Remove the _UCONTEXT_TO_SIGCONTEXT() and _SIGCONTEXT_TO_UCONTEXT() macros
as nothing uses them and they wouldn't work as _MCONTEXT_TO_SIGCONTEXT() and _SIGCONTEXT_TO_MCONTEXT() got deleted a while ago.
