Convert ioctl code to use EPASSTHROUGH instead of -1 or ENOTTY for

indicating an unhandled "command". ERESTART is -1, which can lead to
confusion. ERESTART has been moved to -3 and EPASSTHROUGH has been
placed at -4. No ioctl code should now return -1 anywhere. The
ioctl() system call is now properly restartable.

Rename ISDN devices, per discussion on tech-kern. The network devices

become ippp (ISDN ppp) and irip (ISDN raw IP). The character device now
are called: /dev/isdn (isdnd <-> kernel communication), /dev/isdnctl (dialing
and other control), /dev/isdntrc* (tracing), /dev/isdnbchan* (raw B channel
access, i.e. for user land PPP) and /dev/isdntel* (telephone devices, i.e.
for answering machines).

When enabling hardware interrupts at the end of cpu_startup(), splhigh()

cannot be used to mask them off for deferred processing as autoconfiguration
hasn't even started and thus the IPL_HIGH mask hasn't been calculated yet.
Instead, set the current mask manually to all-masked using splraise().

Add new RF_* options for RAIDframe bits that are no longer built by default.

While we're here, enable RAIDframe (and RAID_AUTOCONFIG) by default for
architectures that I'm comfortable can deal with it being on by default.
Also: bump the number of 'raid' devices from 4 to 8, since 4 seems to
be insufficient in practise.

Overhaul of the ISA autoconfiguration code to support direct

configuration of devices logically attached to the ISA bus:
* Change the isa_attach_args to have arrays of io, mem, irq, drq
resources.
* Add a "pnpnames" and a linked list of "pnpcompatnames" to the
isa_attach_args. If either of these members are non-NULL,
direct configuration of the bus is being performed. Add an
ISA_DIRECT_CONFIG() macro to test for this.
* Drivers are not allowed to modify the isa_attach_args unless
direct configuration is not being performed and the probe fucntion
is returning success.
* Adapt device drivers -- currently, all driver probe routines return
"no match" if ISA_DIRECT_CONFIG() evaluates to true.
