 17.
Michail has proposed a different test, but I have added something
simpler with more coverage.

0cecc908e9 Lock before setting relhassubclass on RELKIND_PARTITIONED_INDEX.
Commit 5b562644fec696977df4a82790064e8287927891 added a comment that
SetRelationHasSubclass() callers must hold this lock.  When commit
17f206fbc824d2b4b14480199ca9ff7dea417eda extended use of this column to
partitioned indexes, it didn't take the lock.  As the latter commit
message mentioned, we currently never reset a partitioned index to
relhassubclass=f.  That largely avoids harm from the lock omission.  The
cause for fixing this now is to unblock introducing a rule about locks
required to heap_update() a pg_class row.  This might cause more
deadlocks.  It gives minor user-visible benefits:

- If an ALTER INDEX SET TABLESPACE runs concurrently with ALTER TABLE
  ATTACH PARTITION or CREATE PARTITION OF, one transaction blocks
  instead of failing with "tuple concurrently updated".  (Many cases of
  DDL concurrency still fail that way.)

- Match ALTER INDEX ATTACH PARTITION in choosing to lock the index.

While not user-visible today, we'll need this if we ever make something
set the flag to false for a partitioned index, like ANALYZE does today
for tables.  Back-patch to v12 (all supported versions), the plan for
the commit relying on the new rule.  In back branches, add
LockOrStrongerHeldByMe() instead of adding a LockHeldByMe() parameter.

34768ee361 Add temporal FOREIGN KEY contraints
Add PERIOD clause to foreign key constraint definitions.  This is
supported for range and multirange types.  Temporal foreign keys check
for range containment instead of equality.

This feature matches the behavior of the SQL standard temporal foreign
keys, but it works on PostgreSQL's native ranges instead of SQL's
"periods", which don't exist in PostgreSQL (yet).

Reference actions ON {UPDATE,DELETE} {CASCADE,SET NULL,SET DEFAULT}
are not supported yet.

6a004f1be8 Add attstattarget to FormExtraData_pg_attribute
This allows setting attstattarget when a relation is created.

We make use of this by having index_concurrently_create_copy() copy
over the attstattarget values when the new index is created, instead
of having index_concurrently_swap() fix it up later.

ecb0fd3372 Reintroduce MAINTAIN privilege and pg_maintain predefined role.
Roles with MAINTAIN on a relation may run VACUUM, ANALYZE, REINDEX,
REFRESH MATERIALIZE VIEW, CLUSTER, and LOCK TABLE on the relation.
Roles with privileges of pg_maintain may run those same commands on
all relations.

This was previously committed for v16, but it was reverted in
commit 151c22deee due to concerns about search_path tricks that
could be used to escalate privileges to the table owner.  Commits
2af07e2f74, 59825d1639, and c7ea3f4229 resolved these concerns by
restricting search_path when running maintenance commands.

Bumps catversion.

59825d1639 Fix buildfarm failures from 2af07e2f74.
Use GUC_ACTION_SAVE rather than GUC_ACTION_SET, necessary for working
with parallel query.

Now that the call requires more arguments, wrap the call in a new

2af07e2f74 Fix search_path to a safe value during maintenance operations.
While executing maintenance operations (ANALYZE, CLUSTER, REFRESH
MATERIALIZED VIEW, REINDEX, or VACUUM), set search_path to
'pg_catalog, pg_temp' to prevent inconsistent behavior.

Functions that are used for functional indexes, in index expressions,
or in materialized views and depend on a different search path must be
declared with CREATE FUNCTION ... SET search_path='...'.

This change was previously committed as 05e1737351, then reverted in
commit 2fcc7ee7af because it was too late in the cycle.

Preparation for the MAINTAIN privilege, which was previously reverted
due to search_path manipulation hazards.

dbbca2cf29 Remove unused #include's from backend .c files
as determined by include-what-you-use (IWYU)

While IWYU also suggests to *add* a bunch of #include's (which is its
main purpose), this patch does not do that.  In some cases, a more
specific #include replaces another less specific one.

