eliminate the PMAP_NEW option by making it required for all ports.
ports which previously had no support for PMAP_NEW now implement
the pmap_k* interfaces as wrappers around the non-k versions.

Added support for VMEbus RAM cards via MACHINE_NONCONTIG.
The kernel should still compile and run without it, but I've
put the config option in std.mvme68k so it's built like this
as standard.
