Remove lint inside #ifdef DEBUG:

* fix pmap debugging format strings: "%x" -> "%lx" for vm_offset_t
         "%x" -> "%p" for pointers.
  * add prottypes for pmap dumping functions
  * add missing return types on pmap dumping functions
  * move initialization of pamp_enter_pv():pv  before #ifdef DEBUG code
    which uses pv.