Some manual adjustments of the automatic result:

- IWYU currently doesn't know about includes that provide global
  variable declarations (like -Wmissing-variable-declarations), so
  those includes are being kept manually.

- All includes for port(ability) headers are being kept for now, to
  play it safe.

- No changes of catalog/pg_foo.h to catalog/pg_foo_d.h, to keep the
  patch from exploding in size.

Note that this patch touches just *.c files, so nothing declared in
header files changes in hidden ways.

As a small example, in src/backend/access/transam/rmgr.c, some IWYU
pragma annotations are added to handle a special case there.

024c521117 Replace BackendIds with 0-based ProcNumbers
Now that BackendId was just another index into the proc array, it was
redundant with the 0-based proc numbers used in other places. Replace
all usage of backend IDs with proc numbers.

The only place where the term "backend id" remains is in a few pgstat
functions that expose backend IDs at the SQL level. Those IDs are now
in fact 0-based ProcNumbers too, but the documentation still calls
them "backend ids". That term still seems appropriate to describe what
the numbers are, so I let it be.

One user-visible effect is that pg_temp_0 is now a valid temp schema
name, for backend with ProcNumber 0.

46a0cd4cef Add temporal PRIMARY KEY and UNIQUE constraints
Add WITHOUT OVERLAPS clause to PRIMARY KEY and UNIQUE constraints.
These are backed by GiST indexes instead of B-tree indexes, since they
are essentially exclusion constraints with = for the scalar parts of
the key and && for the temporal part.

d43bd090a8 Improve some error messages with invalid indexes for REINDEX CONCURRENTLY
An invalid index is skipped when doing REINDEX CONCURRENTLY at table
level, with INDEX_CORRUPTED used as errcode.  This is confusing,
because an invalid index could exist after an interruption.  The errcode
is switched to OBJECT_NOT_IN_PREREQUISITE_STATE instead, as per a
suggestion from Andres Freund.

While on it, the error messages are reworded, and a hint is added,
telling how to rebuild an invalid index in this case.  This has been
suggested by Noah Misch.

c426f7c2b3 Fix assertion failure with REINDEX and event triggers
A REINDEX CONCURRENTLY run on a table with no indexes would always pop
the topmost snapshot from the active snapshot stack, making the snapshot
handling inconsistent between the multiple-relation and single-relation
cases.  This commit slightly changes the snapshot stack handling so as a
snapshot is popped only ReindexMultipleInternal() in this case after a
relation has been reindexed, fixing a problem where an event trigger
function may need a snapshot but does not have one.  This also keeps the
places where PopActiveSnapshot() is called closer to each other.

While on it, this expands the existing tests to cover all the cases that
could be faced with REINDEX commands and such event triggers, for one or
more relations, with or without indexes.

This behavior is inconsistent since 5dc92b844e68, but we've never had a
need for an active snapshot at the end of a REINDEX until now.

f21848de20 Add support for REINDEX in event triggers
This commit adds support for REINDEX in event triggers, making this
command react for the events ddl_command_start and ddl_command_end.  The
indexes rebuilt are collected with the ReindexStmt emitted by the
caller, for the concurrent and non-concurrent paths.

Thanks to that, it is possible to know a full list of the indexes that a
single REINDEX command has worked on.

a11c9c42ea Check collation when creating partitioned index
When creating a partitioned index, the partition key must be a subset
of the index's columns.  But this currently doesn't check that the
collations between the partition key and the index definition match.
So you can construct a unique index that fails to enforce uniqueness.
(This would most likely involve a nondeterministic collation, so it
would have to be crafted explicitly and is not something that would
just happen by accident.)

This patch adds the required collation check.  As a result, any
previously allowed unique index that has a collation mismatch would no
longer be allowed to be created.

