- Pending async I/O was tossed on unconfigure (should not happen, but..) - It could exhaust memory under heavy I/O load. - If memory allocation failed, disk transfers could stall. - v_numoutput was updated without v_interlock held. Additionally: - Make it MPSAFE. - Use kmem_alloc().
