yamt f5ff7e4897 cleanup user.h. 
- remove several #include which are not directly related to
  this header anymore.  tweak *.c accordingly.
- update comments.
- move some !_KERNEL #include to proc.h because it's more appropriate
  place these days.
- whitespace.