743ddafc71 Ensure we preprocess expressions before checking their volatility.
contain_mutable_functions and contain_volatile_functions give
reliable answers only after expression preprocessing (specifically
eval_const_expressions).  Some places understand this, but some did
not get the memo --- which is not entirely their fault, because the
problem is documented only in places far away from those functions.
Introduce wrapper functions that allow doing the right thing easily,
and add commentary in hopes of preventing future mistakes from
copy-and-paste of code that's only conditionally safe.

Two actual bugs of this ilk are fixed here.  We failed to preprocess
column GENERATED expressions before checking mutability, so that the
code could fail to detect the use of a volatile function
default-argument expression, or it could reject a polymorphic function
that is actually immutable on the datatype of interest.  Likewise,
column DEFAULT expressions weren't preprocessed before determining if
it's safe to apply the attmissingval mechanism.  A false negative
would just result in an unnecessary table rewrite, but a false
positive could allow the attmissingval mechanism to be used in a case
where it should not be, resulting in unexpected initial values in a
new column.

In passing, re-order the steps in ComputePartitionAttrs so that its
checks for invalid column references are done before applying
expression_planner, rather than after.  The previous coding would
not complain if a partition expression contains a disallowed column
reference that gets optimized away by constant folding, which seems
to me to be a behavior we do not want.

Per bug #18097 from Jim Keener.  Back-patch to all supported versions.

7841623571 Remove IndexInfo.ii_OpclassOptions field
It is unnecessary to include this field in IndexInfo.  It is only used
by DDL code, not during execution.  It is really only used to pass
local information around between functions in index.c and indexcmds.c,
for which it is clearer to use local variables, like in similar cases.

8c852ba9a4 Allow some exclusion constraints on partitions
Previously we only allowed unique B-tree constraints on partitions
(and only if the constraint included all the partition keys).  But we
could allow exclusion constraints with the same restriction.  We also
require that those columns be compared for equality, not something
like &&.

This reverts the following commits: 4dbdb82513, c2122aae63,
5b1a879943, 9e1e9d6560, ff9618e82a, 60684dd834, 4441fc704d,
and b5d6382496.  A role with the MAINTAIN privilege may be able to
use search_path tricks to escalate privileges to the table owner.
Unfortunately, it is too late in the v16 development cycle to apply
the proposed fix, i.e., restricting search_path when running
maintenance commands.

cfc43aeb38 Fix marking of indisvalid for partitioned indexes at creation
The logic that introduced partitioned indexes missed a few things when
invalidating a partitioned index when these are created, still the code
is written to handle recursions:
1) If created from scratch because a mapping index could not be found,
the new index created could be itself invalid, if for example it was a
partitioned index with one of its leaves invalid.
2) A CCI was missing when indisvalid is set for a parent index, leading
to inconsistent trees when recursing across more than one level for a
partitioned index creation if an invalidation of the parent was
required.

This could lead to the creation of a partition index tree where some of
the partitioned indexes are marked as invalid, but some of the parents
are marked valid, which is not something that should happen (as
validatePartitionedIndex() defines, indisvalid is switched to true for a
partitioned index iff all its partitions are themselves valid).

This patch makes sure that indisvalid is set to false on a partitioned
index if at least one of its partition is invalid.  The flag is set to
true if *all* its partitions are valid.

The regression test added in this commit abuses of a failed concurrent
index creation, marked as invalid, that maps with an index created on
its partitioned table afterwards.

4dbdb82513 Fix cache lookup hazards introduced by ff9618e82a.
ff9618e82a introduced has_partition_ancestor_privs(), which is used
to check whether a user has MAINTAIN on any partition ancestors.
This involves syscache lookups, and presently this function does
not take any relation locks, so it is likely subject to the same
kind of cache lookup failures that were fixed by 19de0ab23c.

To fix this problem, this commit partially reverts ff9618e82a.
Specifically, it removes the partition-related changes, including
the has_partition_ancestor_privs() function mentioned above.  This
means that MAINTAIN on a partitioned table is no longer sufficient
to perform maintenance commands on its partitions.  This is more
like how privileges for maintenance commands work on supported
versions.  Privileges are checked for each partition, so a command
that flows down to all partitions might refuse to process them
(e.g., if the current user doesn't have MAINTAIN on the partition).

