Split /dev/fd/* from std entry. Install floppy does not require them.

Try to make the install floppy work on 4MB machines:
...
- reduce memory disk size
- use x_dd, x_ifconfig and x_route
- use libhack.o (sync with i386)
- don't use mfs as /tmp
Tested by Luigi Filippini <filippini@tiscali.it> and me.

- rename (revert) normal memory disk device name /dev/mdX -> /dev/mdXa
...
- now raw partitions are used for memory disk ioctls
