eliminate the PMAP_NEW option by making it required for all ports.
ports which previously had no support for PMAP_NEW now implement
the pmap_k* interfaces as wrappers around the non-k versions.

Change the pmap_extract() interface to:
boolean_t pmap_extract(pmap_t, vaddr_t, paddr_t *);
This makes it possible for the pmap to map physical address 0.