In passing, adjust a few related comments and error messages, and
add a test for the privilege checks for CLUSTER on a partitioned
table.

0245f8db36 Pre-beta mechanical code beautification.
Run pgindent, pgperltidy, and reformat-dat-files.

This set of diffs is a bit larger than typical.  We've updated to
pg_bsd_indent 2.1.2, which properly indents variable declarations that
have multi-line initialization expressions (the continuation lines are
now indented one tab stop).  We've also updated to perltidy version
20230309 and changed some of its settings, which reduces its desire to
add whitespace to lines to make assignments etc. line up.  Going
forward, that should make for fewer random-seeming changes to existing
code.

d435f15fff Add SysCacheGetAttrNotNull for guaranteed not-null attrs
When extracting an attr from a cached tuple in the syscache with
SysCacheGetAttr the isnull parameter must be checked in case the
attr cannot be NULL.  For cases when this is known beforehand, a
wrapper is introduced which perform the errorhandling internally
on behalf of the caller, invoking an elog in case of a NULL attr.

27f5c712b2 Fix CREATE INDEX progress reporting for multi-level partitioning.
The "partitions_total" and "partitions_done" fields were updated
as though the current level of partitioning was the only one.
In multi-level cases, not only could partitions_total change
over the course of the command, but partitions_done could go
backwards or exceed the currently-reported partitions_total.

Fix by setting partitions_total to the total number of direct
and indirect children once at command start, and then just
incrementing partitions_done at appropriate points.  Invent
a new progress monitoring function "pgstat_progress_incr_param"
to simplify doing the latter.  We can avoid adding cost for the
former when doing CREATE INDEX, because ProcessUtility already
enumerates the children and it's pretty easy to pass the count
down to DefineIndex.  In principle the same could be done in
ALTER TABLE, but that's structurally difficult; for now, just
eat the cost of an extra find_all_inheritors scan in that case.

19d8e2308b Ignore BRIN indexes when checking for HOT updates
When determining whether an index update may be skipped by using HOT, we
can ignore attributes indexed by block summarizing indexes without
references to individual tuples that need to be cleaned up.

A new type TU_UpdateIndexes provides a signal to the executor to
determine which indexes to update - no indexes, all indexes, or only the
summarizing indexes.

This also removes rd_indexattr list, and replaces it with rd_attrsvalid
flag. The list was not used anywhere, and a simple flag is sufficient.

This was originally committed as 5753d4ee32, but then got reverted by
e3fcca0d0d because of correctness issues.

ff9618e82a Fix MAINTAIN privileges for toast tables and partitions.
Commit 60684dd8 left loose ends when it came to maintaining toast
tables or partitions.

For toast tables, simply skip the privilege check if the toast table
is an indirect target of the maintenance command, because the main
table privileges have already been checked.

For partitions, allow the maintenance command if the user has the
MAINTAIN privilege on the partition or any parent.

Also make CLUSTER emit "skipping" messages when the user doesn't have
privileges, similar to VACUUM.

60684dd834 Add grantable MAINTAIN privilege and pg_maintain role.
Allows VACUUM, ANALYZE, REINDEX, REFRESH MATERIALIZED VIEW, CLUSTER,
and LOCK TABLE.

Effectively reverts 4441fc704d. Instead of creating separate
privileges for VACUUM, ANALYZE, and other maintenance commands, group
them together under a single MAINTAIN privilege.

Add 'missing_ok' argument to build_attrmap_by_name
When it's given as true, return a 0 in the position of the missing
column rather than raising an error.

This is currently unused, but it allows us to reimplement column
permission checking in a subsequent commit.  It seems worth breaking
into a separate commit because it affects unrelated code.

c727f511bd Refactor aclcheck functions
Instead of dozens of mostly-duplicate pg_foo_aclcheck() functions,
write one common function object_aclcheck() that can handle almost all
of them.  We already have all the information we need, such as which
system catalog corresponds to which catalog table and which column is
the ACL column.

