in vinvalbuf(), vtruncbuf() and vflushbuf(), don't skip calling
...
VOP_PUTPAGES() just because the vnode has no pages. layered filesystems
will want to pass these calls on through to the underlying filesystem,
and non-layered filesystems may need to remove the vnode from the
syncer queues. fix up MP locking and add some locking assertions.
fixes PRs 12284 and 14640.

* move the code that supplies the integer and FP registers into
separate functions, rather than duplicating the code for the
ptrace and core file cases.
* Use supply_register() rather than just copying directly into
the register array and calling registers_fetched(). This way,
only the registers actually supplied are marked as valid within
the debugger.
* Add support for SSE/SSE2 registers via the PT_{GET,SET}XMMREGS
ptrace(2) request.
(Blocked on the FSF assignment clerk for feeding this back to
the master GDB sources.)

Replace nfs_hash() (with its extremely bad hash) with a macro to call

hash32_buf() to obtain a 32 bit hash. On some tests I ran I obtained
a 30x improvement in hash distribution and a 6x reduction in time spent
in nfs_nget().
