"Arrows" in widgets are those GUI elements mostly represented by
triangles pointing in a particular direction as in scrollbars,
choice widgets, some menus, valuators and Fl_Counter widgets.
The code has been simplified and standardized such that all these
GUI elements are drawn identically per FLTK scheme.
Widget authors no longer need to write code to calculate arrow sizes
and draw polygons etc.
Different schemes can and do implement different drawing functions.

- remove obsolete svn '$Id$' tags from all source files
- update .fl files and generated files accordingly
- replace 'http://www.fltk.org' URL's with 'https://...'
- replace bug report URL 'str.php' with 'bugs.php'
- remove trailing whitespace
- fix other whitespace errors flagged by Git
- add and/or fix missing or wrong standard headers
- convert tabs to spaces in all source files
The only relevant code changes are in the fluid/ folder where
some .fl files and other source files were used to generate
the '$Id' headers and footers.
Previously Up/Down keys worked only if one of the buttons was pressed
before so that it had the focus.
test/valuators.fl: The second Fl_Spinner widget (FL_FLOAT_INPUT) now has
wrap mode disabled whereas the first one (FL_INT_INPUT) uses wrap mode
(default, compatible with FLTK 1.3.x and older).

Everything related to Fl_Tree and its test and demo programs has been
cleaned.
Documentation needs updates.

This does not fully apply the FLTK coding standard, but makes it more
consistent in that indentation is done with tabs as much as possible.

Update FLUID dependencies.
The # copies spinner was just a bit too small for 100 copies.
Didn't set the menu divider if there were exactly 10 files in the history.
Add documentation for Fl_Spinner.