There are a few pg_foo_aclcheck() that don't work via the generic
function and have special APIs, so those stay as is.

I also changed most pg_foo_aclmask() functions to static functions,
since they are not used outside of aclchk.c.

a601366a46 Harmonize more parameter names in bulk.
Make sure that function declarations use names that exactly match the
corresponding names from function definitions in optimizer, parser,
utility, libpq, and "commands" code, as well as in remaining library
code.  Do the same for all code related to frontend programs (with the
exception of pg_dump/pg_dumpall related code).

Like other recent commits that cleaned up function parameter names, this
commit was written with help from clang-tidy.  Later commits will handle
ecpg and pg_dump/pg_dumpall.

0a20ff54f5 Split up guc.c for better build speed and ease of maintenance.
guc.c has grown to be one of our largest .c files, making it
a bottleneck for compilation.  It's also acquired a bunch of
knowledge that'd be better kept elsewhere, because of our not
very good habit of putting variable-specific check hooks here.
Hence, split it up along these lines:

* guc.c itself retains just the core GUC housekeeping mechanisms.
* New file guc_funcs.c contains the SET/SHOW interfaces and some
  SQL-accessible functions for GUC manipulation.
* New file guc_tables.c contains the data arrays that define the
  built-in GUC variables, along with some already-exported constant
  tables.
* GUC check/assign/show hook functions are moved to the variable's
  home module, whenever that's clearly identifiable.  A few hard-
  to-classify hooks ended up in commands/variable.c, which was
  already a home for miscellaneous GUC hook functions.

To avoid cluttering a lot more header files with #include "guc.h",
I also invented a new header file utils/guc_hooks.h and put all
the GUC hook functions' declarations there, regardless of their
originating module.  That allowed removal of #include "guc.h"
from some existing headers.  The fallout from that (hopefully
all caught here) demonstrates clearly why such inclusions are
best minimized: there are a lot of files that, for example,
were getting array.h at two or more levels of remove, despite
not having any connection at all to GUCs in themselves.

There is some very minor code beautification here, such as
renaming a couple of inconsistently-named hook functions
and improving some comments.  But mostly this just moves
code from point A to point B and deals with the ensuing
needs for #include adjustments and exporting a few functions
that previously weren't exported.

Patch by me, per a suggestion from Andres Freund; thanks also
to Michael Paquier for the idea to invent guc_funcs.c.

421892a192 Further reduce warnings with -Wshadow=compatible-local
In a similar effort to f01592f91, here we're targetting fixing the
warnings that -Wshadow=compatible-local produces that we can fix by moving
a variable to an inner scope to stop that variable from being shadowed by
another variable declared somewhere later in the function.

All of the warnings being fixed here are changing the scope of variables
which are being used as an iterator for a "for" loop.  In each instance,
the fix happens to be changing the for loop to use the C99 type
initialization.  Much of this code likely pre-dates our use of C99.

Reducing the scope of the outer scoped variable seems like the safest way
to fix these.  Renaming seems more likely to risk patches using the wrong
variable.  Reducing the scope is more likely to result in a compilation
failure after applying some future patch rather than introducing bugs with
it.

By my count, this takes the warning count from 129 down to 114.

e6dbb48487 Fix subtly-incorrect matching of parent and child partitioned indexes.
When creating a partitioned index, DefineIndex tries to identify
any existing indexes on the partitions that match the partitioned
index, so that it can absorb those as child indexes instead of
building new ones.  Part of the matching is to compare IndexInfo
structs --- but that wasn't done quite right.  We're comparing
the IndexInfo built within DefineIndex itself to one made from
existing catalog contents by BuildIndexInfo.  Notably, while
BuildIndexInfo will run index expressions and predicates through
expression preprocessing, that has not happened to DefineIndex's
struct.  The result is failure to match and subsequent creation
of duplicate indexes.

