Add a new ptrace request, PT_IO, as proposed by Artur Grabowski

on the <bsd-api-discuss@wasabisystems.com> mailing list. PT_IO
is a more general inferior I/D space I/O mechanism. FreeBSD and
OpenBSD have also added PT_IO.
