In fdioctl(), allocate fd_formb dynamically when needed, rather than on

the stack, and remove the no-longer-necessary PHOLD()/PRELE() calls
in fdformat().
(This eliminates 1/3 of the instances of PHOLD()/PRELE() in the kernel code.)