The easiest and most bulletproof fix is to build a new IndexInfo
using BuildIndexInfo, thereby guaranteeing that the processing done
is identical.

While here, let's also extract the opfamily and collation data
from the new partitioned index, removing ad-hoc logic that
duplicated knowledge about how those are constructed.

Per report from Christophe Pettus.  Back-patch to v11 where
we invented partitioned indexes.

2cbc3c17a5 Rework logic and simplify syntax of REINDEX DATABASE/SYSTEM
Per discussion, this commit includes a couple of changes to these two
flavors of REINDEX:
* The grammar is changed to make the name of the object optional, hence
one can rebuild all the indexes of the wanted area by specifying only
"REINDEX DATABASE;" or "REINDEX SYSTEM;".  Previously, the object name
was mandatory and had to match the name of the database on which the
command is issued.
* REINDEX DATABASE is changed to ignore catalogs, making this task only
possible with REINDEX SYSTEM.  This is a historical change, but there
was no way to work only on the indexes of a database without touching
the catalogs.  We have discussed more approaches here, like the addition
of an option to skip the catalogs without changing the original
behavior, but concluded that what we have here is for the best.

This builds on top of the TAP tests introduced in 5fb5b6c, showing the
change in behavior for REINDEX SYSTEM.  reindexdb is updated so as we do
not issue an extra REINDEX SYSTEM when working on a database in the
non-concurrent case, something that was confusing when --concurrently
got introduced, so this simplifies the code.

b0a55e4329 Change internal RelFileNode references to RelFileNumber or RelFileLocator.
We have been using the term RelFileNode to refer to either (1) the
integer that is used to name the sequence of files for a certain relation
within the directory set aside for that tablespace/database combination;
or (2) that value plus the OIDs of the tablespace and database; or
occasionally (3) the whole series of files created for a relation
based on those values. Using the same name for more than one thing is
confusing.

Replace RelFileNode with RelFileNumber when we're talking about just the
single number, i.e. (1) from above, and with RelFileLocator when we're
talking about all the things that are needed to locate a relation's files
on disk, i.e. (2) from above. In the places where we refer to (3) as
a relfilenode, instead refer to "relation storage".

Since there is a ton of SQL code in the world that knows about
pg_class.relfilenode, don't change the name of that column, or of other
SQL-facing things that derive their name from it.

On the other hand, do adjust closely-related internal terminology. For
example, the structure member names dbNode and spcNode appear to be
derived from the fact that the structure itself was called RelFileNode,
so change those to dbOid and spcOid. Likewise, various variables with
names like rnode and relnode get renamed appropriately, according to
how they're being used in context.

Hopefully, this is clearer than before. It is also preparation for
future patches that intend to widen the relfilenumber fields from its
current width of 32 bits. Variables that store a relfilenumber are now
declared as type RelFileNumber rather than type Oid; right now, these
are the same, but that can now more easily be changed.

00377b9a02 CREATE INDEX: use the original userid for more ACL checks.
Commit a117cebd638dd02e5c2e791c25e43745f233111b used the original userid
for ACL checks located directly in DefineIndex(), but it still adopted
the table owner userid for more ACL checks than intended.  That broke
dump/reload of indexes that refer to an operator class, collation, or
exclusion operator in a schema other than "public" or "pg_catalog".
Back-patch to v10 (all supported versions), like the earlier commit.

a117cebd63 Make relation-enumerating operations be security-restricted operations.
When a feature enumerates relations and runs functions associated with
all found relations, the feature's user shall not need to trust every
user having permission to create objects.  BRIN-specific functionality
in autovacuum neglected to account for this, as did pg_amcheck and
CLUSTER.  An attacker having permission to create non-temp objects in at
least one schema could execute arbitrary SQL functions under the
identity of the bootstrap superuser.  CREATE INDEX (not a
relation-enumerating operation) and REINDEX protected themselves too
late.  This change extends to the non-enumerating amcheck interface.
Back-patch to v10 (all supported versions).
