Consistently arrange order of definitions for each type. Specifically:

* move SLIST_EMPTY, SLIST_FIRST, SLIST_NEXT, SLIST_FOREACH further down.
* remove duplicate definition of SLIST_NEXT.
* move TAILQ_FOREACH and TAILQ_FOREACH_REVERSE further up.
* add more comments.
* tweak whitespace.
