thorpej fc96443d15 New callout mechanism with two major improvements over the old 
timeout()/untimeout() API:
- Clients supply callout handle storage, thus eliminating problems of
  resource allocation.
- Insertion and removal of callouts is constant time, important as
  this facility is used quite a lot in the kernel.

The old timeout()/untimeout() API has been removed from the kernel.

matt 7b17d59536 Revamp the VAX interrupt handling code. Make it compatible with all the 
other ports.  Clean up some things.

ragge a2692d780b Fix map register/DMA wait queues. 
Still to do:
	BDP handling. Currently missing.
	Ubareset's won't work at all.
