"off_t" and the return value is a "paddr_t" to allow mappings
at offsets past 2^31 bytes. Somewhat inspired by FreeBSD, which
only changed the offset to a "vm_offset_t".
Includes updates for the i386, pc532 and sh3 mmmmap from Jason Thorpe.
- returned EOPNOTSUPP rather than -1.
- no check for negative offset.
many of these fix potential security problems in these drivers.

- split softc size and match/attach out from cfdriver into
a new struct cfattach.
- new "attach" directive for files.*. May specify the name of
the cfattach structure, so that devices may be easily attached
to parents with different autoconfiguration semantics.
of bwtwo, cgfour, cgsix, and cgeight. Includes support for attaching
the overlay plane of the cgfour and cgeight as a bwtwo instance (a'la
SunOS).
P4 register descriptions, cgfour driver, and cgeight driver partially
derived from OpenBSD.
