Purge CLSIZE, CLSIZELOG2 and MCLOFSET.

Be consistant in the way that MSIZE, MCLSHIFT, MCLBYTES and NMBCLUSTERS
are defined.
Remove old VM constants from cesfic port.
Bump MSIZE to 256 on mipsco (the only one that wasn't already 256).

For MIPS kernel Makefiles, don't set ENDIAN in std.${MACHINE}. Instead,

explicitly set MACHINE_ARCH to the appropriate thing. Makefile.mips will
then set all of the internal variables it needs to accordingly.
