document /usr/lkm

place /dev/fd and /dev/null into alphabetical order
alphabetize and individually document /etc/master.passwd, /etc/passwd,
  /etc/pwd.db, /etc/spwd.db

Partial fix for PR misc/19609 by Igor Sobrado
