[Coverity] Restore std::cout flags and MimeAttr ctor. inits

* Restore std::cout flags (CID11245, CID11246) after using hexadecimal
  and zero-filled flags;
* Fix uninit MimeAttribute::fStatus field in one ctor. CID11292.
