Merge from vmlocking:
- Split vnode::v_flag into three fields, depending on field locking.
- simple_lock -> kmutex in a few places.
- Fix some simple locking problems.

Be more efficient with the hash tables for the buffer and vnode caches.
Note that roll-forward can add more inodes to the filesystem; don't overflow
the tables but reallocate them.
